Contribution of Doppler echocardiography to the evaluation of systolic and diastolic function of obese women versus a control group

OBJECTIVE To study by doppler echocardiography the cardiac systolic and diastolic functions of health, uncomplicated obese subjects. METHODS Fifty-nine obese women with an average body mass index (BMI) of 35 kg/m2 were evaluated and compared with 19 subjects with an average BMI of 23 kg/m2 (control group). RESULTS In the obese group, a clear tendency was observed toward higher systolic pressure, increased wall thickness and, consequently, myocardial mass, elevation on the circumference stress of the left ventricular wall, and an indisputable presence of diastolic abnormalities. Filling abnormalities were observed with impaired relaxation, with prolonged isovolumic relaxation time (IVRT) and augmented atrium contribution representing early indexes of cardiac dysfunction when systolic performance is still normal. CONCLUSION Obesity is generally a chronic condition, and doppler echocardiography can be used as a noninvasive instrument for early evaluation of left ventricular diastolic indexes.

Brain spect in the pre-surgical evaluation of epileptic patients preliminary results

Pre-surgical evaluation of epileptic patients consists of neurological examination, intensive electroencephalographic (EEG) monitoring and anatomical studies (CT and MRI). Functional methods such as PET and SPECT imaging are now used more frequently. We have studied pre-operatively 15 adult epileptic patients (8 female, 7 male) using a rotational scintillation camera interfaced to a dedicated computer. The tomographic images were obtained 15 minutes after intravenous injection of 99mTc_HMPAO. All had MRI scanning and intensive EEG monitoring which generally included seizure recording. Five patients had progressive lesions (3 meningiomas, 2 astrocytomas). In 10 patients, neuroradiological studies did not show the presence of progressive lesions (2 normal scans and 8 cases with inactive lesions). Two patients with meningioma showed hypoperfusion at the lesion site while the third patient had a marked hyperperfusion which might correlate with the clinical diagnosis of epilepsia partialis continua. In the astrocytoma patients SPECT scans showed hypoperfusion at the lesion site. Data obtained from the 10 patients without progressive CNS lesions showed: (a) in 4, SPECT findings correlated well with the anatomical findings; (b) in 5 instances, SPECT was able to disclose additional functional deficits; (c) in one case, there was no SPECT correlate of a discrete anatomical lesion. In 5 of these cases with no progressive lesions (n=10) SPECT findings were useful as a complementary tool in determining the clinical or surgical management of these patients. Despite the small number and hete-rogenicity of the present sample, SPECT seems to be an useful tool as part of the clinical workup of epileptic patients who are candidates for epilepsy surgery.

COMPARISON BETWEEN COMPUTED TOMOGRAPHY AND ULTRASONOGRAPHY IN THE CEREBRAL VENTRICLES MORPHOMETRY

Ventriculos encefalicos fueron evaluados por CT y US en sujetos normales y en pacientes afectados de meningitis. La biometria y el analisis estadistico fueron ejecutados observando el index ventricular (VI%), los diametros de los cuernos frontal y temporal del ventriculo lateral, y los diametros de los ventriculos III y IV. Comparando los dos metodos diagnosticos, el coeficiente de correlacion fue alto para el diametro del ventriculo lateral (r=0.94) y para el VI (0.85). La sensibilidad de la US fue mejor que para la CT en la observacion del diametro del cuerno temporal. La US, por ser de alta precision, de bajo costo y por emplear un equipamiento transportable, resulta un metodo util para la investigacion de los ventriculos cerebrales

Ecodopplercardiografia transesofágica intra-operatória: utilidade na cirurgia da valva mitral

The usefulness of routine intraoperative transesophageal echocardiography (TEE) for mitral valve (MV) surgery was studied in 65 patients (pts) with MV disease (mean age = 31 ± 14.8 to 62 yrs): 1 - Mitral regurgitation (MR): 19 pts-MV prolapse in 12 pts; Ischemic in 6 pts and post repair of ASD: 1 pt; 2 - Mitral stenosis (MS): 23 pts (4 of them with previous surgery); 3 - MS+MR and/or tricuspid valve (TV) lesions: 16 pts; 4 - Bioprosthetic valve (BV) dysfunction: 6 pts and 5 - Left atrial myxoma: 1 pt. Initial precardiopulmonary bypass TEE was used to confirm MV dysfunction, TV involvement and to assess LV function. On the 1 st run bypass (RBP), the MV commissurotomy was performed in 27 pts (23 with MS and 4 with MS+MR); the MV was replaced with BV (pericardial #21 to #29) in 18 pts (6 with BV dysfunction and 12 with MS+MR) and the MV repair was performed in all of 19 pts with MR). The 2nd RBP was required in 7 pts (11%): 1 pt with MR; 4 pts with MS+MR and 2 pts with MS. TEE caused changes in pre pump plans in 9 pts with TV lesions and in 10 pts with MS or MS+MR - preservation of native in 6 pts in which replacement was planned. Thus intraoperative TEE provides helpful information in MV surgery and can help avoid a mitral replacement in pts with MS or MS+MR.
