Three patients with chordomas of the clivus and the upper cervical spine underwent the removal of their tumors through a transoral approach with labiomandibular splitting and microsurgical techniques. This surgical approach provided excellent visualization of the anterior craniocervical area and allowed a gross total removal in each case. Although the lesions in our patients were chordomas, this approach could also be used for other surgical lesions in this area.

A patient with transient ischemic attacks and fibromuscular dysplasia of the internal carotid artery was treated by Grüntzig balloon-catheter dilatation of the involved area of the internal carotid artery. The clinical and angiographic results were excellent.

The clinical experience with lumbar disc disease in 38 patients at a large childrens hospital is reviewed. Thirty children underwent surgical treatment; eight patients were not operated upon. The surgical results were excellent, with 93% of the children cured or markedly improved on follow-up. The main differential aspects of lumbar disc disease between children and adults are emphasized.

The authors report a case of cortical vein and sagittal sinus thrombosis after a transcallosal removal of a colloid cyst of the third ventricle. Thirty-six hours after the operation, the patient developed focal seizures; after progressive deterioration, he died on the sixth postoperative day. At the autopsy, a venous hemorrhagic infarct in the right frontoparietal area and fresh thrombus occluding the sagittal sinus and cortical veins were found. The mechanism of cortical vein and sinus thrombosis subsequent to this procedure is discussed, and suggestions are made to prevent this serious complication.

Nine patients with spinal epidural infection were managed with the suction-irrigation technique with excellent healing of the incision by primary intention in every case. The patients have been followed for 3 to 40 months. One patient developed a subcutaneous stitch abscess, which was treated by removal of the suture material. The surgical technique is described.

A case is reported in which an arteriovenous malformation was removed from a portion of the basal ganglion and region of the lenticulostriate arteries. The patient made an excellent recovery. The importance of stereoangiography and the operating microscope are emphasised.

The irrigation-suction technique was used to treat a patient who developed a deep infection after removal of a herniated lumbar disc. The patient made an excellent recovery. The advantages of this modality of treatment over the standard approach to this problem are emphasized.

A patient with transient ischemic attacks and an occluded right common carotid artery underwent a left superficial temporal artery to right middle cerebral artery vein graft (bonnet bypass). One week after operation, minor trauma to the scalp caused avulsion of the anastomosis and a massive fatal acute subdural hematoma. The mechanism of the vascular avulsion and precautions to be taken to avoid such problems in other patients undergoing this procedure are discussed.
