Psychiatric Symptoms in Rapid-onset Obesity with Hypothalamic Dysfunction, Hypoventilation, and Autonomic Dysregulation Syndrome and its Treatment: A Case Report

To the Editor: Rapid-onset obesity with hypothalamic dysfunction, hypoventilation, and autonomic dysregulation (ROHHAD) syndrome is a rare disease. It is diagnosed in childhood (usually at 3–4 years of age). It affects endocrine, respiratory, and autonomic nervous systems. It shows different symptoms at different ages. Rapid weight gain, obesity, alveolar hypoventilation, hypernatremia, cardiopulmonary arrest, hyperprolactinemia, diabetes insipidus, excessive sweating, increase pain perception, obstructive sleep apnea, cold hands/feet, and digestive dysmotility can be seen. Alveolar hypoventilation is essential for the diagnosis.[1] Behavioral problems, mental retardation, personality changes, and mood disorders may accompany. There is no any special diagnostic test. Its treatment is symptomatic.[2] This study is intended to inform clinicians about psychiatric symptoms of a girl with ROHHAD syndrome.

Dissociative features of fibromyalgia syndrome

Objective: To assess the relationships between the dissociative features of FMS and the pain, psychological status, and functional status. Methods: Twenty-seven women with fibromyalgia syndrome (FMS) and 24 controls from the Istanbul Physical Medicine and Rehabilitation Hospital (2013-2015) were included in this cross-sectional study. The Diagnostic and Statistical Manual of Mental Disorders Structured Clinical Interview for Axis I Disorders was used to evaluate the participants. A visual analogous scale (VAS), Fibromyalgia Impact Questionnaire (FIQ), Beck Depression Inventory (BDI), and Beck Anxiety Inventory (BAI) were used to assess the levels of pain, quality of sleep and functional and psychological statuses. The Dissociative Experiences Scale (DES) was used to evaluate the dissociative features. Results: The BDI, BAI and DES scores were statistically significantly higher in the cases of FMS. There were remarkable associations between all but 2 of the DES and FIQ scores, while positive correlations were found between the DES and, VAS pain and sleep quality scores. The prevalences of current and lifelong dysthymia, and major depressive disorder; not otherwise specified, common anxiety and somatoform disorders were higher in the cases of FMS. Conclusion: Pain, physical function and emotional status appear to be associated with dissociative features in FMS. Further studies are required to define these relationships and improve treatment.

Childhood mental trauma in adult obsessive compulsive disorder patients

Obsessive Compulsive Disorder (OCD) is a mental disorder characterized by obsessions and/or compulsions. Athough some epidemiological studies take part in literature, which claim that traumatic life events in childhood ages are observed more in patients with OCD compared to healthy population, the number of these studies is limited.In this study it is aimed to compare OCD patients with healthy volunteers in terms of traumatic life events in childhood ages. With 25 consecutive patients who are diagnosed as OCD and whose treatment continues, 25 healthy controls equivalented in terms of sociodemographic features are icluded in the study. Sociodemographic Data Form, Childhood Age Trauma Quarter (CTQ) and Maudley Obsessive Compulsive Question List (MOCQL) are applied to the participants. Significance Value in statistical level is accepted as p< 0,05. In OCD patient group, CTQ scores are found high in statistical level compared to healthy controls.It has been determined that there is a significant relationship between total score of MOCQL slowness subscale scores, subscalescores of sexual and emotional abuse, MOCQL rumination subscale scores and CTQ sexual abuse scores. Compared to healthy controls,more findings of traumatic life event in childhood age are observed within OCD patients.

A Case of Munchausen Syndrome Presenting as a Cushing Syndrome in a Patient with Major Depressive Disorder

Munchausen’s syndrome is a psychiatric problem in which patients inflict an illness on themselves with the aim of playing in the role of being sick. İn this paper a 38-yearold woman with a history of MDD (Major Depressive Disorder) is introduced with uncontrolled hypertension, high blood sugar and cushingoid appearance in the result of high dose exogenous steroids injections secretly and without a prescription. She was interested in hospitalization by inducing hypertension crisis and hyperglycemia. Exogenous Cushing Syndrome was diagnosed according to laboratory findings and patient history during admission. Finally, Cushing Syndrome gradually improved over the months by psychotherapy and treatment with antidepressant drugs.

The Comparison Between Personality Characteristics and Family Relations Of The Subjects With Neurotic Level Of Personality Organization With Control Group

Studies to explain neurotic personality organization is no more. Studies emphasize the early family relationships are important in formation of this structure. Our study aims to assess personality traits and family relations of individuals with neurotic personality organization. 31 patients assessed in neurotic personality organization according to SCID-I and SCID-II followed by social psychiatry unit (Structured Clinical Interview for DSM Disorders), 31 control groups not taking diagnostic in the same tests were included in study. Socio-demographic data form was filled by interviewer, Beck Depression Inventory, MMPI (Minnesota Multiphasic Personality Inventory), State-Trait Anxiety Inventory, Family Assessment Scale, Sheehan Disability Scale by the participants. Control group was created from, of volunteers, subjects not taking any psychiatric diagnosis. Compared to neurotic patient group with control group; Shehan Disability Scale for Beck Depression Inventory scores; Family Assessment Scale for social life and family environment, business subscale and household responsibilities, for State-Trait Anxiety Inventory; hypochondria, depression, hysteria, and social introversion subscales for problem solving and behavior control subscale scores between groups and Minnesota Multiphasic Personality Inventory. Neurotic group was taking significantly diagnosis compared to control group for depressive disorder, anxiety disorders and avoidant personality disorder. Considered that avoidant personality structuring of neurotic individuals are at the forefront, the secondary anxiety and depressive symptoms progress. Said all these processes impair domestic problem-solving, behavior control skills of these individuals. Supports this process that the average score of neurotic patients are higher than control group for hypochondria, depression, hysteria, and social introversion subscales as results of MMPI.
