The pathology and pathogenesis of endolymphatic hydrops. New investigations

ZusammenfassungDie Untersuchung eines Falles von einseitiger Menièrescher Erkrankung bestätigt die kürzlich ausgesprochene Annahme, daß die häufig beobachteten Hernien der Sacculus- und Utriculuswand nicht geheilte Risse der ganzen Wand sind, sondern nur Einrisse der inneren Schicht mit Ausbuchtung der äußeren Lage. Wenngleich Risse vorkommen, so sind these sehr selten. Weder in unserer Beobachtung noch in der Literatur konnte bewiesen werden, daß Hernien geheilte Risse der ganzen Labyrinthwand darstellen. Die Theorie, daß die periodisch auftretenden Menièreschen Anfälle durch solche Wandrisse verursacht werden, kann unserer Ansicht nach nicht durch anatomische Befunde gestützt werden.Die Fibrose des perisacculären Gewebes an der erkrankten Seite begünstigt die Annahme, daß Störungen in der Endolymphresorption eine Rolle bei Entstehung des Hydrops spielen.Diese Unterstellung ist im Einklang mit den Befunden im zweiten Fall. Der sehr starke beiderseitige Hydrops ist vergesellschaftet mit Zeichen einer geheilten serofibrinösen Labyrinthitis als Folge einer akuten Mittelohrentzündung in der Kindheit.Das Ausmaß des endolymphatischen Hydrops übertraf bei weitem das, welches normalerweise bei abgeheilter seröser Labyrinthitis beobachtet wird. Da eine knöcherne Obliteration beider Ductus endolymphatici bestand, konnte eine Störung der Endolymphresorption auch hier mitverursachend rein, für die starke Ausweitung der Endolymphräume.SummaryThe examination of a case of unilateral Menières disease confirms the previously expressed opinion that the frequently observed circumscribed herniations of the saccular and utricular wall are not healed ruptures of the entire wall but only ruptures of the inner layer with outward bulging of the outer layer. Neither in this nor in any of the other observations reported in the literature is there any evidence that they could represent healed ruptures of the entire wall; true ruptures occasionally occur but are extremely rare. The theories which try to explain the pathogenesis of the Menière attacks with periodic ruptures of the walls of the endolymphatic structures with subsequent healing are in our opinion not supported by the actual anatomical findings.The fibrosis of the perisaccular tissue on the diseased side favours the assumption that disturbances in the absorption of the endolymph in this area could play a role in producting the hydrops.This supposition is in good agreement with the findings in the second case. Here a very bilateral endolymphatic hydrops was found together with signs of a healed predominately serous labyrinthitis which had followed an acute middle ear infection in childhood with extensive paralabyrinthitis.The degree of the endolymphatic hydrops by far exceeded the one usually seen in a healed serous labyrinthitis. Since there was a bony obliteration of both endolymphatic ducts, a disturbance in the resorption of the endolymph could again have been a contributory factor in bringing about the marked dilatation of the endolymphatic spaces.

Diese befinden sich in guter Überein-stimmung mit den Befunden am übrigen Körper.Von besonderem Interesse ist das Vorliegen von Otoskleroseherden in den Nischen des ovalen and des runden Fensters ohne Stapesankylose and der Befund von zahlreichen typischen „blauen Mänteln” im Bereich der Bogengänge in streng symmetrischer Anordnung.Es erhebt sich die Frage, ob es sich hier um ein zufälliges Zusammentreffen oder um eine zugrundeliegende allgemeine Stoffwechselstörung handelt.SummaryThe histological changes in the temporal bones in a case of gargoylism are reported. They are in good agreement with the findings in the rest of the body. Of particular interest is the presence of otosclerotic foci in the oval and round window niches without stapedial ankylosis and of numerous typical blue mantles in the semicircular canal region, in strictly symmetrical arrangement. The question arises whether this is purely coincidental or a manifestation of the underlying generalized metabolic disorder.
