Telehealth - Home-Based Self-monitoring of Elderly Chronically Sick or At-Risk Patients

Many chronically sick people of advanced years are already living alone without any support from family members living nearby. Currently, attractive patient-focused and trans-sectoral care solutions based on innovative technology are still at the drawing-board stage. The long-term goal is to design sustainable and affordable solutions that are suitable for older people and accessible to a wide segment of the population to enable this form of care and support to become generally established. Development partnerships between care providers and technology providers, as well as trial projects with patients and funding bodies, are essential prerequisites for a successful market launch. On the basis of a telehealth project for COPD patients, it is shown how attractive solutions can be developed from the perspective of patients, care providers, funding bodies, and technology providers.
