Preliminary results from a phase I/II study of perillyl alcohol intranasal administration in adults with recurrent malignant gliomas

BACKGROUND Activation of the p21-ras signaling pathway from aberrantly expressed receptors promotes the growth of malignant human astrocytomas. Perillyl alcohol has shown to have both chemopreventive and chemotherapeutic activities in preclinical studies. The underlying action mechanism(s) of POH has yet to be delineated but may involve effects on the TGF-beta and/or the Ras signaling pathways. The intranasal delivery allows drugs that do not cross the BBB to enter the CNS; moreover, it eliminates the need for systemic delivery, thereby reducing unwanted systemic side effects. METHODS We are conducting a phase I/II study to evaluate the antitumoral activity of POH intranasal delivery in a 4x daily schedule in patients with recurrent MG. The objective was to determine PFS at 6 months and the safety for POH in adult patients who failed conventional treatment. Assessments were performed every 27 days. Thirty-seven patients with progressive disease after prior surgery, radiotherapy, and at least temozolomide-based chemotherapy were enrolled, 29 of whom had GBM, 5 who had anaplastic astrocytoma, and 3 had AO. RESULTS One patient (3.4%) with GBM and 1 patient (33.3%) with AO achieved partial response; 13 patients (44.8%) with GBM, 3 patients (60%) with AA, and 1 (33.3%) with AO achieved stable disease; 15 (51.7%) patients with GBM, 2 (40%) patients with AA, and 1 (33.3%) with AO showed progressive disease. Progression-free survival (partial response and stable disease) was 48.2% for patients with GBM, 60% for patients with AA, and 66.6% for patients with AO. CONCLUSIONS There were no toxicity events. Perillyl alcohol is well tolerated and regression of tumor size in some patients is suggestive of antitumor activity. This work discusses POH intranasal delivery as a potential adjuvant therapeutic strategy for patients with malignant gliomas.

Influence of papain in biofilm formed by methicillin-resistant Staphylococcus epidermidis and methicillin-resistant Staphylococcus haemolyticus isolates

Methicillin-resistant Staphylococcus epidermidis (MRSE) and methicillin-resistant Staphylococcus haemolyticus (MRSHa) are important coagulase-negative staphylococci. They are often isolated from bacteremia in humans mainly due to their ability to form biofilm on the surfaces of medical devices. Papain is a complex mixture of proteolytic enzymes and peroxidases extracted from the latex of Carica papaya and it is recognized by accelerating the healing process of wounds. This study aimed to evaluate the ability of the MRSE and MRSHa isolates to produce biofilms. Besides this, the ability of papain to inhibit the formation of biofilms or to disrupt the ones already formed by those bacteria was analyzed. Thirty MRSHa and 30 MRSE were isolated from bacteremia and used in this study. It was observed that papain has ability to reduce biofilms formed by MRSE (p < 0.06) and by MRSHa (p = 0.0005). In addition, papain was able to disrupt mature biofilms made by MRSE (p = 0.014). No antibacterial activity of papain was observed for any isolates of MRSE and MRSHa tested. Papain has been demonstrated as a potential product for reducing biofilm.

Effectiveness of papain gel in venous ulcer treatment: randomized clinical trial

Life cycle thinking in graduate education: an experience from Brazil

PurposeThis study presents an educational experience, of a Brazilian center of technological education, focused on the involvement of undergraduate and graduate students, according to directions of UNEP/SETAC Life Cycle Initiative. It describes an institutional collaborative work, aligned with the Life Cycle Initiative, and evaluates the main actions implemented through a set of indicators, which show the organizational dynamics and changes in the professional profiles of students, between 2006 and 2012.MethodsThe description of the institutional collaborative work was based on direct observations conducted by the authors, who participated in the experience, and on a documentary research involving data from academic system. This effort led to representations of (a) key interactions among actors of organizations and (b) a set of 11 indicators of changes for individuals and organizations. Interviews with students and representatives of organizations also contributed to generate the structure of the network and to fulfill the set of indicators.Results and discussionAmong the research findings, two were considered more relevant from the direct observation: (a) the structuring of an effective laboratory supported by public funds to conduct the educational experience and (b) the introduction of life cycle thinking (LCT), life cycle management (LCM), and the methodology of life cycle assessment (LCA) as optional areas of study for students. The offer of regular classes, supported by software, and the organization of small-scale research projects made the work feasible, altogether, improving the interest and cooperation of different graduate and undergraduate student groups. This outcome increased the interactions with other public and private organizations; much additional research was accomplished.ConclusionsThe experience contributed to materialize the LCT, LCM, and LCA themes and persuades the actors to reflect on new reflections concerning the necessity of changes in their professional profiles. This suggests a potential to produce organizational improvements and innovations. However, some challenges must be faced in Brazil: organization of the national inventory, improvement of academic knowledge and more engagement of scholars, as well as the lack of guidance and assistance to consolidate local and national collaborative work.

Evaluation of the profile of drug therapy administered through enteral feeding tube in a general hospital in Rio de Janeiro

Enteral nutrition (EN) is the method of choice for patients that cannot adequately receive oral feeding despite good gastrointestinal tract condition. Enteral diets may be administered through tube or ostomy placed in the stomach, duodenum or jejunum. The administration of drugs via enteral feeding tube (EFT) is a common practice in hospitals due to patient clinical status, and requires special attention from professionals involved in this process. This study entailed an analysis of the profile of drug therapy through EFT based on evaluation of medical prescriptions of the Medical Clinic of the Hospital dos Servidores do Estado (HSE) of Rio de Janeiro sent to the Pharmacy Service between January and June 2007, according to standard protocols in place. Prescription of drugs via EFT outside recommended guidelines was observed, besides potential drug-nutrient incompatibilities associated with this practice. These results point to the need for improvement of enteral route access and the adoption of measures to promote safe and effective use of drugs and nutritional therapy.

Variables were analyzed by Wilcoxon and McNemar test (p < 0.05). RESULTS Most participants were female; aged between 51 and 59 years; obese; with hypertension. Regarding ulcers, there was an average decrease of 7.9 cm2 (50% of its original size) in 90 days; 20% of the ulcers completely healed within 56.67 days. There was an increase in epithelialization, significant reduction in the slough and edema, improved depth, in the type and amount of exudate (p < 0.0001). The Antifungal Activity of Naphthoquinones: An Integrative Review

Naphthoquinones are the most commonly occurring type of quinones in nature. They are a diverse family of secondary metabolites that occur naturally in plants, lichens and various microorganisms. This subgroup is constantly being expanded through the discovery of new natural products and by the synthesis of new compounds via innovative techniques. Interest in quinones and the search for new biological activities within the members of this class have intensified in recent years, as evidenced by the evaluation of the potential antimicrobial activities of quinones. Among fungi of medical interest, yeasts of the genus Candida are of extreme importance due to their high frequency of colonization and infection in humans. The objective of this review is to describe the development of naphthoquinones as antifungals for the treatment of Candida species and to note the most promising compounds. By using certain criteria for selection of publications, 68 reports involving both synthetic and natural naphthoquinones are discussed. The activities of a large number of substances were evaluated against Candida albicans as well as against 7 other species of the genus Candida. The results discussed in this review allowed the identification of 30 naphthoquinones with higher antifungal activities than those of the currently used drugs.

Magistral drugs in hospitalized newborns and children.

Abstract Objective: Study the use of magistral oral solutions and suspensions in infants and children at a university hospital. Methods: This is a descriptive study based on the analysis of the assessed hospitals magistral drug request forms regarding the patients in the neonatal ICU, Obstetrics, Pediatrics and Pediatric Emergency from January 2012 to December 2013. The frequency of drug requests and dispensation was evaluated and the consumption of each active ingredient of the preparations was expressed as number of “infant defined daily dose” (iDDD) and of iDDD/100 bed-days. Results: A total of 657 forms were analyzed - a monthly average of 27 pediatric preparations. The neonatal ICU accounted for 69.6% of these requests. Twenty-one drug items were used, of which the most common were folinic acid (88 requests), sulfadiazine (85) and captopril (73). The consumption of the active principle in these preparations varied in number of iDDD, from 7.5 (hydralazine) to 16,520.0 (folic acid), and in number of iDDD/100 bed-days in the neonatal ICU, from 0.1 (zinc sulfate) to 146.1 (folic acid). Conclusions: The constant consumption of magistral oral solutions and suspensions by newborns and children of the assessed hospital indicates the need for such preparations as a pediatric therapeutic alternative in this hospital.
