Risk Factors for Laryngeal Cancer in Montenegro

Risk Factors for Laryngeal Cancer in Montenegro Laryngeal cancer is the most common head and neck cancer. There might be many risk factors for laryngeal cancer. Smoking, especially cigarette smoking and alcohol are indisputable risk factors. The authors of this paper assessed the presumed risk factors in order to identify possible aetiological agents of the disease. A hospital-based case-control study was conducted. The study group consisted of 108 histologically verified laryngeal cancer patients and 108 hospital controls matched by sex, age (±3 years) and place of residence. Laryngeal cancer patients and controls were interviewed during their hospital stay using a structured questionnaire. According to multiple logistic regression analysis six variables were independently related to laryngeal cancer: hard liquor consumption (Odd Ratio/OR/=2.93, Confidence Interval/CI/95% = 1.17 to 7.31), consumption more than 2 alcoholic drinks per day (OR=4.96, CI 95% = 2.04 to 12.04), cigarette smoking for more than 40 years (OR=4.32, CI 95% = 1.69 to 11.06), smoking more than 30 cigarettes per day (OR=4.24, CI 95% = 1.75 to 10.27), coffee consumption more than 5 cups per day (OR=4.52, CI 95% = 1.01 to 20.12) and carbonated beverage consumption (OR=0.38, CI 95%=0.16 to 0.92). The great majority of laryngeal cancers could be prevented by eliminating tobacco smoking and alcohol consumption. Faktori rizika od karcinoma larinksa u Crnoj Gori Maligni tumori larinksa najčešći su tumori glave i vrata. Glavni faktori rizika od razvoja malignih tumora grkljana su pušenje i konzumiranje alkoholnih pića. Cilj rada bio je ispitivanje potencijalnih faktora rizika od nastanka malignih tumora larinksa. Sprovedena je studija slučaj-kontrola. Studijsku grupu činilo je 108 pacijenata s histološki verificiranim rakom larinksa i 108 kontrola individualno izjednačenih po spolu, dobi (± 3 godine) i mjestu stanovanja. Svi ispitanici su anketirani ciljanim epidemiološkim upitnikom a u analizi podataka korištena je multivarijantna logistička regresijska analiza. Koristeći se multivarijantnom logističkom regresijskom analizom, statistički značajnu povezanost s rakom larinksa dobili smo za sljedeće varijable: konzumiranje žestokih pića (omjer izgleda /OR/=2.93, interval pouzdanosti /CI/ 95% = 1.17 do 7.31), konzumiranje više od 2 alkoholna pića na dan (OR = 4.96, CI 95% = 2.04 do 12.04), konzumiranje cigareta duže od 40 godina (OR = 4.32, CI 95% = 1.69 do 11.06), konzumiranje više od 30 cigareta na dan (OR = 4.24, CI 95% = 1.75 do 10.27), konzumiranje više od 5 šalica kave na dan (OR = 4.52, CI 95% = 1.01 do 20.12) i konzumiranje gaziranih pića (OR = 0.38, CI 95% = 0.16 do 0.92). Obolijevanje zbog malignih tumora larinksa moglo bi se značajno smanjiti prestankom konzumiranja duhana i alkohola.

Prognostic relevance of CD105-assessed microvessel density in laryngeal carcinoma

OBJECTIVE: To examine immunohistochemical expression of CD105 among patients with laryngeal cancer and investigate the prognostic significance of CD105-assessed microvessel density (MVD). STUDY DESIGN: Cross-sectional study. SETTING: University hospital. SUBJECTS AND METHODS: The study comprised 80 patients with laryngeal squamous cell carcinoma who underwent complete excision. Clinicopathological data were collected retrospectively. Immunohistochemical analysis was performed with CD105 (endoglin) antibody. Positive-stained microvessels for CD105 were counted on hot spots of tumors at ×200 magnification. RESULTS: Average CD105-assessed MVD in considered laryngeal squamous cell carcinomas (SCCs) was 12.9 (SD 3.84). High expression of CD105 correlated significantly with advanced T (tumor) classification (P = 0.008), advanced TNM (tumor, node, metastasis) stage (P = 0.001), tumor recurrence (P = 0.001), and age ≥65 years (P = 0.026). The multivariate logistic regression showed that a high CD105+ MVD (odds ratio [OR] 4.27; P = 0.019) and advanced TNM stage (OR 3.72; P = 0.047) were independent markers of tumor recurrence. High MVD, advanced clinical stage, the presence of lymph node metastasis at the time of diagnosis, and age <66 years were associated with worse disease-free survival. Cox regression analysis revealed that expression of CD105 (P = 0.016) and advanced clinical stage (P = 0.05) were the independent factors for disease-free survival. CONCLUSION: The present results suggest that MVD evaluation with CD105 is a promising prognostic factor for the outcome of patients with laryngeal SCC. CD105-assessed MVD could help to identify patients with more aggressive disease and increased risk of developing malignancy recurrence after treatment.

Prevalence of Smoking and Other Smoking-Related Behaviours Among Students Aged 13 to 15 Years in Montenegro: Results from the Global Youth Tobacco Survey of 2008 The Global Youth Tobacco Survey (GYTS) is an international study that provides data on youth tobacco use for development of tobacco control programs. It is a school-based survey that uses a standardised methodology for sampling, core questionnaire items, training protocol, field procedures, and data management. This article reports the findings from a GYTS conducted in Montenegro in 2008, which included 5723 adolescents. More than 30 % of students aged 13 to 15 tried smoking, 5.1 % smoked cigarettes, and 3.6 % of students used tobacco products other than cigarettes. Four in 10 ever smokers started to smoke before the age of 10. More than half the students reported secondary smoke exposure at home. Almost all (96.5 %) current smokers bought cigarettes in a store. Two in 10 students owned an artifact with a cigarette or tobacco brand logo on it. The GYTS study has shown that there is an urgent need to introduce effective child-oriented smoking prevention programmes in early elementary school classes. These should be accompanied by public awareness campaigns on smoke-free homes. Zastupljenost pušenja i pušačke navike učenika dobi 13 do 15 godina u Crnoj Gori: rezultati globalnog istraživanja o potrošnji duhana kod mladih (GYTS) 2008 Globalno istraživanje o potrošnji duhana kod mladih (GYTS) međunarodna je studija koja pruža podatke o pušačkim navikama mladih i omogućava razvijanje programa čiji je cilj kontrola duhana. U ovom radu dati su rezultati istraživanja GYTS koje je provedeno 2008. godine u Crnoj Gori. Istraživanje GYTS provodi se u školama pri čemu je standardizirana metodologija za formiranje uzorka, upitnik, prikupljanje i obrada podataka. Ukupno su 5723 učenika sudjelovala u istraživanju. Više od 30 % učenika dobi 13 do 15 godina eksperimentiralo je cigaretama. U ovoj populaciji ima 5.1 % stalnih pušača cigareta, dok 3.6 % učenika koristi neke druge duhanske proizvode. Četiri od 10 pušača počela su pušiti prije desete godine života. Više od pola ispitanika odgovorilo je da su izloženi duhanskom dimu u svojoj kući. Gotovo svi pušači (96.5 %) najčešće kupuju cigarete u prodavaonicama. Dva od 10 ispitanika imaju neki predmet s logom proizvođača cigareta. Rezultati istraživanja pokazuju da je potrebno hitno pokrenuti preventivne programe kontrole duhana u nižim razredima osnovne škole. Također, potrebno je osmisliti i implementirati kampanje čiji je cilj podizanje svijesti građana o štetnosti pušenja u zatvorenom prostoru.
