Two hundred and forty-two patients with squamous cell carcinoma of the mobile tongue were reviewed. Most of them were initially treated by irradiation and then treated surgically for salvage. Cervical node metastases were frequently developed during or after the initial therapy. The patients in the advanced stage showed poor prognosis due to uncontrolled cervical node metastases and/or local recurrence. In the patients with no initial cervical node involvement, better local control and less frequent incidence of subsequently developed cervical node metastases were observed in the group treated by surgery than in those treated by irradiation. In the patients with initial cervical node involvement, no significant difference was noted in the survival yielded by either treatment modality. In advanced carcinoma, combined treatment with radiotherapy and surgical therapy seemed to give better results than with either radiotherapy or surgical therapy alone in this study.

Eleven patients with olfactory disturbances caused by inflammation of the ethmoid sinuses are reviewed, and the functional results of endonasal surgery are evaluated.Rhinoscopic examination showed the presence of a purulent discharge and mucosal thickening of the superior meatus.Radiographic examination showed opacity in the ethmoid sinus and the olfactory region.A standard olfactory acuity test, each side separately in 9 patients and both sides together in 2 patients (20 tests in 11 patients), and an intravenous olfaction test with alinamine were performed before and after surgery.The standard olfactory acuity test showed that 16 out of 20 (80%) recovered more than 1 grade in the detection threshold, and 15 out of 20 (75%) recovered more than 1 grade in the recognition threshold after endonasal ethmoidectomy.We conclude that patients with olfactory disturbance caused by ethmoid sinusitis can expect good results of surgical treatment.

The prognosis of maxillary sinus cancer in recent seven year period was discussed. The study of 107 patients of primary maxillary sinus cancer revealed that the five-year survival rate was 47%. The most common cause of death was local tumor and, therefore, sure treatment for local tumors is required. What is important is the selection of patients who do not need radical operations.

Fifty patients were examined in order to define the clinical value of carcinoembryonic antigen (CEA) in maxillary cancer.Plasma CEA levels of all cases were measured. At the time of testing, 37 of the 50 were tumor-bearing and the others were tumor-free more than six months after treatment.Plasma CEA level higher than 2.6ng/ml was estimated positive and 16 of the 37 patients who were tumor-bearing and none of the 13 patients who were tumor-free were positive.In 18 out of the 37 patients who were tumor bearing, tumor CEA levels were measured. In 30% of the cases studied, the tumor CEA level was higher than that in the mucous membrane of maxillary sinusitis.Correlation between the tumor CEA level and the plasma CEA level was observed.No significant findings were shown in immunohistological study.Plasma CEA levels in the patients with maxillary cancer correlated with the tumor volume and the tumor CEA level.Serial determination of plasma CEA levels may have some value in monitoring tumor metastasis.CEA in maxillary cancer seems to be much more affected with CEA-like substances.
