It can always happen: the impact of urinary incontinence on elderly men and women.

Objective. To determine the impact of urinary incontinence (UI) on the quality of life of the elderly in the general population and to identify factors with the greatest effect. Design. Qualitative and quantitative analyses of interview data. Setting. Patients from the nine family practices of the Nijmegen University Research Network. Subjects. Independently living patients aged 60 and over. Main outcome measures. All independently living patients aged 60 and over with uncomplicated UI were interviewed at home using the Incontinence Impact Questionnaire and open-ended questions. Results. In total, 56 men and 314 women were interviewed. A majority do not have such an impact. In the Incontinence Impact Questionnaire (IIQ) emotional well-being was most affected. Half to one-third of the patients felt nervous, embarrassed, or frustrated because of their incontinence. In the social domain “clothing” and “fear of odour” scored the highest impact. The most affected practical consequence in the IIQ was “going to places where you are not sure about the availability of a toilet” followed by “travelling longer than 20 minutes” and “entertainment”. Men reported higher impact scores than women, despite the fact that incontinence was less severe in men. The most important effect of incontinence reported in men was “being out of control” while most women considered “feeling impelled to take several precautions” to be the most important consequence of UI. Conclusion. UI affects nearly half of patients, particularly as regards their emotional well-being and in public activities. Men experienced more impact compared with women and experienced loss of control more often than women.

Urinary incontinence in community dwelling elderly: are there sex differences in help-seeking behaviour?

Objective – To assess sex differences in disorder- and patient-specific factors influencing help-seeking behaviour by independently living elderly people with urinary incontinence. Design – Qualitative and quantitative analyses of the collected data. Setting – Patients from nine family practices involved in the Nijmegen Monitoring Project. Subjects – Independently living elderly aged 60 or over. MAIN outcome measures – The study population consisted of participants in a study on the prevalence of incontinence among elderly people. Independently living elderly people aged 60 or over from nine family practices with uncomplicated urinary incontinence were interviewed at home using a disorder-specific questionnaire and open-ended questions. Results – In total, 56 men and 314 women were interviewed. Half of them had sought help from a general practitioner. Men sought help from their GP when they had less severe incontinence than women. Help-seeking behaviour in women was related to the duration of symptoms (odds ratio 3.66, 95% CI 1.44–9.39), the presence of incontinence-related symptoms such as constant feeling of vaginal pressure and pain in the lower abdomen (odds ratio 2.69, 95% CI 1.52–4.76) and the severity of incontinence (odds ratio 1.69, 95% CI 0.99–2.88). In men help-seeking was related to the distress experienced in daily life (odds ratio 7.10, 95% CI 1.15–43.91). Most men and women who did not seek help consider their incontinence as not being serious enough, or they had inappropriate beliefs such as that “incontinence is age-related” and “there is nothing that can be done about incontinence”. Conclusion – Help-seeking in women is determined by the duration of the symptoms, the presence of concomitant complaints and the severity of incontinence. In men help-seeking behaviour is mostly related to the distress experienced in daily life. In men and women the most important reasons for not seeking help are that they consider the incontinence not serious enough and that they believe that there are no treatment options available.

Urinary incontinence in the elderly: attitudes and experiences of general practitioners. A focus group study.

OBJECTIVEnTo assess general practitioners (GPs) attitudes to urinary incontinence in elderly patients and their experiences in the application of the Dutch College of General Practitioners guideline in daily practice.nnnDESIGNnTwo existed groups of six GPs working in villages and seven GPs working in urban practices.nnnMETHODnTwo focus-group discussions with recording of discussions and transcription. Transcripts were analysed by two independent researchers.nnnRESULTSnDuring the discussions three main themes of attitudes came forward: (1) therapeutic nihilism of GPs and low motivation of patients, (2): GPs experienced lack of time because of difficulties in explaining the therapy and because of impaired mobility of older patients, (3) because of the complexity of the problem and co-morbidity, GPs as well as patients were reluctant to treat the UI. The most remarkable findings in the application of the guideline were: (1) because of the barriers mentioned above, physical examination did not take place in spite of GPs conviction as to the benefit of it; (2) GPs knowledge of treatment options in the elderly with UI is substandard.nnnCONCLUSIONnSeveral patient (comorbidity, impaired mobility, low motivation, and acceptance of the problem) and GP factors (therapeutic nihilism, lack of time and knowledge) interfere with good management of UI in the elderly.

Hormoontherapie en CVA

SamenvattingIn een Deens cohort van bijna een miljoen vrouwen blijkt dat orale hormoontherapie de kans op het krijgen van een cerebrovasculair accident (CVA) verhoogt, al zijn de absolute aantallen klein. Het risico is afhankelijk van het type preparaat, de dosis, de duur van de behandeling, de toedieningswijze en de leeftijd. Transdermale toediening lijkt voordelen te hebben; bij lokale klachten gaat de voorkeur uit naar een lokaal oestrogeen. Continue combinatiepreparaten zoals tibolon geven het grootste risico.

SamenvattingWanneer plaatsing van een spiraal door de huisarts ongecompliceerd verloopt en de controle is normaal, is een echo niet nodig. Een echo is wel zinvol bij een gecompliceerde plaatsing, bij onvindbare draadjes, bij vermoeden van lokalisatie in het cervixkanaal en soms bij bloedingsproblemen die ontstaan na eerder probleemloos gebruik.

Huisarts Wet 2017;60(10):507-9.n Bij een veneuze trombose in de voorgeschiedenis van de patiënte of een familiaire trombofilie is een oestrogeen bevattende vorm van anticonceptie gecontra-indiceerd. Van oudsher zijn het koper-IUD en sterilisatie bekende, veilige en effectieve keuzen bij deze patiëntengroep. Barrièremiddelen zijn tevens een optie, maar die zijn minder betrouwbaar. Er zijn meer mogelijkheden, zo blijkt uit recent onderzoek. De progesteron only-middelen levonorgestrel-IUD (Mirena), de desogestrel only-pil (minipil/Cerazette) en het etonogestrel-hormoonstaafje (Implanon NTX) verhogen het tromboserisico niet. Een lichte verhoging van het tromboserisico zien we wanneer de patiënte start met orale anticonceptie in de vorm van levonorgestrel en 30 µg ethinylestradiol bij een lichte familiaire trombofilie (heterozygote factor V Leiden en protrombine G20210A). Samen met de patiënte kiest de huisarts uit de bovengenoemde middelen, waarbij ze de risico’s en voordelen meewegen. Daarbij houden ze rekening met de wensen van de patiënte, de verwachtingen rond het gekozen anticonceptiva, en de voor- en nadelen van de verschillende anticonceptiva, zoals bijwerkingen, gebruikersgemak, bloedingenpatroon, enzovoort. Medroxyprogesterondepot (de prikpil) is niet geschikt als anticonceptivum omdat de hoge dosering progesteron het risico op een veneuze trombose verhoogd.

Vaak is de oorzaak niet ernstig en is deze kwaal gemakkelijk te behandelen. Aan de hand van een goede anamnese kan de huisarts erachter komen of het urineverlies te maken heeft met een anatomische of neurologische afwijking, met verkeerde plasgewoonten, gedrag of infecties. Lichamelijk onderzoek en een mictiedagboek kunnen helpen inzicht in het probleem te krijgen. Met simpele oplossingen, zoals het geven van uitleg en mictieadviezen, het behandelen van eventuele obstipatie en/of het geven van medicatie, kan de huisarts de klachten verhelpen.AbstractSteffens S, Teunissen TAM, Lagro-Janssen ALM. Accidental wetting in children. Huisarts Wet 2016;59(11):508-12.n Involuntary urine loss is common in children. The cause is often not serious and it is easy to treat. An adequate history can help the general practitioner to determine whether it is caused by anatomical or neurological problems, bad toilet habits, behaviour problems, or infections. A physical examination and a micturition diary can provide additional information. The problem can be helped with simple interventions, such as explanations and advice about micturition, treatment of constipation, and/or medication.
