The auditory brain stem response I-V amplitude ratio in normal, cochlear, and retrocochlear ears

Frank E. Musiek; Karen Kibbe; Lisa Rackliffe; Dudley J. Weider

The auditory brain stem response I-V amplitude ratio was measured for 25 normal, 25 cochlear, and 25 retrocochlear ears. Results indicated that amplitude ratios of less than 1.00 seldom occurred in the normal or cochlear ears. However, 44% of the retrocochlear ears revealed an amplitude ratio of less than 1.00. Details of this analysis and discussion of the clinical implications of these results are presented.

Repair of a Cerebrospinal Fluid Perilymph Fistula Primarily through the Middle Ear and Secondarily by Occluding the Cochlear Aqueduct

Dudley J. Weider; Richard L. Saunders; Frank E. Musiek

A 35-year-old man had a 5-year history of fluctuating hearing loss in his only hearing ear. History and diagnostic tests indicated a perilymph fistula, a diagnosis subsequently confirmed by exploration. Primary and secondary repairs temporarily ameliorated symptoms. A spinal fluid to middle ear fluid pathway was identified by radioactive tracer. A patent cochlear aqueduct indicated on computed tomography scan was found and repaired through a posterior cranial fossa approach. Hearing was preserved, remaining relatively stable during the 2-year follow-up period.

Auditory brainstem response and eighth nerve lesions: a review and presentation of cases.

Frank E. Musiek; Ernest Sachs; Nathan A. Geurkink; Dudley J. Weider

A review of salient features in the use of auditory brainstem response for the detection of eighth nerve lesions is presented. Three cases of acoustic tumors are discussed. These cases represent a normal pure-tone audiogram, one with a moderate loss, and one with no measurable hearing on the affected side. Interpretation of the auditory brainstem response in these cases is discussed in light of the various auditory brainstem response measures presented and the pure-tone audiometric data.

Bilateral congenital oval window microfistulae in a mother and son

Dudley J. Weider; Frank E. Musiek

This article details case histories of a son and his mother who were studied over a seven‐year period before making the diagnosis of bilateral oval window microfistulae in both of them. The boy demonstrated a slowly progressive bilateral and symmetrical sensorineural hearing loss closely resembling thnt of his mother. The case reports emphasize their respective histories, diagnostic evaluation, surgical observations, and treatment.

Reversible Audiological Results in a Patient with an Extra-Axial Brain Stem Tumor

Frank E. Musiek; Dudley J. Weider; Rita J. Mueller

A case of a 32-yr-old patient with an extra-axial brain stem tumor is presented. Many postoperative audiological test results were reversed compared to the preoperative results. A variety of auditory measures were conducted to show which tests were sensitive to this patients lesion. Also reported are some rather unique postoperative auditory changes which, in retrospect, provided an interesting view as to the extent of the lesion.
