This study describes a case of a lymphoepithelial cyst ith uncommon localization anterior to the anterior aucial pillar but not into the tonsil fossa. The proximity f the lesion to the area of the second branchial cleft and he existence of a duct to the anterior faucial pillar onstitute a very rare finding. The histologic examinaion and the anatomic relations of the cyst support the ypothesis of second branchial cleft origin.

Bilateral mandibular dentigerous cysts: a case report

Dentigerous cysts are frequently found in the maxilla. After radicular cysts, dentigerous cysts are those most commonly diagnosed, accounting for 20% of all jaw cysts. They are often asymptomatic and diagnosed incidentally during routine examinations. Clinical complications such as dental displacement, ectopic eruption, dental impaction, adjacent tooth root resorption, cortical expansion with facial asymmetry, paresthesia, pathological fracture, and even malignant transformation may occur. Despite these classical features, definitive diagnosis must always be based on histological examination. Most dentigerous cysts are solitary. The aim of this article is to report a case of bilateral mandibular dentigerous cysts in a non-syndromic patient and, through a literature review, present the available treatment modalities used successfully in this case.

Comparative Analysis of the Effectiveness of the Topical Administration of Benzocaine and EMLA® on Oral Pain and Tactile Sensitivity

Objectives To compare the effectiveness of the topical administration of benzocaine and EMLA on oral pain and tactile sensitivity. Materials and Methods A randomized, double-blind, split-mouth clinical trial was carried out with 20 volunteers. The sensorial and quantitative tests were applied before the contact with topical anesthetic and after the application. Results In the superficial tactile perception test, when we compared each group singly, there were statistically significant values in the decrease of superficial tactile perception when compared to the moment prior to the application of anesthetic agents. For the sensitivity to mechanical pain, no statistical significant difference was observed at evaluated times. In the needle penetration test, in an intergroup analysis, we found a decrease in the pain sensitivity to needle penetration at 5 min (p=0.053) and at 10 min (p=0.019) after the contact of the anesthetic drug with the oral mucosa. Conclusion The application of topical anesthetic drugs reduces the discomfort associated with this procedure, mainly until the first 10 minutes. Only the needle penetration sensitivity test showed sufficient sensitivity to reveal a difference in the anesthetic effect between EMLA and benzocaine. This trial is registered with RBR-2N2GSW.

Ankylosis Due Sequel Of Fracture Of The Mandibular Condyle: Case Report

The Ankylosis of the Temporomandibular joint (TMJ) is a disorder of craniofacial complex that results in the merger between the condyle and the mandibular fossa, causing partial or complete immobilization of the mandible. The etiological factors are local and systemic inflammation, infection in the area of TMJ, rheumatic diseases and neoplasms, having the trauma as the main etiologic factor. The traumas are responsible for 31% to 98% of cases of ankylosis. The diagnosis is made from the anamnesis and imaging scans (computed tomography) pointing to the union of joint components. The treatment of ankylosis is a big challenge due the high rate of recurrence that can be affected by factors such as type of ankylosis, surgical technique, age of the patient, post-operative physiotherapy and systematic follow-up of the patient. The various forms of treatment require careful analysis of type of ankylosis if it is intra or extra-articular, unilateral or bilateral and if it is bony or fibrous, There is no consensus in current literature regarding the best treatment. The aim of this work is to present through the report of a clinical case, a surgical treatment of Unilateral Temporomandibular joint Ankylosis, due to sequel of condylar fracture with re-establishment of the stomatognathic functions in postoperative follow-up.

TUMOR ODONTOGÊNICOQUERATOCÍSTICO: REVISÃO DE LITERATURA

RESUMO O tumor odontogenicoqueratocistico (TOQ) e uma neoplasia odontogenica benigna, apesar do comportamento agressivo e da elevada recidiva. Estudos recentes associam a patogenese e o desenvolvimento do TOQ ao processo de transicao epitelio-mesenquimal, em que ocorrem alteracoes de coesao, polarizacao e mobilidade das celulas do epitelio oral.As caracteristicas radiograficas mostram, em alguns estudos, a presenca de uma area radiolucida afetando grande parte da regiao na qual o tumor esta inserido, podendo ou nao envolver remanescente dental. Este trabalho consiste de uma revisao da literatura que tem como objetivo levantar as caracteristicas gerais do TOQ, enfatizando seu diagnostico, tratamento, incidencia e prevalencia, recidiva e localizacao. Para compor um diagnostico correto e preciso e necessario realizar um exame histopatologico que mostre a presenca de celulas do epitelio escamoso e estratificado orto ou paraqueratinizado. Em relacao ao tratamento foi observado na literatura que o TOQ apresenta altas taxas de recidiva e que de acordo com o tipo de tratamento escolhido essa taxa pode variar sendo menor quando tratada por opcoes terapeuticas mais agressivas.O tumor odontogenicoqueratocistico e uma lesao de discreta sintomatologia, porem potencialmente agressivo. O tratamento deve ser norteado por uma tecnica cirurgica que ocasione o minimo de morbidade ao paciente e com efetivo controle de recidiva. Simplification on the Reduction of Comminuted Mandibular Fractures for Stable Internal Fixation

Background: The fractures of the comminuted type have a prevalence of 30 to 50% when related to the ones affecting the mandibular bone. They are characterized by the presence of multiple bone fragments involving several lines of fracture, resulting in small fragments within the same area. Usually resulting from high-energy trauma, they cause large displacements, tooth loss, as well as associated lesions in soft tissues. Cases Reports : This study aimed to report two cases addressing the simplification method in the reduction of comminuted fractures treated by the method of open reduction and functionally stable fixation, which emphasized the importance of establishing a sequence of reduction maneuvers and application of osteosynthesis for a functional, occlusal and aesthetic result of the lower facial third. Conclusion: The simplification of mandibular comminuted fractures proves to be an excellent ally when one intends to reduce difficulties of reduction of the fracture and the dental occlusion on the transoperative, as well as to facilitate the application of 2.4 reconstruction plates.

Pediatric Facial Fractures: Case Series and Critical Review

Background : The purpose of the present study is to report twenty cases of children with facial fractures admitted to the department of Oral and Maxillofacial Surgery at Hospital da Restauracao/FUSAM – Recife/Brazil and review the English medical literature. Methods and Findings : A retrospective case series study was developed. A total of 20 fractures were treated. Male was the main gender with average age of 8.7 years old. Motor vehicle accident was the main cause of injury (35%). Mandible fracture was the most common type of fracture (80%) and it was unilateral in 75% of cases. The sites most often affected were the symphysis (44%) and condyle (38%) Conclusion : Facial fractures in pediatric patients present important repercussions of functional and aesthetics nature. The socioeconomic and environmental aspects directly influence the incidence of facial trauma in this period of life. The parental supervision directly affects the etiologic factors (fall and motor vehicle accidents) and can be used as a factor to prevent these conditions.

Oroantral fistulas closure using Bichat's fat pad

The oroantral fistula is one of the complications most common after dental extractions in posterior maxillary, mainly in the region of second and third molar. The diagnosis is based on clinical signs and symptoms, which may be present in pain, fever, hyposmia and drainage of purulent discharge; additional examinations such as computed tomography and sinus nasal endoscopy improve diagnostic precision. It is therefore of paramount importance to correct early diagnosis and treatment avoiding in this way, the symptons of sinusitis, infections and dysphonia. Several surgical techniques have been proposed for the closure of the bucosinusal fistula; the use of Bichat´s fat pad has become a successful alternative among them. Many surgical treatments have been to propose for closure of bucosinusal fistula; Bichat fat pad has been a good alternative. The propose for this article is report two cases with closure of bucosinusal fistulas through Bichats fat pad, as well as to discuss the surgical techniques, characteristics, indications and peculiarities.
