Lipoma arborescens; successfully treated by yttrium-90 radiosynovectomy

Although radiosynovectomy (RS) applications have been carried out for many years, clinical indications of this non-invasive procedure is though to be limited probably due to the lack of information of clinicians. Clinicians’ preferential indication for RS is the treatment-resistant synovitis of individual joints, i.e. despite systemic pharmacotherapy and intra-articular steroid injections. We present here a case of “lipoma arborescens” treated by yttrium-90, which is a rare intra-articular lesion characterized by villous proliferation of the synovial membrane and hyperplasia of subsynovial fat. The results of clinical, biochemical and hematological examinations, magnetic resonance (MR) imaging, arthroscopy and histological analysis have shown that the etiology was lipoma arborescens in a female patient, aged 36 having swelling and sometimes associating pain at her right knee for 4 years. We have applied to our patient’s right knee RS with 185 MBq yttrium-90 colloid together with 40 mg of methylprednisolone acetate, although in our literature survey we have not met any similar case being treated with such indication. Even a year after the application, the patient has absolutely benefited from the treatment clinically, and this was also confirmed by comparative MR images (pre- and post-treatment). Consequently, we consider that Y-90 treatment might be applicable in suitable cases with lipoma arborescens.

Carpal tunnel syndrome leads to significant bone loss in metacarpal bones

Abstract. The present study was designed to determine bone density modifications at the forearm and metacarpal bones in patients with carpal tunnel syndrome (CTS). Bone mineral density (BMD) was measured by dual-energy X-ray absorptiometry at the one-third distal end and for the total of the radius–ulna, together with the third and fourth metacarpal bones, in 48 clinically and electrophysiologically diagnosed (18 unilateral and 15 bilateral) affected extremities in 33 premenopausal women (mean age 38.9 ± 6.5 years) with CTS. BMD values for non-affected extremities were used as controls for comparison with affected extremities. Bone mass was decreased approximately 7% in the forearm region (P < 0.02) and 18% in metacarpal bones (P < 0.01) of the thenar atrophy associated group compared with controls. A significant correlation was observed between disease duration (mean duration 3.2 ± 2.7 years) and the decrease in metacarpal bone density (r = 0.43; P = 0.004). This is the first clinical report of quantified bone loss in affected extremities in patients with CTS, and the results suggest the need for further studies to assess the clinical significance and morbidity of this pathology, especially in patients with thenar atrophy.

Effects of high-frequency bio-oxidative ozone therapy in temporomandibular disorder-related pain.

Objective: It was the aim of this study to compare the efficacy of ozone therapy and drug treatment in patients with painful temporomandibular joint (TMJ) disorder (TMD). Subjects and Methods: A total of 63 patients with TMD were enrolled; 33 were treated with bio-oxidative therapy and 30 with a ketoprofen tablet thiocolchicoside capsule 2 × 1 for 7 days. Maximum voluntary interincisal mouth opening (MMO) was measured in millimeters using a scale and recorded during the pre- and posttreatment periods. The patients evaluated their subjective pain using a visual analogue scale (VAS). Data were analyzed using the Mann-Whitney U test, the Kolmogorov-Smirnov test, and the independent t test. Results: The mean MMO of the group that received ozone therapy during the pretreatment period was 46.51 ± 8.2 mm, and it immediately increased to 48.78 ± 7.5 mm after 1 week of ozone therapy, which was statistically significant (p = 0.04). For those who received medication, the mean MMO during the pretreatment period was 46.30 mm, and at the end of 1 week it was 46.9 mm. In the ozone group, 29% of patients showed a gradual decrease in their VAS pain scores compared to pretreatment values (6.3 ± 2.1 to 3.0 ± 2.2). In the medication group, 24% of patients showed a significant decrease in VAS pain scores during the follow-up period (6.9 ± 1.4 to 5.0 ± 1.5). Conclusion: This study showed that bio-oxidative therapy was a more effective treatment than medication therapy for relieving TMJ pain.

A longitudinal study on lung disease in dental technicians: What has changed after seven years?

ObjectivesThe aim of this 7-year follow-up study was to determine respiratory changes in dental technicians.Material and MethodsIn our region, in the year 2005, 36 dental technicians were evaluated with a cross-sectional study on respiratory occupational diseases, and in 2012 we evaluated them again. Inclusion of information on respiratory symptoms and demographic features questionnaires was applied. Pulmonary function tests (PFT) were performed. Chest X-rays (CXR) were evaluated according to the ILO-2000 classification. For the comparisons of the technicians’ findings in 2005 and 2012, data analyses were performed with the Wilcoxon test in addition to descriptive statistical procedures.ResultsIn 2012, 19 out of the 36 technicians continued to work in the same place, so we were able to evaluate their findings. The prevalence of respiratory symptoms in dental technician was as follows: dyspnea 7 (37%), cough 6 (32%), and phlegm 5 (26%). According to ILO classifications in 2005, among the 36 technicians, 5 (13.8%) had pneumoconiosis. At the end of 7 years, there were 9 pneumoconiosis cases among the 19 remaining technicians (47%). Thus, there was a statistically significant progression on the profusion of the radiologic findings (p < 0.005). Also there was a significant worsening on spirometric findings (p < 0.05).ConclusionIn dental technicians, a determination of both radiologic and functional progressions at the end of 7 years demonstrate that the primary and secondary preventive measures are necessary for these workplaces. Workplaces must be regularly controlled for worker health and hygiene.

The Effects of Er:YAG, Nd:YAG, and Ho:YAG Laser Surface Treatments to Acrylic Resin Denture Bases on the Tensile Bond Strength of Silicone-Based Resilient Liners.

OBJECTIVE The present study was to assess the effect of surface treatments of Er:YAG, Nd:YAG, and Ho:YAG lasers on the tensile bond strength of a silicone-based resilient liner to an acrylic denture in an in vitro setting. METHODS Experimental dumbbell-shaped specimens (75 mm) were produced by combining two acrylate pieces fabricated from heat-polymerized acrylic resin (36 mm) with 3 mm of Molloplast(®)-B filling between them. The specimens (n=200) were randomly divided in half for thermocycling, and each 100 specimen set was randomized into five groups (n=20) with different surface treatments: control (no surface treatment), sandblasting, Er:YAG laser, Nd:YAG laser, and Ho:YAG laser. A tensile bond strength test was performed. The effect of the laser surface treatments was examined with scanning electron microscopy. RESULTS Only the Er:YAG laser increased the tensile bond strength compared with the other treatments. The other laser groups showed lower bond strengths. The Ho:YAG laser resulted in considerably reduced tensile bond strength. The scanning electron microscopy images showed that applying laser surface treatments modified the surface of the denture base resin. CONCLUSIONS There was not an overall improvement with the use of the studied laser modalities in the adhesion quality of resilient denture liner to acrylic resin, although Er:YAG laser showed a potential to improve their adhesion. These laser modalities need to be subjected to further studies to determine optimal setup for use in prosthodontics.

The effects of therapeutic x-ray doses on mechanical, chemical and physical properties of poly methyl methacrylate.

AIM The aim of this study was to investigate the effect of radiation doses very close to the human dose for oral cancers on mechanical, chemical and physical properties for poly methyl-methacrylate (PMMA). METHODS PMMA samples were divided into four different groups: no irradiated group, 25-Gy irradiated group, 50-Gy irradiated group and 75-Gy irradiated group. Each group contained nine samples. After 24 h, a three-point loading test was applied to each PMMA groups. The transverse strength and the elastic modulus were calculated using the test results. The results were analyzed statistically by using one-way analysis of variance. The structural characterizations of the PMMA samples were carried out by a Fourier Transform Infrared (FTIR) spectrophotometer to evaluate the chemical structure differences. RESULTS The transverse strength values of 25-Gy, 50-Gy and 75-Gy radiation groups were significantly higher than that of the no radiation group (p < 0.05). There was no significant difference among the elastic modulus values of the study groups (p > 0.05). The FTIR findings demonstrated that the irradiation process did not change the chemical structure of the PMMA polymeric materials. CONCLUSION The therapeutic radiation doses increase the mechanical properties of the PMMA; however, the chemical and structural properties have no effect. When the findings of this study are taken into account, it can be said that patients can wear dentures during the radiotherapy.

Effect of irradiation on the shear bond strength of self-adhesive luting cement in different preparation depths

Objective: The purpose of this study was to evaluate the effects of pre- and post-irradiation application on the shear bond strength of self-adhesive luting cements to dentin and enamel. Materials and Methods: Thirty-two extracted human maxillary incisor teeth were used in this study. Teeth were divided into two main groups according to preparation depth (0.5 mm and 1 mm) as Group E and Group D and were divided into four subgroups according to treatment protocol (n = 12). Teeth were irradiated and preparation was done after radiation. Adhesive luting cement was placed on the irradiated enamel and dentin surface (Groups E1, D1). Preparation was done before irradiation and resin cement was placed on the irradiated enamel and dentin surface (Groups E2, D2). The resin cement was first placed on their enamel and dentin surfaces and then the specimens were irradiated (Groups E3, D3). Irradiation was done with a total dose of 60 Gy, applied in fractions over 6 weeks for each groups (2-Gy/day fractions, 5 days per week). Nonirradiated groups were determined as controls groups (Groups C1, C2). The shear bond strengths of adhesive luting cement were examined. Results: According to the two-way ANOVA results, depth of preparation and treatment protocol and their interactions were significant on shear bond strength of resin cement (P < 0.05). Conclusions: This study detected significant differences between the irradiated and nonirradiated groups, probably due to the changes in the crystalline structure of dental hard tissues.

Histopathological evaluation of the effects of fiber reinforced acrylic resins on living tissues

Abstract Objective. The aim of this study was the histopathological evaluation of the effects of the fiber reinforced acrylic resins on living tissues. Materials and methods. The study was performed on 21 rabbits. Three groups, each including seven subjects, were formed. There was no applied plate in the control group. For the second group, heat-polymerized acrylic resin plates were inserted. For the third group, heat-polymerized acrylic resin plates containing proportionally 5% chopped silanated E type glass fiber were inserted. Plates were fixed to the palatine bone of the rabbits with titanium screws. Before the implementation of the plates and 1 month after the plates were applied, soft tissue samples were taken from the buccal mucosa of the rabbits. Also, tissue samples were taken from the control group. All samples were evaluated histopathologically. Results. In the control group, only a focal atrophy was observed. In the acrylic group, large decomposition containing erythrocytes under the parahyperkeratotic region and micro-vesicle like spongiotic tissue reactions were observed. In the fiber reinforced acrylic group, widespread focal atrophy, bulgy look of the epithelium cells similar to apoptosis, over-distension and sub-corneal decomposition had been observed. In terms of atrophy and hyperkeratosis there were no statistically significant differences among groups. However, in respect to sub-corneal decomposition, there was a statistically significant difference in the fiber reinforced group (p < 0.01). Conclusions. The statistically significant difference in the sub-corneal decomposition of the fiber reinforced group had made us think that fiber edges had a traumatic effect on the reaction.

Gerec ve Yontem: Sivas’ta faaliyet gosteren 4 farkli laboratuvardan, kumlama ve kumlama+cila islemi uygulanmis 1 mm capinda, 1.5 mm kalinlikta disk seklindeki metal ornekler elde edildi. Hazirlanan metal ornekler etilen oksit gaziyla sterilize edildi ve L929 fibroblast hucre serisi kullanilarak elde edilen kulture yerlestirildi. Agar overley testi kullanlarak sitotoksisitenin belirlenmesinde ISO 1999 yl 10993-5 numarali protokolu takip edildi. Bulgular: Hucrelerin lizis miktarina gore yapilan puanlama degerlerine gore 4 laboratuvardan 3 tanesinden elde edilen ornekler sitotoksik bulunmamisken, 1 tanesinden elde edilen ornekler orta derecede sitotoksik bulunmustur. Ayrica c alismamizda tum gruplarda kumlama ve kumlama+cila arasinda sitotoksite acisindan fark bulunamamistir.
