BackgroundWhereas several studies have underlined the association between severe psoriasis and metabolic syndrome (MetS), the association of androgenetic alopecia (AGA) and MetS have yielded inconsistent results.ObjectiveTo investigate the relationship between AGA and the components of MetS in a population of psoriatic male patients.MethodsA non-interventional, cross-sectional, multicenter study was conducted in France. A standardized questionnaire was completed, including information on components of MetS and other possible risk factors. MetSwas defined in this study as a combination of three or more of the four components of MetS:waist circumference, hyperlipidemia, diabetes mellitus and hypertension. In addition, a standardized simplified Norwood classification limited into 5 grades (0–4) was used.ResultsIn a total of 1073 male patients, hypertension, high waist circumference, diabetes mellitus and hyperlipidemia were observed in 28%, 59%, 11%, and 31%, respectively. In age-adjusted multivariate analysis, severe AGA (grade 3–4 versus grade 0) was associated with the presence of at least one component of MetS. By groups of age, a statistically significant association of severe AGA and MetS was demonstrated in patients over 59 years. SevereAGAwas also associated with a first degree familial history of major cardiovascular event in patients older than 59 years.ConclusionsOur study, based on a simplified but stringent definition of MetS, confirmed the link between severe AGA and individual components of MetS in psoriatic patients. This argues for careful follow-up with regular screening in male psoriatic patients with severe AGA in order to early detect determinants of MetS.

occurred in a young patient without underlying liver disease and treated with low-dose interferon alfa-2b. Alternative causes such as hepatic micrometastases of melanoma, or infectious or autoimmune hepatitis have been ruled out. The chronologic sequence between interferon alfa-2b and liver toxicity is established in our case. Furthermore, complete remission on dechallenge gives strong evidence that interferon alfa-2b was involved. The lengthy latent period and the progression of liver toxicity despite dose reduction support an idiosyncratic reaction. Affected individuals may possess a rare combination of genetic and nongenetic factors. In conclusion, this report suggests that interferon can induce liver failure at low dose without any clear signs of autoimmunity. Normalization of liver enzymes is not sufficient to rule out the progression of a toxic reaction. Thus, careful monitoring of complete liver profile including prothrombin time, factor V, and albumin is necessary when signs of liver toxicity appear. Hence, early drug withdrawal will prohibit the progression toward an irreversible fatal reaction.

thigh, showing hyperkeratosis, acanthosis, vacuolization of basal cells, a dense band-like lymphocytic infiltrate in the upper dermis, and pigment incontinence (Fig. 1c). Treatment with 8 mg/d oral methylprednisolone (0.5 mg/kg/ d) was considered. After three weeks of therapy, flattening of some papules and plaques excluding the linear band-like lesions was observed. Unfortunately, the patient was lost to follow-up. The clinical features of this patient represent a convincing example of superimposed LLP. The linear involvement appeared earlier and was more pronounced than the non-segmental lesions of lichen planus. Kanwar and De reported a linear involvement followed by the appearance of less severe non-linear lesions in two of 100 children with lichen planus. A similar course was recently described in an adult with lichen planopilaris. For obvious reasons, the configuration of superimposed LLP should not be described as zosteriform because the Blaschko-linear arrangement of such lesions has nothing to do with the dermatomes of the human body. It should be noted that the concept of a superimposed segmental manifestation of a polygenic skin disorder has not yet been proven at the molecular level. Cases such as that presented here, however, may be of great interest in future research on the polygenic basis of lichen planus. For this purpose, DNA obtained from the tissue of superimposed linear involvement should be compared with DNA obtained from other skin areas. This applies not only in lichen planus but in all common skin disorders that sometimes show a segmental arrangement.

Recent data have demonstrated that Toll-like receptors (TLRs) play a key role in both innate and adaptive immune responses. The human skin is consistently exposed to a wide range of exogenous stimuli and can exhibit anti-infectious properties. The aim of this review is to describe TLRs and assess the expression of TLRs in human epidermis and the function of these receptors within the epidermis under physiological and pathological conditions.
