Immunohistochemical Detection of Estrogen Receptor in Invasive Human Breast Cancer: Correlation with Heat Shock Proteins, pS2 and Oncogene Products

The authors immunohistochemically studied the expression of the estrogen receptor (ER), 27-kD heat shock protein (HSP27) and pS2 in 118 invasive primary human breast cancers. Positive nuclear staining of the ER was detected in 64% of the cases and was closely correlated with the biochemical assay (p < 0.0001). ER-positive tumors were significantly decreased with tumor size and stage (p < 0.001 each), but not with lymph node status. Positivity of the ER was correlated with the cytoplasmic expression of HSP27 (p < 0.005), pS2 (not significant) and HSP70 (not significant). ER negativity was significantly correlated with the expression of p53, epidermal growth factor receptor (EGFR) and c-erbB-2 (p < 0.05 each). Thus, it was concluded that ER-positive breast carcinomas, relatively small in size, preferentially expressed HSP27, HSP70 and pS2 and that ER-negative tumors, relatively large in size, were predisposed to express p53, EGFR and c-erbB-2.

AN ULTRASTRUCTURAL STUDY OF EXTRASKELETAL MESENCHYMAL CHONDROSARCOMA

A rare case of extraskeletal mesenchymal chondrosarcoma in a 47‐year‐old woman is reported. The tumor was located in the soft tissue of left upper arm without any involvement of the humerus. The outer portion of the tumor was histologically composed of primitive mesenchymal cells, and the inner portion showed zones of cartilaginous differentiation. Ultrastructurally, the tumor cells in the primitive areas had a relatively small number of mitochondria, rough endoplasmic reticulum, and free polysomes, and the matrix of the tumor consisted of dense bundles of collagen fibrils. The cartilaginous cells seen in the central portion of tumor were embedded in the matrix of abundant collagen fibrils and matrix granules. They had polysomes, well‐developed Golgi complexes, and ample rough endoplasmic reticulum frequently with dilatation. Clear zones adjacent to the cells were seen in the extracellular matrix. With a review of the literature, the histogenesis of this particular tumor is discussed. ACTA PATHOL. JPN. 34: 1355–1363, 1984.

Cytologic Diagnosis of Nonpalpable Breast Cancer Presenting Nipple Discharge

乳頭異常分泌 (ND) を主徴とする無腫瘤性乳癌に対する細胞診手法とその診断能を検討した。対象は1980年1月から1995年12月までの教室で扱ったNDを呈した無腫瘤性乳癌61例である。分泌物細胞診 (S法), 乳管洗浄細胞診 (W法) の正診 (ClassIIIb以上) 率, 確診 (Class IV・V) 率はS法 : 11.5%, 8.2%, W法 : 32.8%, 16.4%であり, この2法によるClassIIIa以上の異型細胞検出率も54.1%と高率ではなかった。乳管内視鏡の開発, 導入 (1989年8月) 以後はそれ以前に比しS法やW法の診断率が低下したが, 乳管内視鏡下に考案, 施行されたキュレット細胞診 (C法) の正診率45.5% (10/22), 確診率36.4% (8/22) であり, 末梢乳管に位置する微小乳癌を検出し得たことが注目される。画像診断法の発展とともに微小な乳癌の検出数が増加し, 新しい細胞採取法であるC法の診断的意義が大きい。画像診断と細胞診の発展がNDを示す無腫瘤性乳癌の早期検出・診断に不可欠である。

The Characteristics and Problems of Interval Breast Cancer.

中間期乳癌を検診後2年以内に検診以外の方法で検出された乳癌として, その特性と意義について検討した。対象は1979年から1994年までにわれわれが行ってきた北海道郡部における乳癌集団検診後に検出された中間期乳癌9例と中間期に自覚症状が出現し繰返し受診で検出された乳癌18例である。検診は視・触診を基本スクリーニングとして行われた。中間期乳癌はTis : 1例, Stage II : 8例で, 7例がn0, 2例がn1αであり, 自覚症状出現繰返し受診乳癌は18例中10例 (55.6%) がStageII以上で, n1β・n2が6例 (33.3%) と進行例が多かった。後者は自覚症状出現時に専門医療機関を受診したならば中間期乳癌となったものと思われることから, 検診発見乳癌と対比した。その結果, 中間期乳癌に準ずるものと考えられた。組織型と発育進展様式において, 中間期乳癌は圧排性増殖を示すものが過半数を占め, 自覚症状出現繰返し受診乳癌では乳管内進展型が半数を占めた。このことは前者が後者に比しドラマチックに触知される病型であることを推察させるものである。中間期乳癌は検診法の評価の1指標とされる。中間期乳癌には腫瘍増殖速度の速いもの以外に, 本邦における現行の検診法では検診見逃し例や早期発見の難しい発育進展様式を示す乳癌が存在すると考えられる。したがって, マンモグラフィ導入など検診法の改善と受診者への知識啓蒙を図らねばならない。

Current Status and Topics of Breast Cancer Screening in Hokkaido, with Special Reference to Hokkaido's Regional Peculiarity

北海道は広域で乳腺診療施設も都市に集中していることから, 患者の受診は地理的, 経済的にも容易ではない。この地域特殊性を考慮し, 乳癌集団検診の一次検診の場に二次検診としてマンモグラフィや穿刺吸引細胞診を導入した出張方式による検診を行ってきた。過去15年間の検診の結果, 149例の乳癌を検出した (検出率0.15%) 。初回受診者における乳癌検出率は0.24%, 繰返し受診者のそれは0.10%と高く, 中間期乳癌は8例であった。検出乳癌は外来受診乳癌に比し早期例が多く, 特に自覚症状のない “検診発見乳癌” では77.5%が早期乳癌であった。本道の地域特殊性からみた乳癌検診の現状を述べるとともに, 教室の検診成績をもとに問題点を考察した。検診の充実と拡大を推進するために, 一次検診にマンモグラフィなど画像診断法の適切な導入が早急に望まれる。

HETEROTOPIC CARCINOMA OF THE STOMACH, REPORT OF A CASE

A 55-year-old woman underwent a medical check-up. X-ray examination revealed an elevated lesion on the an-terior wall of the antrum (Figs., 1, 2). Endoscopic examination of the stomach showed an elevated lesion with bridging folds (Fig. 3). Gastric biopsy proved the lesion to be nonmalignant. Subtotal gastrectomy was performed because it seemed probable that the lesion might contain a submucosal tumor or carcinoma. We diagnosed adenocarcinoma of the stomach in the frozen section. Gross findings of the resected specimen showed an elevated lesion, 2.3×1.5cm in size (Fig. 5). The lesion was located in the muscular region of the stomach, well-defined and separated from the surrounding normal tissue. The tip of the lesion was slightly depressed and erosive. The histological diagnosis was well-differentiated tubular adenocarcinoma of the stomach-tub, , INFβ, ly1 v0, awθowθn1θ0/28, n2θ0/8. Histologically, it was proved that the adenocarcinoma was produced in heterotopic growth in the subumucosal layer of the stomach (Fig. 8a). This is a rarely seen lesion although a few reports concerning similar lesions have been made.
