Computed tomography in gastrointestinal stromal tumors

Abstract.The aim of this study was to define the imaging characteristics of primary and recurrent gastrointestinal stromal tumors (GIST) in computed tomography with respect to the tumor size. Computed tomography was performed in 35 patients with histologically confirmed gastrointestinal stromal tumors and analyzed retrospectively by two experienced and independent radiologist. The following morphologic tumor characteristics of primary (n=20) and (n=16) recurrent tumors were evaluated according to tumor size, shape, homogeneity, density compared with liver, contrast enhancement, presence of calcifications, ulcerations, fistula or distant metastases and the anatomical relationship to the intestinal wall, and the infiltration of adjacent visceral organs. Small GIST (<5xa0cm) showed a sharp tumor margin with homogeneous density and structure on unenhanced and contrast-enhanced images, and were characterized by an intraluminal tumor growth. Intermediate sized GIST (>5–10xa0cm) demonstrated an irregular shape, inhomogeneous density on unenhanced and contrast-enhanced images, a combined intra- and extraluminal tumor growth with aggressive findings, and infiltration of adjacent organs in 9 primary diagnosed and 2 recurrent tumors. Large GIST (>10xa0cm), which were observed in 8 primary tumors and 11 recurrent tumors, showed an irregular margin with inhomogeneous density and aggressive findings, and were characterized by signs of malignancy such as distant and peritoneal metastases. Small recurrent tumors had a similar appearance as compared with large primary tumors. Computed tomography gives additional information with respect to the relationship of gastrointestinal stromal tumor to the gastrointestinal wall and surrounding organs, and it detects distant metastasis. Primary and recurrent GIST demonstrate characteristic CT imaging features which are related to tumor size. Aggressive findings and signs of malignancy are found in larger tumors and in recurrent disease. Computed tomography is useful in detection and characterization of primary and recurrent tumors with regard to tumor growth pattern, tumor size, and varied appearances of gastrointestinal stromal tumors, and indirectly gives hints regarding dignity and therefore prognostic outcome.

Whole-body MRI in the detection of bone marrow infiltration in patients with plasma cell neoplasms in comparison to the radiological skeletal survey

To compare the diagnostic value of whole-body MRI versus radiological skeletal survey (RSS) in staging patients with plasma cell neoplasms (PCN) and to evaluate the possible therapeutic impact of the replacement of RSS by whole-body MRI. Fifty-four patients with PCN [multiple myeloma (MM), n=47; monoclonal gammopathy of unknown significance (MGUS), n=7] were studied by whole-body MRI and RSS in a monocenter prospective analysis from August 2002 to May 2004. The MRIs were performed using a rolling table platform “AngioSURF” for unlimited field of view with a 1.5-T system (Magnetom Sonata/Maestro Class, Siemens Medical Solutions, Erlangen, Germany). A coronal STIR sequence (TR5500-4230/TE102-94/TI160) was used for imaging of the different body regions, including the head, neck, thorax, abdomen, pelvis and upper and lower extremities. The RSS consisted of eight different projections of the axial and appendicular skeleton. In 41/54 (74%) patients, the results of the whole-body MRI and RSS were concordant. In 11/54 (20%) patients, both imaging techniques were negative. Bone involvement was observed in 30/54 (55%) patients; however, whole-body MRI revealed this more extensively than the RSS in 27/30 (90%) patients with concordant positive imaging findings. In 3/30 (10%) patients, both imaging techniques demonstrated a similar extent of bone marrow infiltration. In 10/54 (19%) patients, the whole-body MRI was superior to RSS in detecting bone marrow infiltration, whereas the RSS was negative. In 3/54 (6%) patients, the RSS was proven to be false positive by the clinical course, whereas the whole-body MRI was truly negative. Whole-body MRI is a fast and highly effective method for staging PCN patients by the use of a rolling table platform. Moreover, it is more sensitive and specific than RSS and reveals bone marrow infiltration and extensive disease more reliably. Therefore, whole-body MRI should be performed as an additional method of exactly staging PCN patients and - with more data in the field - may even prove to be an alternate and more sensitive staging procedure than RSS in PCN patients.

Dual-Energy CT Virtual Noncalcium Technique: Detecting Posttraumatic Bone Marrow Lesions—Feasibility Study

PURPOSEnTo evaluate traumatized bone marrow with a dual-energy (DE) computed tomographic (CT) virtual noncalcium technique.nnnMATERIALS AND METHODSnIn this prospective institutional review board-approved study, 21 patients with an acute knee trauma underwent DE CT and magnetic resonance (MR) imaging. A software application was used to virtually subtract calcium from the images. Presence of fractures was noted, and presence of bone bruise was rated on a four-point scale for six femoral and tibial regions by two radiologists. CT numbers were obtained in the same regions. Consensus reading of independently read MR images served as the reference standard. Image ratings and CT numbers were subjected to receiver operating characteristic curve analysis.nnnRESULTSnAfter exclusion of 16 regions owing to artifacts, MR imaging revealed 59 bone bruises in the remaining 236 regions (19 of 114 femoral, 40 of 122 tibial). Fractures were present in eight patients. Visual rating revealed areas under the curve of 0.886 and 0.897 in the femur and 0.974 and 0.953 in the tibia for observers 1 and 2, respectively. For CT numbers, the respective areas under the curve were 0.922 and 0.974. If scores of 1 and 2 (strong or mild bone bruise) were counted as positive, sensitivities were 86.4% and 86.4% and specificities were 94.4% and 95.5% for observers 1 and 2, respectively. The kappa statistic demonstrated good to excellent agreement (femur, kappa = 0.78; tibia, kappa = 0.87).nnnCONCLUSIONnThis DE CT virtual noncalcium technique can subtract calcium from cancellous bone, allowing bone marrow assessment and potentially making posttraumatic bone bruises of the knee detectable with CT.

Computer-navigated iliosacral screw insertion reduces malposition rate and radiation exposure.

AbstractInsertion of percutaneous iliosacral screws with fluoroscopic guidance is associated with a relatively high screw malposition rate and long radiation exposure. We asked whether radiation exposure was reduced and screw position improved in patients having percutaneous iliosacral screw insertion using computer-assisted navigation compared with patients having conventional fluoroscopic screw placement. We inserted 26 screws in 24 patients using the navigation system and 35 screws in 32 patients using the conventional fluoroscopic technique. Two subgroups were analyzed, one in which only one iliosacral screw was placed and another with additional use of an external fixator. We determined screw positions by computed tomography and compared operation time, radiation exposure, and screw position. We observed no difference in operative times. Radiation exposure was reduced for the patients and operating room personnel with computer assistance. The postoperative computed tomography scan showed better screw position and fewer malpositioned screws in the three-dimensional navigated groups. Computer navigation reduced malposition rate and radiation exposure.n Level of Evidence: Level II, therapeutic study. Sinonasal computed tomography in patients with Wegener's granulomatosis.

The goal of this study was to describe pathologies of sinonasal CT in patients with Wegeners granulomatosis. Between 1993 and 2004, sinonasal CT was performed in 28 patients (15 male, 13 female) with Wegeners granulomatosis. The following imaging findings were assessed on the CT scans: mucosal thickening, subtotal opacification, air-fluid level, bony destruction, sclerosing osteitis, bony thickening, orbital mass, and saddle nose deformity. Of the 28 patients, 61% showed mucosal thickening in the nasal cavity and 75% in the paranasal sinuses. A subtotal opacification of the paranasal sinuses was detected in 25%, and of the mastoid cells in 7% of patients. Fifty-seven percent of patients had bony destruction of the nasal cavity, and 54% of the paranasal sinuses. CT revealed sclerosing osteitis of the paranasal sinuses in 21%, and of the mastoid cells in 18% of patients. Bony thickening of the paranasal sinuses was detected in 18%, and of the mastoid cells in 14% of patients. With respect to all imaging findings, the maxillary sinuses were the most frequently affected paranasal sinuses. The authors conclude that the following sinonasal CT findings are observed in patients with Wegeners granulomatosis: (1) bony destruction, mainly of the nasal cavity, maxillary sinuses, and mastoid cells; (2) sclerosing osteitis, mainly of the maxillary sinuses and mastoid cells; (3) bony thickening, mainly of the maxillary sinuses and mastoid cells; and (4) mucosal thickening, mainly of the maxillary sinuses. These findings are, however, nonspecific and should be combined with other clinicopathologic and laboratory studies to confirm the diagnosis of Wegeners granulomatosis.

Radiological findings in Boerhaave's syndrome

The aim was to define the diagnostic value of chest radiography, esophagography, and computed tomography (CT) in patients with Boerhaaves syndrome. CT findings in 14 patients (11 male, 3 female; mean age: 60 years; median age: 66 years; age range: 36–78 years) with spontaneous esophageal perforation were retrospectively reviewed and compared to those of esophagography (n=11) and chest radiography (n=14). In six patients unenhanced CT scans were available; in ten patients (2/10 patients had an unenhanced CT before) a contrast-enhanced CT was performed as an emergency investigation. In 5/14 patients additional oral contrast medium was given. Chest radiography revealed pleural effusions in 9/14 and infiltrates in 9/14 patients. Pneumothorax and pneumopericardium were observed in two patients each. Pneumomediastinum was seen in three patients. Two chest radiographs were normal. Esophagography performed in 11 patients demonstrated contrast medium extravasation at a supradiaphragmatic level in seven patients, indicating esophageal perforation with esophagopleural fistula, and a submucosal contrast medium collection in four cases. Unenhanced CT scans revealed four intramural hematomas with typical localization. Unenhanced and contrast-enhanced CT demonstrated periesophageal air collections indicating esophageal perforation in all patients. Pleural effusions were seen in 11/14 and infiltrates in 8/14 patients. Contrast medium extravasation was observed in 5/14 patients, whereas a thickening of the esophageal wall was depicted in 11/14 patients. Periaortic air tracks (n=7), pneumothorax (n=3), pneumomediastinum (n=6), and mediastinal fluid collections (n=7) were depicted in CT. By demonstrating periesophageal air tracks suggestive of esophageal perforation, CT reveals the decisive criteria for diagnosing Boerhaaves syndrome and should therefore be performed in the diagnostic work-up of patients in whom this syndrome is part of the differential diagnosis.

Thoracic Aorta: Prospective Electrocardiographically Triggered CT Angiography with Dual-Source CT—Feasibility, Image Quality, and Dose Reduction

PURPOSEnTo prospectively investigate the feasibility, image quality, and radiation dose for prospective electrocardiographically (ECG) triggered sequential dual-source computed tomographic (CT) angiography of the thoracic aorta in comparison to retrospective ECG-gated helical dual-source CT angiography.nnnMATERIALS AND METHODSnThis study was approved by the institutional review board; informed consent was obtained. One hundred thirty-nine patients referred for ECG-assisted dual-source CT angiography of the thoracic aorta were prospectively enrolled. Inclusion criteria were stable sinus rhythm and heart rate of 80 beats per minute or less. Tube voltage was adjusted to body mass index (< 25.0 kg/m(2), 100 kV, n = 58; > or = 25.0 kg/m(2), 120 kV, n = 81). In both cohorts, patients were randomly assigned to prospective or retrospective ECG-assisted data acquisition. In both groups, tube current (250 mAs per rotation) was centered at 70% of the R-R cycle. The presence of motion or stair-step artifacts of the thoracic aorta was independently assessed by two readers. Effective radiation dose was calculated from the dose-length product.nnnRESULTSnSubjective scoring of motion and stair-step artifacts was equivalent for both techniques. Scan length was not significantly different (23.8 cm +/- 2.4 [standard deviation] vs 23.7 cm +/- 2.5 for prospective and retrospective ECG-triggered CT angiography, respectively; P = .54). Scanning time was significantly longer for prospective ECG-triggered CT angiography (18.8 seconds +/- 3.4 vs 16.4 seconds +/- 3.3, P < .001). Mean estimated effective dose was significantly lower for prospective data acquisition (100 kV, 1.9 mSv +/- 0.5 vs 4.1 mSv +/- 0.7, P < .001; 120 kV, 5.3 mSv +/- 1.1 vs 9.5 mSv +/- 3.0, P < .001).nnnCONCLUSIONnProspective ECG-gated sequential dual-source CT angiography of the thoracic aorta is feasible, despite the slightly longer acquisition time. Thus, motion-free imaging of the thoracic aorta is possible at significantly lower radiation exposure than retrospective ECG-gated helical dual-source CT angiography in certain patients with a regular heart rate.

Comparison of Radiologist and CAD Performance in the Detection of CT-confirmed Subtle Pulmonary Nodules on Digital Chest Radiographs

Objectives:Detection of subtle pulmonary nodules on digital radiography is a challenging task for radiologists. The aim of this study was to evaluate the performance of a newly approved computer aided detection (CAD) system. Materials and Methods:The sensitivity of 3 radiologists and of a CAD system for the detection of pulmonary nodules from 5 to 15 mm in size on digital chest radiography of 117 patients was compared. The reference standard was established by consensus reading of computed tomography scans by 2 experienced radiologists. Computed tomography scans and chest radiographs were performed within 4 weeks. Sixty-six pulmonary nodules from 42 patients, with a mean nodule diameter of 7.5 mm (standard deviation: 2.2 mm), were included in the statistical analysis. Seventy-five of the 117 patients did not have nodules from 5 to 15mm of size. Results:Two hundred and eighty-eight false-positive detections of the CAD system were found with an average of 2.5 false-positives per image. Sensitivity of the CAD system was 39.4% (95% confidence interval: 11.8%), when compared with 18.2% to 30.3% (95% confidence interval 9.3% to 11.1%) of the 3 radiologists. Substantial agreement for nodule detection ([kappa]N: 0.64–0.73) was found among the 3 radiologists, whereas only moderate agreement was found between the radiologists and the CAD performance ([kappa]N: 0.45–0.52). Conclusions:The CAD systems diagnostic sensitivity in detecting pulmonary nodules of 5 to 15 mm of size was superior to the 1 of radiologists. The CAD system may be used for assisting the radiologist in the detection of lung nodules on digital chest radiographs.

Technologies for image distribution in hospitals

After the establishment of web-based image distribution, three challenges for image distribution can be identified today. Firstly, PACS (picture archiving and communication system) and the distribution of radiological images and reports need to be integrated with the emerging electronic medical record. Secondly, report and image data should be available on mobile devices like PDAs (personal digital assistants) or smartphones in the future. Thirdly, future systems must be available not only to transmit sectional images, but also to allow access to three- and four-dimensional data that are produced by multidetector CT and modern MR scanners.

MR findings in a rare case of sclerosing mesenteritis of the mesocolon

Sclerosing mesenteritis is a rare, usually benign disorder of the mesentery. Depending on the predominant tissue component (inflammation, fat, or fibrosis), it is known as mesenteric panniculitis or retractile mesenteritis. We present a rare case of retractile mesenteritis of the mesocolon as a cause of severe abdominal pain. US, CT, and MRI were the imaging modalities used. We emphasize the MR finding of a fibrous capsula in retractile mesenteritis, as this is to our knowledge the first study to describe this entity. This finding may be valuable for establishing a diagnosis of sclerosing mesenteritis, as well as for differentiating this disease from other mesenteric diseases. J. Magn. Reson.
