8 Ankle-Brachial Index (ABI) Certification/Recertification Theresa DeVeaux, RN, MS, ACNP, CV, CCRN, Debra Kohlman-Trigoboff, RN, MS, ACNP-BC, CVN, Marge Lovell, RN, CCRC, CVN, BEd, Med, Carolyn Robinson, and Marie Rossi, BS, RN The SVN has established a Registry of Vascular Nurses with expertise in Peripheral Arterial Disease (PAD) to provide education and ABI training sessions to health care professionals. Training may occur in areas such as hospitals, outpatient settings, long term care centers and professional societal meetings. The Registry consists of SVN members who have completed a comprehensive PAD education and ABI train-the-trainer workshop. Your namewill be added to the SVNABI Registry with successful completion of this session at the Annual Convention. Abstract 9 Perioperative Participation in OSA9 Perioperative Participation in OSA Gail Davis, MSN, CNS, APRN-BC, CCRN, and Amy Cooper More than 12 million Americans could have OSA (Obstructive Sleep Apnea). This presentation will look at a proactive approach that initiated a preoperative screening assessment to establish potential risk in surgical patients. Our division took a proactive approach at ensuring OSA protocols were tediously followed in the preoperative holding area and recovery room. Abstract 10 Applying a Conceptual Framework to Guide Advocacy Patty Flanagan, MSN, RN-BC10 Applying a Conceptual Framework to Guide Advocacy Patty Flanagan, MSN, RN-BC Applying a conceptual framework to guide advocacy is a program that will take you through each step in applying a conceptual framework to guide advocacy. The educational program will utilize examples, as they move through the stages of advocacy to demonstrate tangibility of the conceptual framework. And I will demonstrate, although the stages are conceptually sequential, in practice they are generally simultaneous. Abstract 11 Preoperative Regional Anesthesia for Postoperative Pain Management11 Preoperative Regional Anesthesia for Postoperative Pain Management Gail Davis, MSN, CNS, APRN-BC, CCRN Financial impact of health care has motivated hospitals to creatively train and staff divisions to facilitate higher acuity and flow of patient volume without increasing full time employees. Divisions within perianesthesia care unit (PACU) previously trained staff to preop patients and others to recovery. Our unit spans two floors and six divisions. The computerized documentation system is different from the rest of the hospital. Asking for nursing support from the hospital when resources are stretched could not occur. They did not have access and are not familiar with our system. Economic changes motivated changes in staffing structure and training. Abstract 12 Stroke Center Readiness in the PACU Sandra Filla, BSN, CCRN, RN12 Stroke Center Readiness in the PACU Sandra Filla, BSN, CCRN, RN Our institution is certified as the first Comprehensive Stroke Center in Missouri. This recognizes the commitment of our hospital to deliver complex stroke care to patients in the hospital and the community. During certification there were opportunities noted for improvement. The PeriAnesthesia Care Unit (PACU) collaborated with the neurosciences department to ensure appropriate education for RNs was completed and patient education was completed to prepare for the next survey. Audits of charting were done to measure compliance. Abstract 13 Methods of Optimizing Limb Preservation in the Endovascular Era: A Balanced Approach13 Methods of Optimizing Limb Preservation in the Endovascular Era: A Balanced Approach R. Clement Darling, III, MD Abstract 14 Applying Evidence-based Guidelines to Prevent, Diagnose and Treat Post Thrombotic Syndrome14 Applying Evidence-based Guidelines to Prevent, Diagnose and Treat Post Thrombotic Syndrome M. Eileen Walsh, PhD, APN, RN-BC, FAHA Current evidence-based guidelines to prevent, diagnose and treat Post-thrombotic Syndrome will be described. Evidence related to primary prevention, anticoagulation, compression therapies, including pumps and stockings, thrombolysis, endovascular and surgical interventions, and the role of exercise interventions will be discussed. Abstract 15 Infectious Aortitis: A Case Study Deidre Dennison, RN, BS15 Infectious Aortitis: A Case Study Deidre Dennison, RN, BS Infectious aortitis is a serious disease that often presents as an infected aneurysm. This case study will examine the clinical presentation, findings, treatment, and outcome of a 79 year old woman with Salmonella-infected aneurysm. A late-course finding of this case was that the patient had a confirmed case of Salmonella infection approximately two years prior to aneurysmal symptoms. This begs the question which came first, the infection or the aneurysm? Abstract 16 Anchors Away: Keeping the Endograft in Place Melody Heffline, MSN, RN, ACNS-BC, ACNP-BC16 Anchors Away: Keeping the Endograft in Place Melody Heffline, MSN, RN, ACNS-BC, ACNP-BC This session will discuss the use of the Endoanchor to reduce the risk of endograft migration and late endograft failure in

Opioid Administration Post-operatively: Reviewing the Process and Identifying Standards and Expectations for Safe, Effective Care

Gail Davis; Elizabeth Cox

As healthcare faces demands to decrease length of stay and increase patient satisfaction, hospital staff seeks to streamline the discharge process. Evidence suggests an interdisciplinary discharge process results in increased nurse satisfaction, reduced incidence of medical errors and hospital acquired infections. The purpose of the Vascular Surgery and Transplant Unit (VSTU) Discharge by Eleven Project is to identify barriers to discharge and increase efficiency throughout the discharge process. A patient database was developed to detect variables affecting early discharge. An improvement plan was implemented focusing on timeliness of writing discharge orders by surgical residents and early recognition of potential discharge by nursing, medical and casemanagement staff. Subsequently, the ‘VSTUResidentSurvival Guide’was createdhighlighting the expectationsof nurses rounding daily with attending physicians and residents to identify discharges for the next day. Methods of communication included the team leaders, who served as discharge champions, and utilization of the assignment board as a visual cue.Nurseswere instructed to communicate that discharge occurs before eleven to every admission. Discharge times and customer satisfaction scores were posted weekly. The five month study showed an increase in the satisfaction with speed of the discharge process (75 to 89.3) and overall satisfaction with the process (83.6 to 89.3) based on Press Ganey raw scores. The percent of discharges by eleven increased from 11% to 15% and late discharges decreased from 50% to 37%. These findings suggest that by targeting unit specific variables early discharges can be increased.
