Comparative radiologic study of the influence of ankle joint bandages on ankle stability

A comparative radiologic study was carried out to test the stabilizing value of ankle strapping and taping on unstable tibiotalar joints. Unstable ankle joints of 51 athletes were examined without bandages, with strap ping, and with taping before and after an activity pro gram. The use of taping proved to give the greatest decrease of the talar tilt (TT) angle. This improvement was greater, even after activity, than the stabilization obtained by strapping before any activity.

Fractures of the capitellum humeri in adolescents

Abstract Six children between 11 and 15 years old with a displaced fracture of the capitellum were treated by open reduction and fixation with one small screw. In all cases union occurred. There were no complications and all the children regained normal elbow function. There were no cases of avascular necrosis.Résumé Six enfants entre 11 et 15 ans ont été traités pour une fracture déplacée du capitellum par réduction ouverte et fixation interne par vis. La consolidation s’est toujours effectuée sans complications et avec une récupération complète de la fonction du coude. Il n’y avait pas de cas de nécrose avasculaire.

Split coronal fractures of the lumbar spine: Treatment by posterior internal fixation and transpedicular bone grafting

Abstract The clinical and radiological results of 12 patients with a split coronal vertebral fracture were analysed with minimum follow-up of 24 months. All patients were treated operatively with an AO internal spinal fixator and transpedicular bone grafting. The operation was supplemented with posterior bone grafts in 4 patients. All fractures healed without marked deformity. There were no vertebral body pseudarthroses. There were no neurological or vascular injuries due to the placement of pedicle screws or to transpedicular bone grafting. 10 patients, had excellent or good results.Résumé Les résultats cliniques et radiologiques de douze patients victimes d’une fracture du corps vertébral à trait frontal ont été analysé. Tous les patients avaient été opérés suivant la même technique: fixation interne de l’AO associéà une greffe osseuse transpédiculaire. Quatre de ces patients ont bénéficié en outre d’une greffe osseus postérieure. Toutes les fractures ont consolidées sans déformation marquante. Il n’y a pas de pseudarthrose du corps vertébral. Il n’y a pas eu de troubles neurologiques ou vasculaires dûs aux vis pédiculaires ou à la greffe transpédiculaire. Sur un suivi de minimum 24 mois, dix des douze patients ont un résultat classé excellent ou bon.

Subperiosteal ganglion cyst of the tibia

We report a patient with a subperiosteal ganglion cyst of the tibia which was imaged by radiography, arthrography, CT and MRI. The images were correlated with the arthroscopic surgical and histological findings. Spiculated formation of periosteal new bone on plain radiographs led to the initial suspicion of a malignant tumour. Demonstration of the cystic nature of the tumour using cross-sectional imaging was important for the precise diagnosis. Communication between the ganglion cyst and the knee was shown by a delayed arthrographic technique, and the presence of this communication was confirmed at arthroscopy and surgically.

Fracturas del extremo distal del húmero en la edad pediátrica

Las fracturas del extremo distal del humero en la edad pediatrica son lesiones frecuentes y el proceso desde el diagnostico a su tratamiento resulta una tarea ardua. Difieren de las fracturas del adulto por su etiologia, diagnostico, tipo de fractura, tratamiento y complicaciones. Las diferencias anatomicas son considerables y deben tenerse en cuenta durante el tratamiento. Los ninos tienen cartilagos de conjuncion y nucleos de crecimiento. Las lesiones de la fisis y de la epifisis son especificas de la poblacion pediatrica. Las fracturas del humero distal en los ninos suelen deberse a un traumatismo de baja velocidad y en su mayoria son consecutivas a un traumatismo indirecto, como una caida con el brazo en extension. Las que se deben a un traumatismo directo son infrecuentes. Aunque la capacidad de remodelacion de una fractura en los ninos sea considerable, las fracturas del extremo distal del humero pueden ocasionar complicaciones o un resultado mediocre. Abarcan un amplio espectro de gravedad, desde la fractura simple, no desplazada, a la muy desplazada, que se acompana en ocasiones de una lesion nerviosa o vascular. Las mas frecuentes de ellas son, por orden decreciente: las fracturas supracondileas (que suponen el 60% de las fracturas del codo), las del condilo lateral (que representan el 15%) y las fracturas del epicondilo medial (que suponen un 12%). Las fracturas del condilo medial, del capitellum y del epicondilo lateral son infrecuentes. Aun menos habituales son las avulsiones de toda la epifisis humeral distal y las fracturas supra e intercondileas.

Fratture dell’estremità inferiore dell’omero nel bambino

Riassunto Le fratture dell’omero distale nel bambino differiscono da quelle dell’adulto. Infatti, esistono importanti differenze anatomiche che occorre considerare per poter trattare tali lesioni nel miglior modo possibile. In ordine decrescente di frequenza, si osservano: le fratture sovracondiloidee, le fratture del condilo laterale e le fratture dell’epitroclea. Le fratture del condilo mediale, del capitello e dell’epicondilo laterale sono rare. Storicamente le fratture dell’omero distale sono sempre state associate a un elevato tasso di complicazioni, dovute generalmente a errori diagnostici o a trattamento inappropriato. Attualmente le complicazioni sono meno frequenti, ma non per questo meno temibili. Una migliore conoscenza dei differenti tipi di frattura e delle tecniche di riduzione e fissazione appropriate consente di ridurre le difficolta nel trattamento di tali lesioni.

Fratture del femore prossimale nel bambino

Riassunto Le fratture del femore prossimale nel bambino sono rare. Sono causate nella maggior parte dei casi da traumi a elevata energia, come quelli da incidente stradale o conseguenti a una caduta dall’alto. La classificazione piu utilizzata e quella di Delbet, che suddivide le fratture in quattro tipi. Il trattamento dipende prevalentemente dal tipo di frattura e dall’eta del bambino. Nel bambino piccolo le fratture composte o poco scomposte possono essere trattate con gesso pelvipodalico. Classicamente tutte le fratture scomposte devono essere sottoposte a riduzione e fissazione interna. Nei bambini piu piccoli si utilizzano fili di Kirschner per le fratture con distacco epifisario e per alcuni tipi di frattura transcervicale. Per le fratture transcervicali nei bambini piu grandi e per le fratture intertrocanteriche si utilizzano viti a filettatura corta, nel rispetto delle cartilagini di accrescimento. Le complicanze delle fratture del collo del femore nel bambino sono frequenti. Oltre alla pseudoartrosi e alla deformita in varo, la necrosi avascolare resta la complicanza piu temuta e piu grave. La necrosi avascolare post-traumatica dell’epifisi prossimale del femore puo causare un’invalidita permanente.

Fractura del extremo superior del fémur en el niño

Resumen Las fracturas del extremo superior del femur son raras durante la infancia. Esta dase de fracturas suele ser consecuencia de un traumatismo grave, en el marco de un accidente de circulacion o de una caida desde varios metros de altura. La clasificacion mas empleada es la de Delbet, quien distingue cuatro tipos de fractura. El tratamiento depende, en gran medida, del tipo de fractura y de la edad del nino. Las fracturas sin desplazamiento o con escaso desplazamiento pueden tratarse mediante yeso pelvipedio en el nino pequeno. Las fracturas desplazadas deben ser objeto de reduccion y fijacion interna. En los ninos pequenos se utilizan alambres de Kirschner para las fracturas con despegamiento epifisario y en ciertos casos de fractura transcervical. Cuando se trata de ninos mayores con fractura transcervical o intertrocanterea, se aplican tornillos de rosca corta, respetando siempre el cartilago de crecimiento. Las complicaciones de las fracturas de cuello femoral son frecuentes en el nino. Ademas de la seudoartrosis y la deformidad en varo, la necrosis avascular es la complicacion mas temida y la mas grave: la necrosis avascular de la epifisis femoral superior de origen postraumatico puede dar lugar a una discapacidad definitiva.
