Prevalence of Orofacial Changes in Wind Instrumentalists: A Cross-Sectional Pilot Study in Brazil

OBJECTIVEnThe aim of the present study was to identify the prevalence of orofacial changes and factors associated with the frequent playing of wind instruments by musicians in Diamantina, Minas Gerais, Brazil.nnnMETHODSnA cross-sectional study was conducted with 40 professional musicians who played wind instruments. The musicians filled out questionnaires about their habits and musical career. The Research Diagnostic Criteria were applied to investigate temporomandibular disorders. Other oral parameters examined included periodontal status, plaque index, and decayed, missing, and filled teeth (DMF) index. A significance level of p<0.05 was adopted for statistical analysis.nnnRESULTSnThe population included 22 (55%) men and 18 (45%) women, with a mean age of 25.88 yrs. The most common instruments played were the saxophone (n=12, 30%), clarinet (n=9, 22.5%), and flute (n=6, 15%). The most frequent pain complaint after playing was numbness in the upper lip or no sensation (25% of musicians), and the most frequent dental change observed was inclination of the upper teeth (15% of musicians). There was no significant association between dental changes, pain, and classification of instrument type.nnnCONCLUSIONnThe main oral change that affected musicians who played wind instruments was the inclination of anterior teeth.

Cross-cultural adaptation of the Brazilian version of the Dentine Hypersensitivity Experience Questionnaire (DHEQ-15)

The dentine hypersensitivity (DH) is able to impair the oral health related quality of life (OHRQoL). However, there isnt any specific validated questionnaire to be used in Brazil. The objective was to adapt and to validate the English version of the Dentine Hypersensitivity Experience Questionnaire (DHEQ-15) for use in Brazil. DHEQ-15 was cross-culturally adapted into the Brazilian-Portuguese language and then validated in a cross-sectional study with 100 participants recruited at a University clinic. Study sample comprised 2 groups: 100 individuals with DH, and 100 individuals without. The instrument was self-administered twice 7 to 10 days apart. The participants answered a global rating of oral health. The psychometric properties of the Brazilian version of DHEQ-15 were verified through internal consistency (Cronbachs α) and test-retest reliability (intraclass correlation coefficient - ICC), convergent (Spearman correlation) and discriminant (Mann-Whitney test) validity. The significance threshold was set at p<0.05. Sample comprised 69 men and 131 women, of mean age 30.4y. The Brazilian DHEQ-15 demonstrated very good internal consistency (α = 0.945). Test-retest reliability revealed excellent reproducibility (ICC = 0.959, p < 0.001). There was statistically significant correlation between the scores obtained on all DHEQ-15 domains and the global rating of oral health (p<0.001). Participants with DH scored significantly higher than those without DH (p<0.001). This study provides evidence supporting the cross-cultural validity of the Brazilian version of DHEQ-15 for use in Brazil.

Associated factors to cervical dentin hypersensitivity in adults: a transversal study

BackgroundThis cross-sectional study aimed to identify the factors associated to the cervical dentin hypersensitivity (DH) in Brazilian adult population.MethodsThree hundred and eighty patients (67.2% women and 32.8% men) were assessed by questionnaire and thermal test with ice. Participants marked in a visual analogue scale (VAS) the intensity of pain, and a calibrated examiner (ICC 0.990) measured the scores using a caliper. Pain scores above 0.5xa0mm were considered sensitive teeth. The DH associated factors were investigated by clinical examination. The association between variables was assessed by Spearman correlation and the Chi-square test. Logistic regression was used to determine the variables that predict DH (pxa0<u20090.05).ResultsThe mean age of participants was 24.08xa0years. In this population, 8958 teeth were evaluated, of those 3037 (33.9%) were diagnosed sensitive. The most prevalent associated factors to DH were abnormal tooth positioning (9.0%), occlusal trauma (6.5%) and gingival recession (5.6%). The erosion predicted significantly the DH on both simple (OR 7.85, pxa0<u20090.001) and multiple(OR 4.36, pxa0<u20090.001) analysis.ConclusionThe exposure of dentinal tubules by erosion is probably the major predictor of dentin hypersensitivity. The healthy tooth is able to protect against DH.

Relationship between implant stability on the abutment and platform level by means of resonance frequency analysis: A cross-sectional study

Resonance frequency analysis (RFA) has become the main tool used to assess the osseointegration of dental implants. The objective of this study was to verify the relationship between the ISQ values with different prosthetic abutments and with the implant platform. The hypothesis was that ISQ values changes according to the abutment height. Twelve patients were included, whose contribution to the study was 31 dental implants (external hexagon connection implants, 4.1x10 mm). The temporary implant-supported crown and prosthetic components were removed and the following smartpegs were inserted, one at a time: type 1, in the implant platform (G1); type A3, in the microunit component with 1mm transmucosal height (G2) and type A3, in the microunit component with 5mm transmucosal height (G3). In all the smartpegs, RFA measurements were taken on mesial, distal, buccal and lingual surfaces. All evaluations were performed by a single calibrated examiner (ICC = 0.989). Data were analyzed by Friedman and Spearman correlation tests and log-linear marginal regression (p<0.05). The mean age of participants was 52.83 (± 3.77) years. There was statistically significant difference (p<0.001) among the mean ISQ of G1 (88.27 ±5.70); G2 (72.75 ±4.73) and G3 (66.33 ±3.67). There was statistically significant negative correlation between the ISQ and the measurement distance (rs:-0.852; p<0.001; R2:0.553). Measurement distance was significantly associated (p<0.001) with ISQ value in the log-linear regression. The abutment height has a significant impact on resonance frequency analysis measurements. The higher the transmucosal abutment height, the lower the implant stability quotient value. Clinically, the ISQ measured on the abutment cannot be compared with values measured on the implant platform.

Relationship between sjögren syndrome and periodontal status: A systematic review

OBJECTIVEnThis study aimed to examine whether Sjögren syndrome (SS) is related to periodontal status.nnnSTUDY DESIGNnA systematic review was performed on the basis of PRISMA (PROSPERO: CRD42017055202). A search was performed in the PubMed/MEDLINE, LILACS, Web of Science, and Science Direct databases. Hand searches and review of the gray literature were also performed. Three researchers independently selected studies, extracted data, and assessed methodologic quality. Studies that correlated primary and/or secondary SS with plaque index, gingival index, probing depth, and bleeding on probing were included. The risk of bias was estimated on the basis of the Newcastle-Ottawa scale.nnnRESULTSnSeventeen studies were included in the review and 9 included in the meta-analysis, with a total of 518 and 544 patients, with or without SS, respectively. The mean difference of plaque index (0.29; 95% confidence interval [CI] 0.17-0.41), gingival index (0.52; 95% CI 0.14-0.89), and bleeding on probing (9.92; 95% CI 4.37-15.47) were larger in patients with SS than in controls. In primary SS (0.47; 95% CI 0.10-0.83) and secondary SS (0.74; 95% CI 0.10-1.38), only the mean gingival index was larger compared with that in control group. The majority of the included studies were judged as having a high risk of bias.nnnCONCLUSIONSnThe present review did not provide strong evidence that periodontal status is affected by SS.

Effect of dentin hypersensitivity treatment on oral health related quality of life — A systematic review and meta-analysis

OBJECTIVEnThis study aimed to evaluate if in patients with dentin hypersensitivity (DH), the DH treatments are able to improve individuals oral health related quality of life (OHRQoL).nnnDATA AND SOURCESnA systematic review was performed based on PRISMA guidelines (PROSPERO CRD42016050864). Clinical trials reporting OHRQoL before and after DH treatment were included. The search was performed in the PubMed/MEDLINE, Scopus, Web of Science, Cochrane Library, LILACS, EMBASE and Scielo databases until May 2017. Hand searches and grey literature were included. Three researches independently selected the studies, extracted data, and assessed the methodological quality. The risk of bias was estimated based on the Cochrane Handbook for Systematic Reviews of Interventions. Meta-analysis was performed by I2 test. The quality of evidence was assessed using Grading of Recommendations Assessment, Development and Evaluation (GRADE).nnnSTUDY SELECTIONnSix clinical trials were included. DH was assessed by evaporative, cold, and tactile stimuli. OHRQoL was evaluated by OHIP-14 and DHEQ questionnaires. In-home and in-office desensitizing agents for DH treatment were used. The revised studies reported statistically significant reduction of DH and significant improvement in quality of life after treatment (p<0.05). Two studies were judged as high risk of bias. The studies presented high heterogeneity (I2=0.8407). The evidence was very low to moderate.nnnCONCLUSIONSnThe studies indicated decreasing of DH and improving of OHRQoL after DH treatment, although, they presented low to moderate methodological quality.nnnCLINICAL SIGNIFICANCEnThe Oral Health Relate Quality of Life of whom complaint of DH can be improved after DH treatment.

Local Flap Technique After Removing Premaxilla for Closure of Oronasal Fistula

This letter aimed to describe a surgical technique for oronasal fistula closure that promotes efficient healing and minimizes recurrence. The oronasal fistula was repaired by buccal flap. The technique proved to be simple and efficient.
