La governance ibrida

The transformation of the welfare system contains incoherencies with the characteristics of the governance process. The diversification and the fragmentation of the system has made evident the increase in its complexity and its consequent inability to govern regulation processes based on hierarchy or on competition. The competition, the hierarchy and the social network are not necessarily alternative governance processes, but in practice they overlap. The competition, the hierarchy and the social network have to be integrated in a hybrid governance process. The distance between the system and its regulation requires a redefinition of governance process. Such realignment must be found on the strengthening of the authority, the thrust and the social capital.

Socio-economic differences in healthcare access from a welfare system perspective, Italy: 2007-2010

Inequalities between poorer and wealthier people in accessing healthcare services have been widely studied, but the mechanisms generating them are still to be fully understood. Among these, there is still a lack of evidence of relationships between health prevention/health promotion policies, welfare systems and social differences. We analysed 68 201 females from the PASSI Italian surveillance system for the years 2007-2010. The prevalence of women undergoing Pap testing was used as an example of access to preventive services. An odds ratio gradient was found with regard to different welfare system clusters: the probability of undergoing a screening test is higher for more advanced welfare systems. A strong association was found between having received a letter from the local health unit and having undergone the screening test. Significant differences still exist between high- and low-income women and their access to Italian preventive public services. As we expected, social determinants play an important role in health disparities, as these are also strongly influenced by typologies of welfare systems and by health policies.

Modelli di welfare e sistemi sanitari: quali omogeneità e specificità?

The welfare systems have been consolidated in the second half of the last century. The transition of post modernity society has incresed the signs of the crisis and pushed for their redefinition. The current changes depend on globalitation, but also by increased local autonomy. This article has tried to take stock of the international dibate on the welfare and health sistems classification. In the second part, this article evidence the differences between regionals welfare system and propose a new classification. The regionals health systems are different from each other, and have characteristics unrelated to the regional welfare systems.

Welfare e sviluppo locale

I sistemi di welfare stanno attraversando un processo di profonda trasformazione interna e di legami con il sistema economico. Il passaggio dalla modernita alla neo modernita cambia il rapporto fra economia e welfare, non piu visto come fattore di risposta ai fallimenti del mercato, ma come motore dello sviluppo locale. Le politiche sociali incidono sulla salute, sulle capability e sul capitale sociale delle persone. Questi fattori costituiscono i determinanti sociali dello sviluppo locale. Lo studio dei sistemi locali di welfare e dei loro legami con lo sviluppo fa emergere alcune evidenze empiriche della relazione fra determinanti sociali e sviluppo locale.

La valutazione nella promozione dell’attività motoria

L’inattivita fisica e associata ad una serie di malattie croniche, che interessano la popolazione. I dati sono allarmanti e numerosi Stati hanno attivato politiche di promozione alla salute concernenti l’attivita motoria, usufruendo di un approccio di tipo interdisciplinare. La ricerca valutativa, a questo proposito, sta sviluppando metodologie per intervenire in situazioni di complessita e multi-settorialita. Il presente contributo descrive un progetto di valutazione di programmi di promozione dell’attivita motoria attivi in tre realta regionali italiane: Veneto, Emilia-Romagna e Provincia Autonoma di Trento. Il processo valutativo attivato in ciascun caso regionale, rappresenta una particolare strategia valutativa che ha tenuto in considerazione gli obiettivi e il contesto territoriale di ogni programma di promozione, coinvolgendo gli stakeholders e impostando un lavoro nell’ottica della sostenibilita futura. Il progetto valutativo proposto si presenta come un contributo allo sviluppo di modalita e strumenti operativi efficienti e sostenibili nel campo della valutazione di programmi di promozione della salute.

Complessità e valutazione : l'impatto sulle pratiche dei servizi socio-sanitari

Il temine valutazione e entrato appieno nel dibattito sui servizi e sulle politiche sociosanitarie, ma le pratiche concrete sono ancora episodiche e non sempre hanno contribuito a migliorare i processi decisionali. I risultati positivi sono dovuti alla motivazione degli attori, al ruolo da loro giocato, ai processi e ai disegni di valutazione adottati. La valutazione deve farsi carico della complessita sottesa agli interventi socio sanitari e deve costituire un occasione di apprendimento riflessivo per le organizzazioni. I disegni devono consentire di evitare l’autoreferenzialita e attivare un confronto fra gli attori capace di creare senso, ma anche sviluppare uno scambio informativo basato sulle evidenze e farsi carico del rapporto fra informazioni prodotte e processo decisionale.
