Evaluation of the correlation between disc displacements and degenerative bone changes of the temporomandibular joint by means of magnetic resonance images

The aim was to evaluate the correlation between disc displacements and degenerative bone changes in magnetic resonance images (MRI) of 112 patients of both genders, with signs and symptoms of temporomandibular disorder. For this purpose, a calibrated examiner evaluated 224 MRI by assigning scores for the displacement of the disc and degenerative bone changes. Disc displacement was found in 58.42% of the temporomandibular joints (TMJs) evaluated. Anterior displacement of the disc with reduction was the most common, occurring in 67.18% cases of joints with disc displacement. Degenerative bone changes were observed in 53.94% of the TMJs analysed. There was significant correlation between disc displacement with reduction and condylar flattening, disc displacement without reduction and condylar flattening, disc displacement without reduction, and associated degenerative bone changes (flattening and erosion, flattening, osteophyte and erosion; flattening and osteophytes, erosion and sclerosis, flattening and sclerosis, flattening, osteophytes and sclerosis). The correlation between advanced cases of disc displacement and the occurrence of degenerative bone changes emphasises the importance of MRI for an accurate diagnosis and development of an appropriate treatment plan and in cases in which clinical examination is not sufficient for these purposes.

Heat treatment of pre-hydrolyzed silane increases adhesion of phosphate monomer-based resin cement to glass ceramic.

This study evaluated the influence of different forms of heat treatment on a pre-hydrolyzed silane to improve the adhesion of phosphate monomer-based (MDP) resin cement to glass ceramic. Resin and feldspathic ceramic blocks (n=48, n=6 for bond test, n=2 for microscopy) were randomly divided into 6 groups and subject to surface treatments: G1: Hydrofluoric acid (HF) 9.6% for 20 s + Silane + MDP resin cement (Panavia F); G2: HF 9.6% for 20 s + Silane + Heat Treatment (oven) + Panavia F; G3: Silane + Heat Treatment (oven) + Panavia F; G4: HF 9.6% for 20 s + Silane + Heat Treatment (hot air) + Panavia F; G5: Silane + Heat Treatment (hot air) + Panavia F; G6: Silane + Panavia F. Microtensile bond strength (MTBS) test was performed using a universal testing machine (1 mm/min). After debonding, the substrate and adherent surfaces were analyzed using stereomicroscope and scanning electron microscope (SEM) to categorize the failure types. Data were analyzed statistically using two-way test ANOVA and Tukeys test (=0.05). Heat treatment of the silane containing MDP, with prior etching with HF (G2: 13.15 ± 0.89a; G4: 12.58 ± 1.03a) presented significantly higher bond strength values than the control group (G1: 9.16 ± 0.64b). The groups without prior etching (G3: 10.47 ± 0.70b; G5: 9.47 ± 0.32b) showed statistically similar bond strength values between them and the control group (G1). The silane application without prior etching and heat treatment resulted in the lowest mean bond strength (G6: 8.05 ± 0.37c). SEM analysis showed predominantly adhesive failures and EDS analysis showed common elements of spectra (Si, Na, Al, K, O, C) characterizing the microstructure of the glass-ceramic studied. Heat treatment of the pre-hydrolyzed silane containing MDP in an oven at 100 °C for 2 min or with hot air application at 50 ± 5 ºC for 1 min, was effective in increasing the bond strength values between the ceramic and resin cement containing MDP.

Survival in patients with oral and maxillofacial diffuse large B-cell lymphoma

The purpose of this study was to determine the survival and prognostic factors of patients with diffuse large B-cell lymphoma (DLBCL) of the oral cavity and maxillofacial region. Retrospectively, the clinical records of patients with a primary diagnosis of DLBCL of the oral cavity and maxillofacial region treated at the A.C. Camargo Hospital for Cancer, São Paulo, Brazil, between January 1980 and December 2005 were evaluated to determine (A) overall survival (OS) at 2 and 5 years and the individual survival percentage for each possible prognostic factor by means of the actuarial technique (also known as mortality tables), and the Kaplan Meier product limit method (which provided the survival value curves for each possible prognostic factor); (B) prognostic factors subject to univariate evaluation with the log-rank test (also known as Mantel-Cox), and multivariate analysis with Coxs regression model (all the variables together). The data were considered significant at p≤0.05. From 1980 to 2005, 3513 new cases of lymphomas were treated, of which 151 (4.3%) occurred in the oral cavity and maxillofacial region. Of these 151 lesions, 48 were diffuse large B-cell lymphoma, with 64% for OS at 2 years and 45% for OS at 5 years. Of the variables studied as possible prognostic factors, multivariate analysis found the following variables have statistically significant values: age (p=0.042), clinical stage (p=0.007) and performance status (p=0.031). These data suggest that patients have a higher risk of mortality if they are older, at a later clinical stage, and have a higher performance status.

Which low-fusing porcelain glaze treatment technique is better to promote a vitreous surface on Y-TZP ceramic?

OBJECTIVE: to evaluated the influence of two types of low-fusing porcelain glaze application on the surface of a zirconia ceramics stabilized by yttrium (Y-TZP). METHODS: Y-TZP specimens were divided into five groups (n=5) according the surface treatments: control (CO), no treatment was performed; application of a thin low-fusing porcelain glass layer by the powder/liquid technique (GPL); GPL followed by 10% hydrofluoric acid (HF) etching (GPL-HF); glaze spray application (GS); GS followed by 10% HF etching (GS-HF). Roughness measurements contact angle, x-ray diffraction, energy-dispersive spectroscopy (EDS) and scanning electron microscope (SEM) analysis were performed. RESULTS: ANOVA and Tukey tests revealed that GPL had significantly lower contact angle values than the other groups (GPL:50.31o; GPL-HF: 72.73o; GS-HF: 81.73o; CO: 96.48o; GS:101.30o; p=0.001). GPL-HF presented a significant higher roughness (Ra/Rq: 2658.0/3367.0 ηm; p=0.001) than the other groups (GPL: 872.1/1162.0 ηm; GS-HF: 383.0/603.6 ηm; GS: 303.4/391.3 ηm; CO: 263.4/339.1 ηm). The X-ray diffraction analysis just tetragonal phase in all groups. EDS and SEM analysis show a highest amount of silica on GPL and GPL-HF surface. CONCLUSION: the application of the low-fusing porcelain glaze by the powder/liquid technique followed by the HF etching promoted a better surface to micromechanical and chemical adhesion.

Can low-fusing glass application affect the marginal misfit and bond strength of Y-TZP crowns?

To evaluate the effect of different surface treatments on the marginal misfit and retentive strength between Y-TZP crowns and an epoxy resin. Forty (40) epoxy resin (G10) abutments (height: 5mm, conicity: 60, finish line: large chamfer) with equal dimensions were milled and included in polyurethane to simulate the periodontal ligament. Next, 40 Y-TZP crowns (thickness: 1mm) were milled (Cerec in Lab) and randomly divided into four groups (n=10) according to the surface treatment: GS(glaze spray), GP(glaze powder/liquid), P(zirconia primer) and RS(tribochemical silica coating). The conditioned surfaces were cemented with dual self-adhesive cement, light cured and submitted to thermomechanical cycling (2x106, 100N, 4Hz, 5°/55°C). Marginal misfit was analyzed by a stereomicroscope and SEM. Retentive strength test was performed (1mm/min) until crown debonding. Glaze layer thickness was also performed to GS and GP groups. Marginal misfit data were analyzed by Kruskal Wallis and Dunn tests; one-way ANOVA and Tukey (5%) analyzed the tensile strength data. The marginal misfit of the GS (48.6±19.9μm) and GP (65.4±42.5μm) were statistically lower than the RS (96±62.9μm) and P (156±113.3μm) (p=0.001). The retentive strength of the GP (470.5±104.1N) and GS (416.8±170.2N) were similar to the P (342.1±109.7N), but statistically higher than those of the RS (208.9±110N). The GS and GP glaze layer was 11.64μm and 9.73μm respectively. Thus, glaze application promoted lower marginal discrepancy and higher retentive strength values than conventional techniques.

Bond strength of glass fiber posts submitted to different luting protocols

Objective: evaluate the effect of different luting protocols on the bond strength of glass fiber posts. Methods: Forty human canine teeth were randomly divided into four groups: Relyx U100/Endofill (R+E), Panavia F2.0/EndoFill (P+E), Relyx U100/Sealer 26 (R+S), Panavia F2.0/Sealer 26 (P+S). After the luting procedures, the samples were subjected to thermocycling (6.000 cycles; at 5±1 oC and 55±1 oC) and then sectioned in a precision cutting machine (Isomet 1000 Precision Saw) into 4 cuts of 2mm regarding to different deepness: Apical, Medium Apical, Medium Coronal, and Coronal. Next, the cuts were submitted to push-out test in a universal testing machine (EMIC) with load cell of 50N, at crosshead speed of 0.5mm/min. Results: They were obtained according to the different luting deepness (p≥0.05) (MPa): 1) apical – R+E (1.35±1.28), P+E (2.70±0.89), R+S (3.89±0.70), P+S (4.30±1.02); 2) medium apical – R+E (1.38±1.28), P+E (2.98±1.43), R+S (4.02±1.14), P+S (4.49±1.82); 3) medium coronal – R+E (2.46±0.73), P+E (3.72±0.78), R+S (4.85±0.71), P+S (6.14±0.73); 4) coronal R+E (5.59±1.08), P+E (5.74±1.06), R+S (6.71±1.07), P+S (7.40±1.51). Conclusion: There was a relationship between the luting protocol of the glass fiber post and the endodontic cement. Eugenol plays an important role inside the dentinal tubules by interfering negatively in the bond strength to root dentin.
