Analysis of Sexual Function in Kidney Transplanted Men

INTRODUCTION Sexual dysfunction among renal failure and kidney transplant patients remains controversial. The aim of this study was to evaluate sexual functions of men on hemodialysis compared with patients undergoing kidney transplantation. MATERIALS AND METHODS Our study was based on 36 end-stage renal disease (ESRD) patients undergoing hemodialysis versus 32 kidney transplanted patients. A control group was composed of 23 healthy patients. The patients underwent an anamnesis, a physical examination, and the International Index of Erectile Function about sexual performance. Statistical analysis was performed by Students t-test or the chi-square test with the level of significance set at P < .05. Data are reported as mean values +/- standard error of the means. RESULTS The mean scores of the control, ESRD, and transplanted group were, respectively: for erectile function, 27.4 +/- 0.5, 22.4 +/- 1.3, 23.4 +/- 1.3; for orgasmic function, 9.5 +/- 0.1, 7.6 +/- 0.5, 8.9 +/- 0.5; for sexual desire function 9.4 +/- 0.1, 7.1 +/- 0.3, 9.0 +/- 0.5; for intercourse satisfaction 12.8 +/- 0.3, 9.4 +/- 0.7, 11.0 +/- 0.7; and for satisfaction related to sexual life 9.2 +/- 0.2, 7.7 +/- 0.3, 8.6 +/- 0.6, proving that there were significant differences regarding orgasmic function, sexual desire, and intercourse satisfaction. CONCLUSION It was possible to conclude from our study that kidney transplants do improve sexual function of patients with ESRD on hemodialysis.

Effects of acupuncture and electroacupuncture on estradiol-induced inflammation and oxidative stress in health rodents

PURPOSE To investigate the effects of classical acupuncture (Ac) and electroacupuncture (EAc) on estradiol-induced inflammation and oxidative stress in health rodents. METHODS Twenty-four eight-week old female rats were treated with estradiol valerate (EV) 4.0 mg i.m. single dose and randomly assigned to four groups (n=6): G1(control), G2 (Ac), G3 (EAc 2 Hz) and G4 (EAc 100 Hz). After 60 days all rats were anesthetized with chloral hydrate 10% (0.1 ml/30 g weight of the animal) and submitted to Ac/EAc for twenty minutes. The procedures were repeated on days three, five, seven and nine of the study. The equivalent of the human right ST-36 (Zusanli) and SP-6 (Sanyinjiao) acupoints were chosen for needling and electrical stimulation. On the 10th day of the experiment, all rats were anesthetized for collection of blood and tissues (ovaries) samples for biochemical analysis and histological examination. RESULTS Glutathione (GSH) and malonaldehyde (MDA) concentrations increased significantly in all groups (plasma and ovary) while myeloperoxidase (MPO) activity decreased significantly in all groups compared with control group (G1). CONCLUSIONS Both classical acupuncture and electroacupuncture decrease systemic and local oxidative stress and ovary inflammation in healthy rats exposed to estrogenic stimulation. EAc enhances lipid peroxidation at systemic and local levels in female rats exposed to estrogenic stimulation.

Effects of misoprostrol on pentylenetetrazol-induced seizures in mice

Os efeitos do misoprostrol, um analogo da prostaglandina E, sobre convulsoes induzidas por pentilenotetrazol (PTZ) foram estudados em camundongos. Misoprostrol (200-800 mg/Kg) administrado por via subcutânea 45 minutos antes da dose subconvulsiva de PTZ (30 mg/Kg, i.p.) provocou crises tonico-clonicas (30 a 100%) de maneira dose-dependente e mortalidade em camundongos. Na dose de 300 g/Kg, s.c., o pre-tratamento com misoprostrol diminuiu significativamente (p<0,05) o periodo de latencia da primeira convulsao bem como a mortalidade induzida por uma dose convulsiva de PTZ (60 mg/Kg, i.p.). Nesta dose o misoprostrol diminuiu 21 % e 36% os valores de CD50 e de LD50 do PTZ, respectivamente.The effects of prostaglandin E-analogue misoprostol on the susceptibility to pentilenetetrazol (PTZ)-induced seizures were examined in mice. Misoprostol (200-800 micrograms/kg), given subcutaneously 45 min before the subconvulsive dose of PTZ (30 mg/kg, i.p) provoked dose-dependent clonic-tonic seizures (30 to 100%) and mortality in mice. At 300 g/kg, s.c., misoprostol pretreatment significantly (p < 0.05) lowered the onset latency to first convulsion as well as the latency to mortality induced by a convulsive dose of PTZ (60 mg/kg, i.p.). At this dose misoprostol was found to lower the CD50 and Ld50 values for PTZ by 21% and 36% respectively. The results suggest that prostaglandins are likely to lower the threshold for convulsions.

Uterine Cavity Calcifications: A Report of 7 Cases and a Systematic Literature Review

We report 7 uterine cavity calcification cases and systematically review the literature on cases presenting this finding. In our series of cases, the mean age of patients was 31.6 ± 5.4 years, with an infertility period ranging from 2 to 8 years. None of our patients had a history of intrauterine contraceptive device use, and 3 had undergone operative termination of pregnancy. Diagnostic and therapeutic hysteroscopy with complete removal of the calcifications was performed in all patients with fertility restoration in 5 of them. Our review included 35 articles published from 1989 to 2013 reporting endometrial calcification cases, with a total of 85 cases. Analyzed variables included patient age, abortion antecedents, clinical presentation, diagnostic method, treatment, and outcome. The mean age of the analyzed cases was 34.3 years, ranging from 19 to 62 years. Abortion antecedents were present in most patients (76.5%). The most frequently used diagnostic methods were ultrasound (88.5% of articles) and hysteroscopy (85.7%). Regarding the clinical presentation, infertility was the most common feature (72.9% of cases) followed by menstrual abnormalities (30.3%). Hysteroscopic removal, whether alone or combined with other techniques, was the most used method for the removal of osseous tissue (70.6% of cases) followed by curettage (27.1%). After removal of the bony fragments, 55.6% of the infertile patients achieved a pregnancy. A total of 14 cases reported relief of symptoms other than infertility. All of the data concerning outcome were stratified according to the treatment modality used. We conclude that endometrial calcifications in the uterine cavity are a relevant cause of secondary infertility, which can generally be properly diagnosed and treated through hysteroscopy, with successful restoration of fertility in most cases.

Comparação entre ultrassonografia transvaginal e histerossonografia na avaliação de pacientes com sangramento uterino anormal

OBJECTIVE: To comparatively evaluate the effectiveness of transvaginal ultrasonography (TVUS) and sonohysterography (SHG) in the assessment of endometrial diseases in women with abnormal uterine bleeding. MATERIALS AND METHODS: Cross-sectional study with 30 patients, aged from 29 to 71 years, 21 (70%) of them premenopausal and 9 (30%) postmenopausal. Saline solution (at 9%) was utilized as contrast agent for SHG. The MacNemar test was utilized for comparison of diagnostic studies. RESULTS: Hysteroscopy diagnosed 18 cases (60%) of intracavitary alterations, and 10 polyps (33.3%). TVUS demonstrated 83.3% sensitivity and specificity, and SHG showed 94.4% sensitivity and 91.6% specificity. The MacNemar test showed similar sensitivity (p = 0.500) and specificity (p = 1.000) between TVUS and SHG in the detection of endometrial diseases. In the diagnosis of polyp, SHG showed the highest sensitivity (90.9% vs. 27.3%; p = 0.016) with similar specificity (89.5% vs. 94.7%; p = 1.000). CONCLUSION: SHG and TVUS present a good predictive value for endometrial diseases in patients with abnormal uterine bleeding. SHG and TVUS present similar sensitivity and specificity in the detection of such diseases, but SHG is more sensitive in the detection of polyps.

Analysis of coverage and of the pap test exams not retired of a Basic Health Unit

To analyze the coverage and the colpocytology exams that were not collected from a Health Center. This is a retrospective documentary study, in which the data were evaluated for coverage of 2007 and all 225 tests that were performed but were not collected by the patients unit until January 2008. Statistical analysis was performed using the frequency of the studied variables. The average monthly number of tests was 102.6 examinations. The coverage of the examination in 2007 was 11.22% among women with 25 to 59 years of age. Of the 938 tests conducted between February and November 2007, 225 (23.98%) women did not receive the result. Most women (67.5%) who performed the examination and had not returned were 30 years old or younger. The womens attitude of not returning to collect their exam results increases the difficulty of follow up, and providing comprehensive and continuity of care, contributing with an intervention in advanced stages of the disease.To analyze the coverage and the colpocytology exams that were not collected from a Health Center. Assessment of endothelial function by flow-mediated dilation of the brachial artery in adolescents with a history of preeclampsia or a normotensive pregnancy

Objectives: to determine the prevalence of endothelial dysfunction and its association with a history of mild and severe preeclampsia in adolescents. Methods: a cross-sectional study was carried out at the MEAC-UFC with 103 primiparous adolescents postpartum. The assessment of endothelial function was performed by way of flow-mediated dilatation of the brachial artery. Variables (age, body mass index, gestational age at delivery, systolic and diastolic blood pressure and flow-mediated dilation) were compared between groups. p<0.05 was considered to be statistically significant. Results: twenty-four (23.3%) patients had preeclampsia (PE): 11 mild and 13 severe. The overall prevalence of endothelial dysfunction was 23.3% (21.5% of patients with normotensive pregnancies and 29.2% of the PE patients: 18.2% of those with mild PE and 38.5% of those with severe PE). The figures were statistically significant for systolic blood pressure, p=0.007. Conclusions: patients with a history of PE have higher systolic blood pressure than patients with a history of normotensive pregnancy, but did not have more endothelial dysfunction.
