Long-term evaluation of surgically placed amalgam fillings.

The coauthors of this article had developed the clinical impression that whereas surgically placed amalgam fillings are successful on a short-term basis, the long-term prognosis for these cases may not be nearly as favorable. In order to test this hypothesis, as many patients as could be contacted, either directly or indirectly, who had been treated before 1981 were evaluated. Excluded from this study were cases for which failure could be attributed to any reason other than failure from the amalgam reverse filling. Therefore, all cases included had to demonstrate periapical healing prior to ultimate breakdown. On this basis, 60 of 104 teeth (57.7%) were considered to be successful and 44 teeth (42.3%) were determined to be failures.

Predictable endodontic and periodontic failures as a result of radicular anomalies

Abstract A pathosis that is associated with developmental radicular anomalies is described. Such conditions are easy to diagnose only when the diagnostician is aware of their existence. At present, since these anomalies constitute unresolvable periodontal problems, extraction of the involved teeth is the treatment of choice.

Endotoxic activity in periapical lesions

Thirty tissue samples were examined histologically and classified as being inflamed (apical granulomas) or noninflamed (scars or noninflamed cysts). The samples were then homogenized in pyrogen-free water and treated to remove interfering substances. The presence of endotoxin was then determined by means of the limulus assay; 75 percent of the inflamed tissues were positive for endotoxin, while only 20 percent of the noninflamed tissues contained endotoxin. The presence of endotoxin was thus highly correlated (p = 0.015) with the presence of inflammation in these tissues.

Locating refered pulpal pains

Abstract The total effects of pain are dependent upon the intimate interrelationship between perception and reaction to pain, and both facets must be taken into account when the problem is evaluated clinically. Pain can initiate or perpetuate a biologically destructive vicious cycle. It should be combated without undue delay, since continued pain has an injurious effect upon the nervous activity or function of the affected area. Diseases of the teeth usually produce pain locally until the pulp is affected; then referred pain becomes prominent. Referred pain can be a confusing symptom and may be misinterpreted unless properly understood; if evaluated intelligently, it can be of great diagnostic value. Injection analgesia can be effectly employed to distinguish referred pain from pain at the site of a causative lesion, since referred pain is always relieved by interruption of the pain pathway. A series of drawings have been presented in an effort to simplify diagnosis of referred pulpal and associated head pains. Figs. 5 through 13 are based upon statisties gathered by Head 6 and closely corroborated in clinical experience. Figs. 14 through 18 are based upon more than 143 personal clinical observations.

The Anticurvature Filing Method to Prepare the Curved Root Canal

Abstract More than 1,800 patients with a variety of endodontic disorders were treated. Treatment in each case required the use of a rubber dam to eliminate moisture from the operative field. In each instance, Orabase emollient was routinely applied to the tooth or teeth to be treated, to protect the mucosa from the seepage of medications. The emollient paste was applied topically when needed during treatment, and it was removed after treatment. Results were excellent, with no leakage and with complete protection of the surrounding gingiva. No side effects, reactions, or disadvantages were noted. The emollient preparation proved to be superior to petrolatum or to any other material previously used as a protective aid. Also, because of its ability to adhere to mosit tissues, Orabase is an excellent vehicle when mixed with other preparations that must be applied to the gingiva.
