Ultrasound and Radionuclide Studies of Urinary Extravasation with Hydronephrosis

AbstractTwo cases of urinary extravasation with hydronephrosis suspected by ultrasound studies and confirmed by radionuclide scans are reported. The value of ultrasound studies in patients with renal transplantation has been reported previously but studies in patients without transplantation are described rarely. Ureteral obstruction with hydronephrosis may cause urinary extravasation. The possibility of urinary extravasation or urinoma should be considered if a sonolucent lesion is seen around a hydronephrotic kidney and a renal scan with 99mtechnetium diethylenetriaminepentaacetic acid may confirm the finding.

Technical Considerations in B-mode Scanning with an Aspiration Transducer

Proper orientation of the B-mode ultrasonic aspiration transducer is important for accurate imaging during aspiration biopsy. The transducer extension axis must be perpendicular to the plane of the scanning arm for accurate representation of reflector locations. Single pass scans without compound angulation of the transducer are recommended when perpendicularity is not absolute. A simple adaptor to maintain perpendicularity is described.

Spleen Simulating Vascular Renal Mass in Left Renal Agenesis

In a case of left renal agenesis, the spleen was located in the posterior projection in the left renal region. The radionuclide renal angiogram and static renal images were misinterpreted as indicating a vascular renal mass. It is important to identify the kidney, spleen and heart in renal imaging.
