Die Heidelberger Umstrukturierungsskala

ZusammenfassungIm Rahmen des Projekts „Praxisstudie analytische Langzeittherapie” wurde von der Heidelberger Arbeitsgruppe ein Instrument entwickelt, welches dazu dient, im Verlauf von analytischen Psychotherapien die Stufen der Umstrukturierung der Patientenpersönlichkeit zu erfassen. Das Instrument ist im formalen Aufbau angelehnt an die Skala APES von Stiles (1992); abweichend davon sind die Entwicklungsstufen der Veränderung nicht auf allgemeine Problemaneignung, sondern auf ein spezifisch psychoanalytisches Prozessmodell bezogen. Die Skala und das in ihr eingearbeitete Prozessmodell wird dargestellt und theoretisch begründet; ferner werden die Ergebnisse einer Reliabilitätsuntersuchung und erste Erfahrungen bei ihrer Anwendung im Rahmen stationärer Psychotherapien und psychoanalytischer Langzeittherapien dargestellt. Es wird hervorgehoben, dass es sich nicht nur um ein Forschungsinstrument handelt, sondern auch um ein praxisnahes Werkzeug zur Planung und Evaluation psychodynamischer Behandlungen und somit um einen Beitrag zur Qualitätssicherung in diesem Bereich.SummaryAs a part of a project for „Practical studies about long-term psychoanalytic therapies” an instrument was developed of the Heidelberg Group for measuring the stages of re-structuring of the patient’s personality in the course of analytic therapies. The instrument relies in its formal structure on the APES-Scale of Stiles (1992). In our scale, however, the stages of development not to the general requirement of problems, but rather to an specific model of the psychoanalytic process. The scale and the included model are presented and theoretically founded. Furthermore, results of the reliability and first experiences with application in in-patient therapies are described. It should be emphasized that our scale is not only an instrument of research, but also a of practice for planing and evaluating of psychodynamic treatments and therefore a contribution to quality assurance in this field.

PROGRESSIVE CHANGES IN PATIENTS' LIVES AFTER PSYCHOTHERAPY: WHICH TREATMENT EFFECTS SUPPORT THEM?

The endurance of psychotherapeutic effects after conclusion of inpatient treatment is examined in a follow-up study of 49 patients with psychosomatic, neurotic, and personality disorders. The perspective is not symptomatological but rather relates to the concrete changes occurring in the lives of the patients after treatment. The investigators hypothesized that the probability of progressively coping with life demands depends on the extent to which patients have gained insight into their central psychological problems. Using the Heidelberg Structural Change Scale (HSCS) to gauge the extent to which patients succeed in gaining cognitively and emotionally definitive insight into their intrapsychic conflicts and the structural vulnerabilities determining their condition, the authors were able to confirm their hypothesis. The HSCS, compared with other measures, offered the only possibility of predicting progressive (i.e., symptomatic) changes. The authors concluded that the demands of external life present opportunities for therapy success to be realized as progressive changes and that these changes can form a basis for further positive development.

Structural change as a predictor of long-term follow-up outcome

Abstract Based on data from psychoanalytic long-term psychotherapies, the predictive value of three measures of pre–post change for retrospective patient assessments of outcome at 1-year and 3-year follow-up was investigated. Pre–post changes were measured using the Global Severity Index (GSI), the Inventory of Interpersonal Problems (IIP) total score, and the Heidelberg Structural Change Scale (HSCS). In line with psychoanalytic theory, it was assumed that structural changes cause especially persistent changes and would, therefore, be most suitable to predict the follow-up criterion. This expectation was confirmed: Pre–post changes in GSI and IIP were only weakly associated with assessments at 1-year follow-up and not at all with assessments at 3-year follow-up. In contrast, correlations between changes in HSCS and outcome assessments were highly significant at both occasions.

Patterns of interpersonal problems and their improvement in depressive and anxious patients treated with psychoanalytic therapy

Interpersonal problems were studied in 121 patients treated with psychoanalytic therapy using the Inventory of Interpersonal Problems. Four characteristic subtypes were identified, which differed in the quality and flexibility of their interpersonal behavior. Independent of the predominant type of interpersonal problems, the psychotherapy treatment led to strong decreases in interpersonal distress and increases in interpersonal differentiation. Psychoanalytic therapy was highly effective for all identified interpersonal subtypes and seems to help patients achieve more satisfactory relationships.

[The project of the Techniker-Krankenkasse: some comments on the results by the project's scientific advisory board].

The Project of the Techniker Krankenkasse compared 2 systems of quality assurance in outpatient psychotherapy, the established peer review system based on qualitative individual case reports and a new model based on psychometric assessment and feed-back systems. In this publication the members of the project advisory board comment on the results and limitations of the study. 400 psychotherapists in private practice participated in a cluster-randomized comparison study, 200 were allocated to the intervention group, and 200 to the control group. In both groups assessments were performed at 3 points in time, one at the beginning of treatment, one at the end of treatment and one at follow-up 12 months post-treatment. No significant differences in psychotherapeutic outcome were observed in the 2 groups. The hypothesis of a better outcome associated with the psychometric assessment and benchmarking procedure was not confirmed.durch einen unabhängigen wissenschaftlichen Beirat begleitet werden sollte, der zu gewährleisten habe, dass die Studie fachlich angemessen und objektiv durchgeführt und ausgewertet wird. ZusammenfassungAm Beispiel von Burn-out, Mobbing und Trauma wird das öffentliche Interesse an „neuen“ Störungsbildern beschrieben. Im Hintergrund sind Selbsterklärungen von Patienten, aber auch Interessen von Therapeuten zu erkennen. Beide lassen sich als Ausdruck von nicht bewusst reflektierten, zeittypischen Überzeugungen verstehen. Die in der Richtlinienpsychotherapie wahrscheinlich zu häufig diagnostizierten Traumafolgestörungen und Missbrauchsfolgen werfen Fragen der angemessenen Behandlung und eventueller schädlicher Nebenwirkungen auf. Es werden gesellschaftliche Hintergründe der psychotherapeutischen Traumafaszination diskutiert.AbstractThe public interest in “new” clinical symptoms is described exemplified by burnout, mobbing and trauma. In the background self-explanations by patients as well as interests of therapists can be recognized. Both can be understood as an expression of subconsciously reflected contemporary convictions. Post-traumatic stress syndrome and the sequelae of abuse, which are probably diagnosed too often in guideline correct psychotherapy, throw up questions on the appropriate therapy and possible injurious side effects. The social background to the psychotherapeutic fascination with trauma will be discussed.

Outcomes of Psychoanalytic Therapies

Using a naturalistic design, 44 patients in psychoanalysis were examined with regard to qualitative and quantitative outcome of therapy. The results were compared with those from 56 dynamic and 164 inpatient therapies. Comparison of symptoms, diagnoses, and motivation prior to therapy led to the conclusion that the patient groups treated within these settings differed greatly. For this reason randomisation appeared to be an inadequate strategy for comparing groups in different therapeutic settings. Using different outcome criteria, it could be demonstrated that psychoanalysis patients on the whole attained good therapeutic results. The outcomes were the best in comparison with the other two therapy categories investigated. The use of different outcome criteria and observer perspectives (patient and therapist) led to therapy results with clear-cut differences. What Happens After Treatment: Can Structural Change be a Predictor of Long-Term Outcome?

The first systematic follow-up study of therapeutic results was delivered by the Berlin Psychoanalytic Institute and reported by Fenichel [1]. This study set a precedent that many psychoanalysts were to follow in subsequent years [2–4] (for a review see [5]). At times, the work of these analysts had far-reaching social impacts; the studies Duhrssen [6] performed at the Berlin Central Institute for Psychogenic Illnesses led to the inclusion of psychodynamic (PD) and psychoanalytic (PA) treatments among the forms of therapy covered by health insurance in Germany. From the available literature, it is clear that the PA community has cared about the issue of the long-term effects of treatment from early on.

Indikation zur strukturbezogenen Psychotherapie

ZusammenfassungDie Unterscheidung von konfliktbearbeitenden und strukturfördernden psychotherapeutischen Ansätzen im psychodynamischen Bereich ist mittlerweile Allgemeingut geworden und findet inzwischen auch Eingang in den Richtlinien-Kommentar. Die Differenzierung der Behandlung setzt theoretische Kenntnisse der Konflikt- und Strukturdynamik und eine darauf bezogene sorgfältige Eingangsdiagnostik voraus. Auf deren Grundlage kann mit dem Patienten ein entsprechender Behandlungsfokus vereinbart und das Therapieziel verabredet werden. Dieses fokussierte Arbeiten auf ein gemeinsames Ziel hin ist für die zeitlich eng begrenzte tiefenpsychologische Behandlung besonders wichtig. Der aktiv begleitete therapeutische Prozess unterscheidet sich von älteren Vorstellungen, dass positive Entwicklungen auf dem Boden einer therapeutischen Beziehung gleichsam von selbst erfolgen, aber möglicherweise lange Zeit benötigen. Aspekte der Behandlungstechnik und der therapeutischen Haltung, der Einschätzungen der therapeutischen Entwicklung sowie der alterstypischen Modifikationen bei Jugendlichen und Älteren werden angesprochen.AbstractThe distinction of conflict processing and structure promoting psychotherapeutic approaches in the psychodynamic field is now common knowledge and has now been incorporated into the guideline comments. The differentiation of treatment presupposes theoretical knowledge of the dynamics of conflict and structure and corresponding thorough initial diagnostics. Based on these foundations an appropriate focus of therapy can be agreed with the patient and the therapy target can be set. This focussed working towards a joint target is especially important for psychodynamic psychotherapeutic treatment, which is of limited duration. The actively accompanying therapeutic process differs from older assumptions that positive developments on the foundation of a therapeutic relationship occur quasi automatically but possibly need a long time. Aspects of the treatment technique and the therapeutic attitude, estimation of the therapeutic development and typical age-related modifications in adolescents and older persons are discussed.

ZusammenfassungBilder vom Menschen geben vorläufige Antworten auf die Frage, was dessen Situation im Wesentlichen ausmacht. In den psychoanalytisch fundierten Verfahren tauchen seit jeher Bilder auf, die die animalischen Impulse, die Emotionen, die interpersonelle Situation und – im Kontrast dazu – das selbstreflexive Subjekt, seine Fähigkeit zu Sprache, Kommunikation und Kultur in den Vordergrund stellen. In Ergänzung dazu wird in dieser Erörterung besondere Aufmerksamkeit auf die sich verändernden ökonomischen und technologischen Bedingungen gerichtet, die den Menschen in eine neuartige Lebenswelt versetzen und möglicherweise neue Formen der Persönlichkeitsentwicklung hinsichtlich seiner biologischen Natur und auch seiner soziokulturellen Möglichkeiten bewirken können. Als Konsequenz für die Psychotherapie wird über die Bearbeitung individueller psychischer Störungen hinaus die Berücksichtigung dieser anthropologischen Aspekte empfohlen.AbstractImages of humans provide preliminary answers to the question of what essentially constitutes their situation. In psychoanalytically based procedures images have appeared from time immemorial, which especially emphasize the animal impulses, the emotions, the interpersonal situation and in contrast, the self-reflecting subject, the capabilities for speech, communication and culture. In addition to this, in this discussion special attention is paid to the changing economic and technological circumstances, which displace humans into a novel living environment and could possibly bring about new forms of personality development with respect to the biological nature and also the sociocultural possibilities. As a consequence for psychotherapy, the consideration of these anthropological aspects is recommended beyond the processing of individual mental disorders.
