Gigantomastia and Macroprolactinemia Responding to Cabergoline Treatment: A Case Report and Minireview of the Literature

Background. Macroprolactinemia is defined as predominance of high molecular weight prolactin forms in the circulation. Although macroprolactin is considered as a biologically inactive molecule, some authorities suggest treatment in symptomatic cases. Gigantomastia is defined as excess breast tissue and most cases in the literature were treated by surgical intervention. Case. A 44-year-old woman was admitted to our clinic with gigantomastia and galactorrhea. The patient had a demand for surgical therapy. In laboratory examination, she had hyperprolactinemia and macroprolactinemia. Pituitary imaging revealed 6 mm microadenoma in right side of the hypophysis. Since she was symptomatic, cabergolin treatment was started. Macroprolactin became negative, breast circumference decreased significantly, and galactorrhea resolved after treatment. Conclusion. Gigantomastia might be the presenting symptom in patients with macroprolactinemia. In these patients medical treatment with cabergoline may be used initially as an alternative to surgical approach.

Thyroid cancer surgery in the elderly: a comparative study of 1176 patients

Thyroid cancer is the most common malignancy of the endocrine system in the United States. Thyroid cancer is predicted to be the fourth most common cancer in the United States in 2030. Although the increase in prevalence is mostly due to increased use of imaging modalities and due to the higher sensitivity of ultrasonography by some authorities, there are implications for a real increase. Due to the proven correlation between thyroid nodules and aging, improved disease detection methods, and longer life expectancy, it is likely to expect an increase in demand for thyroid surgical procedures in elderly patients. However, debate exists about the feasibility and safety of thyroid surgery in this subgroup of patients.

Ultrasonographical and cytological features of exophytic thyroid nodules: do exophytic nodules pretend to be malignant?

. Material ve Method ➢ Fourty-four exophytic and 34 non-exophytic thyroid nodules in which fine needle aspiration biopsy was indicated throughout 3 months were evaluated prospectively. Mean nodule size was similar in two groups (18.83±8.71 mm and 15.28±7,57 mm, p=0.173). The ratio of presence of peripheral hypoechoic halo and marginal irregularity was also similar in both group (p=0.512 and p=0.153, respectively). Microcalcification was present in 21.4% and 29.4% of exophytic and non-exophytic nodules, respectively (p=0.424). Macrocalcification was detected in 4.5% of exophytic and 11.8% of non-exophytic nodules (p=0.111). 47.6% of exophytic nodules was hypoechoic and 52.4% was isoechoic. 47.1% of non-exophytic nodules was hypoechoic and 52.9% was isoechoic. Color flow doppler pattern was defined as non-vascular, peripheral, central, or of mixed type and was similar in both groups (p=0.138). Cytopathologic results of exophytic nodules were 75% benign, 4.3% follicular lesion or atypia with undetermined significance, 2.3% suspicious for malignancy, 2.3% malign, and 15.9% non-diagnostic. In non-exophytic group, 79.4% was benign and 20.6% was non-diagnostic (p=0.497).
