pharmacy and GPs with their own pharmacy incorporated changes to the medication regimen best (95.0% control, 70.7% intervention; HR = 0.74, 95% CI = 0.50–1.11). The reasons for changes were not documented in the CP or GP files. A transitional pharmaceutical care intervention alone does not improve incorporation of changes to medication regimens in CP and GP files. Continuity in pharmacotherapeutic care can only be ensured when CPs and GPs both receive complete and structured information on the discharge medication regimen and implement this information in their files so that it is readily available in case of a new change to the medication regimen or a transfer to another healthcare setting.

13 Research is an important part of clinicians’ work. However, I have gained the impression that successful researchers are sometimes valued over enthusiastic clinical teachers. Research success is easy to quantify whereas teaching quality may be not, particularly not objectively (Katarey 2012). This per se holds true, but that is where students again come into play. Courses are evaluated regularly, and who is better to judge a teacher’s performance than participating students? I, therefore, propose that universities should systematically identify qualified educators – based in part on student evaluations – and support this important clinical sub-population with the same resources as research staff. And, what enables clinical teachers to be good at their craft? In this student’s opinion, it is as Steve Jobs once put it, ‘‘The only way to do great work is to love what you do’’ (Stanford News Service 2005).

We describe a 73-year-old man who developed adrenal insufficiency 7 months after completing nivolumab therapy for advanced non-small cell lung cancer. He presented with non-specific symptoms of malaise and fatigue with an insidious 13.6 kilogram weight loss, prompting an evaluation for disease progression, which was negative. Subsequent evaluation revealed isolated adrenocorticotropin insufficiency as the aetiology, attributed to a delayed side effect of nivolumab therapy.
