Mechanical behavior and color change of facial prosthetic elastomers after outdoor weathering in a hot and humid climate.

STATEMENT OF PROBLEM A common reason for replacing a maxillofacial prosthesis is the deterioration of its properties. PURPOSE The purpose of this study was to compare the effect of weathering in a hot and humid climate on the tear strength, tensile strength, modulus of elasticity, elongation percentage, and color of 3 maxillofacial materials. MATERIAL AND METHODS Three silicone materials were tested. Specimens were exposed to outdoor weathering for 6 months in a hot and humid environment. Tear and tensile strengths were measured with a universal testing machine before and after outdoor weathering. Color change was evaluated by recording L*, a*, and b* values at base line and after outdoor weathering with a spectrophotometer. The ΔE, ΔL*, Δa*, and Δb* of the specimens were calculated. Data were analyzed with 2-way ANOVA and the post hoc Scheffé test (α=.05). RESULTS For all tested elastomers, outdoor weathering decreased the tear strength and modulus of elasticity values significantly (P<.001). Also, weathering reduced percentage elongation values significantly (P<.05). Tensile strength values were significantly reduced for TechSil S25 and MED-4210 specimens only (P<.05). The effect of weathering on the color of pigmented specimens was greater than the acceptable value (ΔE>3.0). The color change (ΔE) of pigmented specimens ranged from 4.31 to 6.68. A-2186 silicone elastomer experienced the greatest color changes (P<.05). Within nonpigmented specimens, none of the tested silicone elastomers showed significant color changes (P>.05). CONCLUSIONS Outdoor weathering in a hot and humid climate adversely affected the properties of silicone elastomers. The heat-polymerized TechSil S25 elastomer showed better mechanical durability and color stability compared with the room-temperature polymerized A-2186 and MED-4210 materials. TechSil S25 showed the greater values of tear and tensile strengths and elongation of specimens exposed to outdoor weathering conditions. It also showed the least amount of color change among the pigmented specimens exposed to outdoor weathering.

Cervical Interfacial Bonding Effectiveness of Class II Bulk Versus Incremental Fill Resin Composite Restorations.

Cervical interfacial bonding quality has been a matter of deep concern. The purpose of this study was to analyze microtensile bond strength (MTBS) and cervical interfacial gap distance (IGD) of bulk-fill vs incremental-fill Class II composite restorations. Box-only Class II cavities were prepared in 91 maxillary premolars (n = 7) with gingival margin placement 1 mm above the cementoenamel junction at one side and 1 mm below it on the other side. Eighty-four maxillary premolars were divided into self-etch and total-etch groups and further subdivided into six restorative material subgroups used incrementally and with an open-sandwich technique: group 1, Tetric Ceram HB (TC) as a control; group 2, Tetric EvoFlow (EF); group 3, SDR Smart Dentin Replacement (SDR); group 4, SonicFill (SF); group 5, Tetric N-Ceram Bulk Fill (TN); and group 6, Tetric EvoCeram Bulk Fill (TE). Groups 2-6 were bulk-fill restoratives. Tetric N-Bond Self-Etch (se) and Tetric N-Bond total-etch (te) adhesive were used in subgroups 1-5, whereas AdheSE (se) and ExciTE F (te) were used in subgroup 6. In an additional group, Filtek P90 Low Shrink Restorative (P90) was used only with its corresponding self-etch bond. The materials were manipulated, light-cured (1600 mW/cm(2)), artificially aged (thermal and occlusal load-cycling), and sectioned. Two microrods/restoration (n = 14/group) were tested for MTBS at a crosshead-speed of 0.5 mm/min (Instron testing machine). Fracture loads were recorded (Newtons), and MTSBs were calculated (Megapascals). Means were statistically analyzed by the Kruskal-Wallis test, Conover-Inman post hoc analysis for MTBS (multiple comparisons), and Mann-Whitney U test for IGD. The ends of the fractures were examined for failure mode. One microrod/restoration (n = 7/group) was investigated by scanning electron microscopy (×1200) for IGD. MTBS values for SF/te, P90 in enamel, and TC+SDR/te in enamel and cementum were significantly higher compared with those for the control TC/te and TC/se in cementum. Most of the failures were mixed. IGDs were generally smaller at enamel margins, and the smallest IGDs were found in P90 at both enamel and cementum margins. Bulk-fill and silorane-based composites might provide better cervical interfacial quality than incremental-fill restorations.

PMMA denture base material enhancement: a review of fiber, filler, and nanofiller addition

This paper reviews acrylic denture base resin enhancement during the past few decades. Specific attention is given to the effect of fiber, filler, and nanofiller addition on poly(methyl methacrylate) (PMMA) properties. The review is based on scientific reviews, papers, and abstracts, as well as studies concerning the effect of additives, fibers, fillers, and reinforcement materials on PMMA, published between 1974 and 2016. Many studies have reported improvement of PMMA denture base material with the addition of fillers, fibers, nanofiller, and hybrid reinforcement. However, most of the studies were limited to in vitro investigations without bioactivity and clinical implications. Considering the findings of the review, there is no ideal denture base material, but the properties of PMMA could be improved with some modifications, especially with silanized nanoparticle addition and a hybrid reinforcement system.

Marginal Integrity of Bulk Versus Incremental Fill Class II Composite Restorations

Bulk-fill composites have been introduced to facilitate the placement of deep direct resin composite restorations. This study aimed at analyzing the cervical marginal integrity of bulk-fill vs incremental and open-sandwich class II resin composite restorations after thermomechanical cycling using replica scanning electron microscopy (SEM) and ranking according to the World Dental Federation (FDI) criteria. Box-only class II cavities were prepared in 91 maxillary premolars with the gingival margin placed 1 mm above and below the cemento-enamel junction. Eighty-four premolars were divided into self-etch and total-etch groups, then subdivided into six restorative subgroups (n=7): 1-Tetric Ceram HB (TC) was used incrementally and in the open-sandwich technique with 2-Tetric EvoFlow (EF) and 3-Smart Dentin Replacement (SD). Bulk-fill restoratives were 4-SonicFill (SF), 5-Tetric N-Ceram Bulk Fill (TN), and 6-Tetric EvoCeram Bulk Fill (TE). In subgroups 1-5, Tetric N-Bond self-etch and Tetric N-Bond total-etch adhesives were used, whereas in subgroup 6, AdheSE self-etch and ExciTE F total etch were used. One more group (n=7) was restored with Filtek P90 Low Shrink Posterior Restorative (P9) only in combination with its self-etch P90 System Adhesive. Materials were manipulated and light cured (20 seconds, 1600 mW/cm(2)), and restorations were artificially aged by thermo-occlusal load cycling. Polyvinyl-siloxane impressions were taken and poured with epoxy resin. Resin replicas were examined by SEM (200×) for marginal sealing, and percentages of perfect margins were analyzed. Moreover, samples were examined using loupes (3.5×) and explorers and categorized according to the FDI criteria. Results were statistically analyzed (SEM by Kruskal-Wallis test and FDI by chi-square test) without significant differences in either the replica SEM groups (p=0.848) or the FDI criteria groups (p>0.05). The best SEM results at the enamel margin were in TC+EF/total-etch and SF/total-etch and at the cementum margins were in SF/total-etch and TE/self-etch, while the worst were in TC/self-etch at both margins. According to FDI criteria, the best was TE/total-etch at the enamel margin, and the poorest was P9/self-etch at the cementum margin. Groups did not differ significantly, and there was a strong correlation in results between replica SEM and FDI ranking.

In vitro shear bond strength of Y-TZP ceramics to different core materials with the use of three primer/resin cement systems.

STATEMENT OF PROBLEM Durability of the bond between different core materials and zirconia retainers is an important predictor of the success of a dental prosthesis. Nevertheless, because of its polycrystalline structure, zirconia cannot be etched and bonded to a conventional resin cement. PURPOSE The purpose of this in vitro study was to compare the effects of 3 metal primer/resin cement systems on the shear bond strength (SBS) of 3 core materials bonded to yttria-stabilized tetragonal zirconia polycrystalline (Y-TZP) ceramic retainers. MATERIAL AND METHODS Zirconia ceramic (Cercon) disks (5×3 mm) were airborne-particle abraded, rinsed, and air-dried. Disk-shaped core specimens (7×7 mm) that were prepared of composite resin, Ni-Cr, and zirconia were bonded to the zirconia ceramic disks by using one of 3 metal primer/cement systems: (Z-Prime Plus/BisCem, Zirconia Primer/Multilink Automix, or Clearfil Ceramic Primer/Clearfil SA). SBS was tested in a universal testing machine. Stereomicroscopy was used to evaluate the failure mode of debonded specimens. Data were analyzed using 2-way ANOVA and post hoc analysis using the Scheffe procedure (α=.05). RESULTS Clearfil SA/Clearfil Ceramic Primer system with an Ni-Cr core yielded the highest SBS value (19.03 MPa), whereas the lowest SBS value was obtained when Multilink Automix/Zirconia Primer system was used with the zirconia core group (4.09 MPa). Differences in mean SBS values among the cement/primer groups were statistically significant, except for Clearfil SA and BisCem with both composite resin and zirconia cores. Differences in mean SBS values among the core subgroups were not statistically significant, except for zirconia core with BisCem, Multilink, and Clearfil SA. The predominant failure mode was adhesive, except for Clearfil SA and BisCem luting agents with composite resin cores, which displayed cohesive failure, and Multilink Automix with a composite resin, core as well as Clearfil SA with Ni-Cr cores, where the debonded specimens of each group displayed a mixed (adhesive/cohesive) failure pattern. CONCLUSIONS Use of the Clearfil SA/Clearfil Ceramic Primer system, based on methacryloyloxydecyl dihydrogen phosphate (MDP), increased the bond strength of Y-TZP ceramics to core materials.

Effectiveness of Coating Acrylic Resin Dentures on Preventing Candida Adhesion

PURPOSE The aim of this study was to prevent the adhesion of C. albicans on acrylic resin dentures by modifying their surfaces. MATERIALS AND METHODS Ninety acrylic resin plates were divided into three groups. Group I: conventionally processed acrylic resin plates. Group II: plates painted with 2-Octyl Cyanoacrylate adhesive. Group III: plates painted with Adper Single Bond Adhesive. All specimens were immersed separately in containers filled with artificial saliva that contained C. albicans and then incubated for 11 days at 37°C. Three methods of evaluation were used to count the adhered Candida: direct culture, slide count, and serial dilutions. RESULTS C. albicans in 1/10, 1/10², and 1/10³ dilutions showed overgrowth in group I, while overgrowth was noted only with 1/10 dilution in group III. For group III, mean colony numbers of 123, 22, 3.4, and 0 were found for the 1/10², 1/10³, 1/10⁴, and 1/10⁵ dilutions, respectively. Regarding the slide counts, group I showed a mean fungal count of 166 compared to 40 for group III with 1/10 dilution, 21 compared to 9 with 1/10³ dilution, 8.6 compared to 0.7 with 1/10³ dilution, and 1.2 compared to 0 with 1/10⁴ dilution. No plates in group II showed any candidal colonies regardless of the method of evaluation (0%). These differences were statistically significant (p < 0.0001). CONCLUSION Coating the acrylic resin dentures with Adper Single Bond Adhesive was effective in reducing C. albicans adhesion to dentures, while coating with 2-Octyl Cyanoacrylate adhesive completely inhibited such adhesion.

Factors affecting student participation in extra-curricular activities: A comparison between two Middle Eastern dental schools

Objective This study was conducted to assess the level of participation of dental undergraduate students in extracurricular activities (ECAs) and the factors affecting this participation. Methods The study included dental students enrolled in undergraduate programs at the Faculty of Dentistry, Alexandria University, Egypt, and the College of Dentistry, University of Dammam, Saudi Arabia. A questionnaire was developed to collect background information about students, their participation in ECAs, and time allocated for these activities. Students were asked about their perceptions of the relationship between ECAs and academic studies, and their reasons for participating in and satisfaction with ECAs. Results The study included 199 students from Alexandria and 146 students from Dammam, with response rates of 99.5% and 73%, respectively. The percentages of those reporting ECA participation were 27.1% and 43.8%, respectively, mostly in community service, sports, and social activities. About 60% of students did not think that ECAs affected their studies, although the perceived difficulty of balancing ECAs and academics was associated with lower odds of participation (odds ratio = 0.51). Most students participated in ECAs to socialize and make friends, and the majority was dissatisfied with school-organized ECAs (52% and 59%, respectively). Gender and/or perceived relation between ECAs and academic studies affected actual participation in ECAs in one school but not the other. Conclusions ECA participation among these students was low. Gender and perception of ECAs in relation to academic studies affected ECA participation differently in the two schools. Better planning and management of ECAs that incorporate students’ preferences and reasons for participation is needed. Gender issues and the relationship between ECAs and academic performance should be addressed in relation to school and social characteristics.

Microtensile bond strength, 4-point bending and nanoleakage of resin-dentin interfaces: Effects of two matrix metalloproteinase inhibitors

Chronic degradation of hybrid layer collagen by matrix metalloproteinases (MMPs) jeopardizes resin-dentin interfacial integrity and limits the durability of dental restorations. The 4-point bending strength (BS) is a valid but uncommon method of testing the mechanical behavior of resin-dentin interfaces. The present study aims to analyze the influence of two matrix metalloproteinase inhibitors on microtensile bond strength (µTBS), BS and nanoleakage. A total of 48M were divided into three groups according to bonding procedure. Teeth were horizontally sectioned to produce a flat dentin surface. In the control group, etch-and-rinse Prime&Bond One (Dentsply) bonding was used; in the self-etch group, methacryloyloxydodecylpyridinium bromide (MDPB)-containing Clearfil SE Protect (Kuraray) was used; and in the benzalkonium chloride (BAC)-etch group, BAC-etchant (Bisco) was used. A Ceram.X-One (Dentsply) composite was built as three successive layers and was light-cured. Samples were sectioned to produce microrods that were randomly divided into two groups for analysis at baseline and after 6 months of water immersion (n = 32), plus one slab for nanoleakage analysis (n = 8) via scanning electron microscopy (SEM) and digital image analysis (Fiji). Data were analyzed using the Weibull distribution and a mixed-model ANOVA with a post hoc Tukey test. All groups showed deterioration of the initial bonds. The self-etch group had a worse baseline µTBS than the control but had the best BS after aging. BAC-etch did not improve bond stability of etch-and-rinse adhesive. The µTBS and BS test results after aging were moderately correlated. Mixed fractures prevailed with regard to µTBS, whereas adhesive fractures dominated with regard to BS. Nanoleakage was not eliminated in any group and increased after aging. MDPB self-etch resisted bond degradation better than etch-and-rinse adhesives, even after BAC-etching. Integrating BS in studies of µTBS and nanoleakage might provide more clinically relevant outcomes for predicting the performance of dental adhesives.

Degradation Potential of Bulk Versus Incrementally Applied and Indirect Composites: Color, Microhardness, and Surface Deterioration

This study investigated the color stability and microhardness of five composites exposed to four beverages with different pH values. Composite discs were produced (n=10); Filtek Z250 (3M ESPE) and Filtek P90 (3M ESPE) were applied in two layers (2 mm, 20 seconds), and Tetric N-Ceram Bulk Fill (TetricBF, Ivoclar Vivadent) and SonicFill (Kerr) were applied in bulk (4 mm) and then light cured (40 seconds, Ortholux-LED, 1600 mW/cm2). Indirect composite Sinfony (3M ESPE) was applied in two layers (2 mm) and cured (Visio system, 3M ESPE). The specimens were polished and tested for color stability; ΔE was calculated using spectrophotometer readings. Vickers microhardness (50 g, dwell time=45 seconds) was assessed on the top and bottom surfaces at baseline, 40 days of storage, subsequent repolishing, and 60 days of immersion in distilled water (pH=7.0), Coca-Cola (pH=2.3), orange juice (pH=3.75), or anise (pH=8.5) using scanning electron microscopy (SEM). The materials had similar ΔE values (40 days, p>0.05), but TetricBF had a significantly greater ΔE than P90 or SF (40 days). The ΔE was less for P90 and TetricBF than for Z250, SonicFill, and Sinfony (60 days). Repolishing and further immersion significantly affected the ΔE (p<0.05) except for P90. All composites had significantly different top vs bottom baseline microhardnesses. This was insignificant for the Z250/water, P90/orange juice (40 days), and Sinfony groups (40 and 60 days). Immersion produced variable time-dependent deterioration of microhardness in all groups. Multivariate repeated measures analysis of variance with post hoc Bonferroni tests were used to compare the results. ΔE and microhardness changes were significantly inversely correlated at 40 days, but this relationship was insignificant at 60 days (Pearson test). SEM showed degradation (40 days) that worsened (60 days). Bulk-fill composites differ regarding color-stability and top-to-bottom microhardness changes compared with those of other composites. P90 showed better surface degradation resistance. In conclusion, bulk-fill composites are not promising alternatives to incremental and indirect composites regarding biodegradation.

Platelet-Rich Plasma Gel Combined with Bovine-Derived Xenograft for the Treatment of Dehiscence Around Immediately Placed Conventionally Loaded Dental Implants in Humans: Cone Beam Computed Tomography and Three-Dimensional Image Evaluation.

PURPOSE This study clinically and radiographically investigated the potential of platelet-rich plasma (PRP) gel combined with bovine-derived xenograft to treat dehiscence defects around immediate dental implants. MATERIALS AND METHODS This study was performed on 32 sites from 16 patients who each received an immediate implant for a single tooth replacement at a maxillary anterior or premolar site. Patients were divided into two groups according to the augmented materials used. One group received an immediate implant and filling of defects using a PRP gel plus bovine-derived xenograft. The other group received an immediate implant and filling of defects with a bovine-derived xenograft without PRP gel. Cone beam computed tomography (CBCT) was taken before placement, and at 6 and 12 months postsurgery. RESULTS Both treatment procedures resulted in significant improvements for the primary outcome regarding bone fill, as well as the marginal bone level. In addition, statistically significant differences were found in the bone density for the combined therapy compared with sites treated with bovine-derived xenografts alone (P ≤ .01). CONCLUSION Autogenous PRP gel combined with bovine-derived xenograft demonstrated superiority to the bovine-derived xenograft alone, which suggested that it could be successfully applicable for the treatment of dehiscence around an immediate dental implant. Moreover, CBCT can be used to measure dehiscence and to assess bone thickness along the implant.
