We present the surgical technique of reverse abdominoplasty combined with mammary reduction in a single stage. The technical details are described, with special reference to patient selection and postoperative care. Comments about the midsternal scar line are also included.

For closure of radical mastectomy defects, we present a new rotation flap using thoracoabdominal skin which crosses the midline of the trunk. This allows a rapid closure without the necessity of a delay.

ZusammenfassungDie brusterhaltende Therapie des Mammakarzinoms stellt bei kleineren Tumoren eine Alternative zu radikaleren Behandlungsmethoden dar. Die erzielten kosmetischen Ergebnisse sind nicht immer zufriedenstellend. Die Korrektur dieser Deformitäten an der nachbestrahlten Brust ist schwierig und risikoreich. Deswegen solite in die Primäroperation bereits die Rekonstruktion der Brustdrüse mitaufgenommen werden. Für dieses operative Vorgehen werden technische Möglichkeiten aufgezeigt. Vom onkologischen Standpunkt aus erscheint dies vertretbar.SummaryBreast-conserving therapy in mammary carcinoma is an alternative to the more radical forms of therapy when the tumors are small. The cosmetic results obtained are not always satisfactory. Correction of deformities of this kind in breasts which have been subjected to post-operative radiation is difficult and dangerous. Consequently, reconstruction of the mammary gland should be included in the primary operation. The technical possibilities for doing so are listed. It seems to be justifiable from an oncological point of view.

The authors present a simple surgical procedure for changing a pointed chin into a square one, through the resectioning of a horizontal bone fragment of the free edge of the mentum. An amount of soft tissue of this region can be resected during surgery as a secondary reshaping maneuver. A third technical detail can be added to improve the final result, using a small bone resection wedge in the middle of the mentum edge. A lazy groove on the new square chin line is obtained. A comparative, preoperative and postoperative study of the face contour is presented.
