Medium- and long-term performance of 11 516 uncemented primary femoral stems from the Norwegian arthroplasty register

Primary uncemented femoral stems reported to the Norwegian arthroplasty register between 1987 and 2005 were included in this prospective observational study. There were 11 516 hips (9679 patients) and 14 different designs of stem. Kaplan-Meier survival probabilities and Cox regression were used to analyse the data. With aseptic loosening as the end-point, all currently used designs performed excellently with survival of 96% to 100% at ten years. With the end-point as stem revision for any cause, the long-term results of the different designs varied from poor to excellent, with survival at 15 years ranging between 29% and 97%. Follow-up for longer than seven years was needed to identify some of the poorly-performing designs. There were differences between the stems; the Corail, used in 5456 hips, was the most frequently used stem with a survival of 97% at 15 years. Male gender was associated with an increased risk of revision of x 1.3 (95% confidence interval 1.05 to 1.52), but age and diagnosis had no influence on the results. Overall, modern uncemented femoral stems performed well. Moderate differences in survival between well-performing stems should be interpreted with caution since the differences may be caused by factors other than the stem itself.

Increasing risk of prosthetic joint infection after total hip arthroplasty

Background and purpose The risk of revision due to infection after primary total hip arthroplasty (THA) has been reported to be increasing in Norway. We investigated whether this increase is a common feature in the Nordic countries (Denmark, Finland, Norway, and Sweden). Materials and methods The study was based on the Nordic Arthroplasty Register Association (NARA) dataset. 432,168 primary THAs from 1995 to 2009 were included (Denmark: 83,853, Finland 78,106, Norway 88,455, and Sweden 181,754). Adjusted survival analyses were performed using Cox regression models with revision due to infection as the endpoint. The effect of risk factors such as the year of surgery, age, sex, diagnosis, type of prosthesis, and fixation were assessed. Results 2,778 (0.6%) of the primary THAs were revised due to infection. Compared to the period 1995–1999, the relative risk (with 95% CI) of revision due to infection was 1.1 (1.0–1.2) in 2000–2004 and 1.6 (1.4–1.7) in 2005–2009. Adjusted cumulative 5–year revision rates due to infection were 0.46% (0.42–0.50) in 1995–1999, 0.54% (0.50–0.58) in 2000–2004, and 0.71% (0.66–0.76) in 2005–2009. The entire increase in risk of revision due to infection was within 1 year of primary surgery, and most notably in the first 3 months. The risk of revision due to infection increased in all 4 countries. Risk factors for revision due to infection were male sex, hybrid fixation, cement without antibiotics, and THA performed due to inflammatory disease, hip fracture, or femoral head necrosis. None of these risk factors increased in incidence during the study period. Interpretation We found increased relative risk of revision and increased cumulative 5–year revision rates due to infection after primary THA during the period 1995–2009. No change in risk factors in the NARA dataset could explain this increase. We believe that there has been an actual increase in the incidence of prosthetic joint infections after THA.

Increasing risk of revision due to deep infection after hip arthroplasty.

Background and purpose Over the decades, improvements in surgery and perioperative routines have reduced the incidence of deep infections after total hip arthroplasty (THA). There is, however, some evidence to suggest that the incidence of infection is increasing again. We assessed the risk of revision due to deep infection for primary THAs reported to the Norwegian Arthroplasty Register (NAR) over the period 1987–2007. Method We included all primary cemented and uncemented THAs reported to the NAR from September 15, 1987 to January 1, 2008 and performed adjusted Cox regression analyses with the first revision due to deep infection as endpoint. Changes in revision rate as a function of the year of operation were investigated. Results Of the 97,344 primary THAs that met the inclusion criteria, 614 THAs had been revised due to deep infection (5-year survival 99.46%). Risk of revision due to deep infection increased throughout the period studied. Compared to the THAs implanted in 1987–1992, the risk of revision due to infection was 1.3 times higher (95%CI: 1.0–1.7) for those implanted in 1993–1997, 1.5 times (95% CI: 1.2–2.0) for those implanted in 1998–2002, and 3.0 times (95% CI: 2.2–4.0) for those implanted in 2003–2007. The most pronounced increase in risk of being revised due to deep infection was for the subgroup of uncemented THAs from 2003–2007, which had an increase of 5 times (95% CI: 2.6–11) compared to uncemented THAs from 1987–1992. Interpretation The incidence of deep infection after THA increased during the period 1987–2007. Concomitant changes in confounding factors, however, complicate the interpretation of the results.

More re-operations after uncemented than cemented hemiarthroplasty used in the treatment of displaced fractures of the femoral neck: An observational study of 11 116 hemiarthroplasties from a national register

Using data from the Norwegian Hip Fracture Register, 8639 cemented and 2477 uncemented primary hemiarthroplasties for displaced fractures of the femoral neck in patients aged > 70 years were included in a prospective observational study. A total of 218 re-operations were performed after cemented and 128 after uncemented procedures. Survival of the hemiarthroplasties was calculated using the Kaplan-Meier method and hazard rate ratios (HRR) for revision were calculated using Cox regression analyses. At five years the implant survival was 97% (95% confidence interval (CI) 97 to 97) for cemented and 91% (95% CI 87 to 94) for uncemented hemiarthroplasties. Uncemented hemiarthroplasties had a 2.1 times increased risk of revision compared with cemented prostheses (95% confidence interval 1.7 to 2.6, p < 0.001). The increased risk was mainly caused by revisions for peri-prosthetic fracture (HRR = 17), aseptic loosening (HRR = 17), haematoma formation (HRR = 5.3), superficial infection (HRR = 4.6) and dislocation (HRR = 1.8). More intra-operative complications, including intra-operative death, were reported for the cemented hemiarthroplasties. However, in a time-dependent analysis, the HRR for re-operation in both groups increased as follow-up increased. This study showed that the risk for revision was higher for uncemented than for cemented hemiarthroplasties.

Survival rates and causes of revision in cemented primary total knee replacement: A report from the Norwegian Arthroplasty Register 1994–2009

We evaluated the rates of survival and cause of revision of seven different brands of cemented primary total knee replacement (TKR) in the Norwegian Arthroplasty Register during the years 1994 to 2009. Revision for any cause, including resurfacing of the patella, was the primary endpoint. Specific causes of revision were secondary outcomes. Three posterior cruciate-retaining (PCR) fixed modular-bearing TKRs, two fixed non-modular bearing PCR TKRs and two mobile-bearing posterior cruciate-sacrificing TKRs were investigated in a total of 17 782 primary TKRs. The median follow-up for the implants ranged from 1.8 to 6.9 years. Kaplan-Meier 10-year survival ranged from 89.5% to 95.3%. Coxs relative risk (RR) was calculated relative to the fixed modular-bearing Profix knee (the most frequently used TKR in Norway), and ranged from 1.1 to 2.6. The risk of revision for aseptic tibial loosening was higher in the mobile-bearing LCS Classic (RR 6.8 (95% confidence interval (CI) 3.8 to 12.1)), the LCS Complete (RR 7.7 (95% CI 4.1 to 14.4)), the fixed modular-bearing Duracon (RR 4.5 (95% CI 1.8 to 11.1)) and the fixed non-modular bearing AGC Universal TKR (RR 2.5 (95% CI 1.3 to 5.1)), compared with the Profix. These implants (except AGC Universal) also had an increased risk of revision for femoral loosening (RR 2.3 (95% CI 1.1 to 4.8), RR 3.7 (95% CI 1.6 to 8.9), and RR 3.4 (95% CI 1.1 to 11.0), respectively). These results suggest that aseptic loosening is related to design in TKR.

Surgical procedures in the treatment of 784 infected THAs reported to the Norwegian Arthroplasty Register: Best survival with 2-stage exchange revision, but also good results with debridement and retention of the fixed implant

Background and purpose Controversies still exist regarding the best surgical procedure in the treatment of periprosthetic infection after total hip arthroplasty (THA). Based on data in the Norwegian Arthroplasty Register (NAR), we have compared the risk of re-revision after 4 different surgical procedures: 2-stage with exchange of the whole prosthesis, 1-stage with exchange of the whole prosthesis, major partial 1-stage with exchange of stem or cup, and minor partial 1-stage with exchange of femoral head and/or acetabular liner. Methods Between 1987 and 2009, 124,759 primary THAs were reported to the NAR, of which 906 (0.7%) were revised due to infection. Included in this study were the 784 revisions that had been performed by 1 of the 4 different surgical procedures. Cox-estimated survival and relative revision risks are presented with adjustment for differences among groups regarding gender, type of fixation, type of prosthesis, and age at revision. Results 2-stage procedures were used in 283 revisions (36%), 1-stage in 192 revisions (25%), major partial in 129 revisions (17%), and minor partial in 180 revisions (23%). 2-year Kaplan-Meier survival for all revisions was 83%; it was 92% for those re-revised by 2-stage exchange procedure, 88% for those re-revised by 1-stage exchange procedure, 66% for those re-revised by major partial exchange procedure, and it was 76% for those re-revised by minor partial exchange. Compared to the 2-stage procedure and with any reason for revision as endpoint (180 re-revisions), the risk of re-revision increased 1.4 times for 1-stage (p = 0.2), 4.1 times for major partial exchange (p < 0.001), and 1.5 times for minor partial exchange (p = 0.1). With infection as the endpoint (108 re-revisions), the risk of re-revision increased 2.0 times for 1-stage exchange (p = 0.04), 6.0 times for major partial exchange (p < 0.001), and 2.3 times for minor partial exchange (p = 0.02). Similar results were found when the analyses were restricted to the period 2002–2009. Interpretation In the Norwegian Arthroplasty Register, the survival after revision of infected primary THA with 2-stage implant exchange was slightly superior to that for 1-stage exchange of the whole prosthesis. This result is noteworthy, since 2-stage procedures are often used with the most severe infections. However, debridement with exchange of head and/or liner but with retention of the fixed implant (minor revision) meant that there was a 76% chance of not being re-revised within 2 years.

High wear rates and extensive osteolysis in 3 types of uncemented total hip arthroplasty: A review of the PCA, the Harris Galante and the Profile/Tri-Lock Plus arthroplasties with a minimum of 12 years median follow-up in 96 hips

Background High wear rates and femoral and acetabular osteolysis have been—and still are—the main problems in uncemented total hip replacement. We reviewed 96 consecutive cementless total hip replacements of 4 different designs. Patients and methods 21 PCA, 25 Harris Galante Porous/Harris Galante I (stem/cup), 25 Profile pressfit/ Tri-Lock Plus (stem/cup) and 25 Profile HA-coated/Tri-Lock Plus (stem/cup) prostheses were included. The operations were performed in the period 1984–1991. Median follow-up ranged from 12–16 years. Wear and osteolysis were measured. Results Mean linear wear rates ranged from 0.17 to 0.21 mm/year in the 4 groups, and there were no statistically significant differences between the groups (p = 0.9, ANOVA). Moderate or extensive osteolysis was found in 46 of the 96 hips included. The association between high and low wear rates (more or less than 0.20 mm/ year) and extent of osteolysis was statistically significant (p < 0.001, t-test). We found poor 12-year survival of the primary prostheses in all 4 groups (50–70%), mainly due to revisions because of wear of the polyethylene liner and/or osteolysis. The infrequently documented Profile/Tri-Lock Plus systems did not perform differently from the PCA and the HG. Interpretation The poor long-term results with these uncemented total hip arthroplasties illustrate the necessity of regular radiographic evaluation in order to detect osteolysis and liner failure, which are both generally asymptomatic until catastrophic failure appears.

Wear and Migration of Highly Cross-Linked and Conventional Cemented Polyethylene Cups with Cobalt Chrome or Oxinium Femoral Heads: A Randomized Radiostereometric Study of 150 Patients

This randomized study was performed to compare wear and migration of five different cemented total hip joint articulations in 150 patients. The patients received either a Charnley femoral stem with a 22.2 mm head or a Spectron EF femoral stem with a 28 mm head. The Charnley articulated with a γ‐sterilized Charnley Ogee acetabular cup. The Spectron EF was used with either EtO‐sterilized non‐cross‐linked polyethylene (Reflection All‐Poly) or highly cross‐linked (Reflection All‐Poly XLPE) cups, combined with either cobalt chrome (CoCr) or Oxinium femoral heads. The patients were followed with repeated RSA measurements for 2 years. After 2 years, the EtO‐sterilized non‐cross‐linked Reflection All‐Poly cups had more than four times higher proximal penetration than its highly cross‐linked counterpart. Use of Oxinium femoral heads did not affect penetration at 2 years compared to heads made of CoCr. Further follow‐up is needed to evaluate the benefits, if any, of Oxinium femoral heads in the clinical setting. The Charnley Ogee was not outperformed by the more recently introduced implants in our study. We conclude that this prostheses still represents a standard against which new implants can be measured.

Metal-backed acetabular components with conventional polyethylene: A REVIEW OF 9113 PRIMARY COMPONENTS WITH A FOLLOW-UP OF 20 YEARS

The Norwegian Arthroplasty Register has shown that several designs of uncemented femoral stems give good or excellent survivorship. The overall findings for uncemented total hip replacement however, have been disappointing because of poor results with the use of metal-backed acetabular components. In this study, we exclusively investigated the medium-to long-term performance of primary uncemented metal-backed acetabular components. A total of 9113 primary uncemented acetabular components were implanted in 7937 patients between 1987 and 2007. These were included in a prospective, population-based observational study. All the implants were modular and metal-backed with ultra-high-molecular-weight polyethylene liners. The femoral heads were made of stainless steel, cobalt-chrome (CoCr) alloy or alumina ceramic. In all, seven different designs of acetabular component were evaluated by the Kaplan-Meier survivorship method and Cox regression analysis. Most acetabular components performed well up to seven years. When the endpoint was revision of the acetabular component because of aseptic loosening, the survival ranged between 87% and 100% at ten years. However, when the endpoint was revision for any reason, the survival estimates were 81% to 92% for the same implants at ten years. Aseptic loosening, wear, osteolysis and dislocation were the main reasons for the relatively poor overall performance of the acetabular components. Prostheses with alumina heads performed slightly better than those with stainless steel or CoCr alloy in subgroups. Whereas most acetabular components performed well at seven years, the survivorship declined with longer follow-up. Fixation was generally good. None of the metal-backed uncemented acetabular components with ultra-high-molecular-weight polyethylene liners in our study had satisfactory long-term results because of high rates of wear, osteolysis, aseptic loosening and dislocation.
