Reduced occurrence of programmed cell death and gliosis in the retinas of juvenile rabbits after shortterm treatment with intravitreous bevacizumab

OBJECTIVE: Bevacizumab has been widely used as a vascular endothelial growth factor antagonist in the treatment of retinal vasoproliferative disorders in adults and, more recently, in infants with retinopathy of prematurity. Recently, it has been proposed that vascular endothelial growth factor acts as a protective factor for neurons and glial cells, particularly in developing nervous tissue. The purpose of this study was to investigate the effects of bevacizumab on the developing retinas of juvenile rabbits. METHODS: Juvenile rabbits received bevacizumab intravitreously in one eye; the other eye acted as an untreated control. Slit-lamp and fundoscopic examinations were performed both prior to and seven days after treatment. At the same time, retina samples were analyzed using immunohistochemistry to detect autophagy and apoptosis as well as proliferation and glial reactivity. Morphometric analyses were performed, and the data were analyzed using the Mann-Whitney U test. RESULTS: No clinical abnormalities were observed in either treated or untreated eyes. However, immunohistochemical analyses revealed a reduction in the occurrence of programmed cell death and increases in both proliferation and reactivity in the bevacizumab-treated group compared with the untreated group. CONCLUSIONS: Bevacizumab appears to alter programmed cell death patterns and promote gliosis in the developing retinas of rabbits; therefore, it should be used with caution in developing eyes.

Análise da qualidade de vida de portadores de uveítes de causas infecciosas e não infecciosas pelo questionário NEI-VFQ-25

PURPOSE To evaluate the life quality of patients with infectious and non-infectious uveitis evaluated at the uveitis service of the Hospital Universitário Clementino Fraga Filho-UFRJ, using the NEI-VFQ-25 questionnaire in order to clarify the importance of uveitis diagnosis and treatment as well as its consequences to visual and social functions of the patients. METHODS Prospective study of 30 patients with uveitis, who were divided into two groups according to the etiology, infectious and non-infectious, with the NEI-VFQ-25 form that evaluates the life quality regarding general and visual health which was applied twice to each patient. RESULTS The main cause of infectious uveitis was toxoplasmosis, and of the non-infectious was Vogt-Koyanagi-Harada syndrome. Concerning quality of life, general health is better in the infectious uveitis group, but ocular health is regular in both groups. Although visual deficit does not cause great disturbances and social restrictions, both groups show important emotional alteration, but in the non-infectious group this alteration causes a higher dependence level regarding daily activities. CONCLUSION The greater social dependence as well as that regarding performance of daily activities in the uveitis group of non-infectious cause is explained by the chronic and relapsing mode of these affections, which turns life quality lower if compared to the other group.

Reticulação do colágeno corneano com radiação ultravioleta e riboflavina para tratamento do ceratocone: resultados preliminares de um estudo brasileiro

PURPOSE: Assess the clinical and topographical effects of corneal collagen crosslinking in patients with keratoconus on Brazilian population. METHODS: Thirty seven eyes of 37 patients (22 female and 15 male) with diagnosis of progressive keratoconus and corneal paquimetry higher than 395 micra, were treated with riboflavin and ultraviolet A radiation, over 30 minutes, to induce corneal collagen crosslinking. Keratometric values were compared as well as the best corrected visual acuity, before and after the procedure, with ranges of 40 and 90 days. RESULTS: There was a reduction of mean dioptric power (PDM) in central 3mm in 83.8% of patients after three months.The PDM reduced 0.4D on average in central 3mm (p<0,01) and 0.33D in central 5mm (p<0,01). The maximum keratometric value decreased 0.6 D on average (p<0,01). The corrected visual acuity of the treated eye evolved with improvement average of 6,27 letters (p<0,01) after 90 days. CONCLUSION: We could observe a less pronounced improvement of the topographical values and visual acuity in comparison of the findings in other populations. Further studies with a larger number of patients and longer follow-up are necessary to confirm this techniques purpose of stabilizing the disease with only one application, and allow that to become the method of choice for keratoconus treatment.

Alterações oculares e eletrorretinográficas na doença falciforme

PURPOSE The primary objective of this case series was to get electroretinographic data in subjects with sickle cell disease. The secondary purpose was to identify ocular changes, their frequency and stage of evolution. METHODS 90 patients aged between 8 and 75 years were evaluated. They were subjected to complete ophthalmological examination, indirect ophthalmoscopy, retinography, angiography and electroretinography. Electroretinograms with amplitude of wave b less than 400 mW were considered subnormal. Prevalence study was conducted using the Chi-square test, P<0.001. RESULTS The electroretinogram was subnormal in 27 (30%) of 90 patients, but of these, only 4 (4.4%) had proliferative retinopathy. The sign of comma in 95% of individuals, the retinal vascular tortuosity in 36.6% and black sunburst in 24.4% were the most frequent changes. CONCLUSION No correlation between sickle cell disease and electroretinographics changes, as described in other studies, were noted. On the other hand, the ocular findings corroborate the previous researches.

Estudo comparativo entre a curva diária de pressão intra-ocular e a associação da curva ambulatorial com o teste de sobrecarga hídrica no glaucoma primário de ângulo aberto, glaucoma de pressão normal e olhos normais

PURPOSE To evaluate and correlate the peaks and the fluctuation of intraocular pressure seen in the association of the ambulatory intraocular pressure curve with the water-drinking test versus the peaks and the fluctuation seen in the daily intraocular pressure curve. METHODS The sample was as follows: 77 eyes belonging to 77 patients who were divided into three groups composed of 31 eyes belonging to 31 patients with primary open-angle glaucoma, 26 eyes belonging to 26 patients with normal tension glaucoma and 20 normal eyes belonging to 20 individuals. RESULTS A significant correlation could be observed between the pressure peaks collected from the daily intraocular pressure curve and the pressure peaks seen in the ambulatory intraocular pressure curve, on the water-drinking test, as well as in the association of the ambulatory intraocular pressure curve with the water-drinking test. The procedure which showed the highest rate of correlation between the pressure peaks and the peaks of the daily intraocular pressure curve was the ambulatory intraocular pressure curve (r2= 0.81). However, no statistically significant difference was found between the pressure peaks and the correlation coefficients observed in the other methods. The correlation between the fluctuation of intraocular pressure obtained in the association of the ambulatory intraocular pressure curve with the water-drinking test and the fluctuation of the pressure seen in the daily intraocular pressure curve showed a slight association (r2= 0.21). CONCLUSIONS The association between ambulatory intraocular pressure curve and water-drinking test was not efficient to estimate peaks and the fluctuation of daily intraocular pressure curve. The ambulatory intraocular pressure curve and the water-drinking test must be analyzed separately. When predicting the peak and the fluctuation of the daily intraocular pressure curve, the ambulatory intraocular pressure curve was the most efficient procedure.

Perfil socioeconômico e epidemiológico dos pacientes submetidos à cirurgia de catarata em um hospital universitário

PURPOSE: To determine the socio-economic characteristics of the patients who were submitted to cataract surgery in a university hospital, to evaluate the accessibility to the tertiary health service and to determine the waiting time for cataract surgery. METHODS: A cross-sectional study of medical charts from 148 random patients who underwent cataract surgery during the first semester of 2007. RESULTS: The patients in this study had median of age of 70.5 years, and the major prevalence was of female (60.1%), caucasian (48.0%), with low educational level (incomplete fundamental study, 50.7%), best corrected visual acuity 20/200 or worst; the diagnosis of cataract was done at the University Hospital in a routine appointment (47.8%). Only 9.4% of patients submitted to the surgery had his diagnosis done in a cataract campaign. The average time between the diagnosis and the surgery was 3.5 months and the delay was mainly due to pre operative exams. CONCLUSION: The majority of patients in the Brazilian Public Health System have low family income. The system takes a long time to access surgical procedures that need to be addressed in a proper time manner. The result of the long wait is subnormal vision. It is important to highlight the cataract campaign in order to enroll patients with low income in a hospital of high complexity.

A relação entre o comprimento dos processos ciliares medidos pela biomicroscopia ultra-sônica e a duração, localização e gravidade das uveítes

PURPOSE: To determine the relationship between the length of ciliary processes as measured by ultrasound biomicroscopy and the duration, localization and severity of uveitis. METHODS: We analyzed in this prospective study, 58 individuals including patients with different stages of uveitis and normal patients (112 eyes including 18 normal), between August 2001 and August 2002, at the Cleveland Clinic Foundation - Ohio - USA. All patients were referred to ultrasound biomicroscopy by the Uveitis Department, after a complete ophthalmological examination. A 50 MHz-transducer UBM model 840 (Zeiss-Humphrey) was used under topical anesthesia and immersion technique. RESULTS: The most frequent etiology was idiophatic uveitis (27.8%). Recurrent, aggressive and diffuse uveitis lead to significant damage of the ciliary processes. The most significant loss of the ciliary processes was found in the inferior quadrant and the highest in normal patients and in the temporal quadrant. Hypotonic eyes have their ciliary processes reduced. CONCLUSIONS: Ultrasound biomicroscopy has proved to be useful in evaluating anatomical changes of ciliary processes in uveitis and in ocular hypotony, and provides a basis to, make recomendations regarding to baseline screening of uveitis patients. Patients who are found to have atrophy of the ciliary processes may need a more aggressive treatment for any signs of inflammation to prevent further damage and eventual hypotony.

Número, tipo e topografia dos pontos de vazamento em pacientes com coriorretinopatia serosa central

PURPOSE To evaluate the number, shape and topography of leakage points in patients with central serous chorioretinopathy (CSC). METHODS Digital photographic files of fluorescein angiography with the diagnosis of CSC were reviewed by the author. The cases considered in remission and the examinations related to the same episode were excluded. The author filled out a form containing medical record number, initials, sex, age, date of the examination and leakage point characteristics (number, shape, and topography) of the selected cases. RESULTS The examinations of 418 patients (455 eyes) were included, with a male:female ratio of 2.32:1. Ninety-one point fifteen percent of the patients presented the disease unilaterally and 8.85% bilaterally. The mean age was 43.04 years (range, 19-68 years). Among the 455 selected eyes, 88.35% presented typical forms of CSC while 10.11% decompensation of the retinal pigment epithelium and 1.54% with isolated retinal pigment epithelium (RPE) detachment. The evaluation of typical forms of CSC showed a leakage point with uniform dye spread (inkblot) in 88.81% and 11.19% with a smoke-stack phenomenon. Regarding the leakage points, 46.01% were located in the upper nasal quadrant, 23.27% in the upper temporal quadrant, 19.18% in the lower nasal quadrant, 11.01% in the lower temporal quadrant and 0.53% in the subfoveal area. CONCLUSIONS In our series men were affected approximately two times more often then women, most of them with a unilateral involvement and in around the fourth decade of life. About 90% of the patients presented the typical forms, with 1 or 2 inkblot points or leakage. The superior nasal quadrant was the most affected. The isolated RPE detachment rarely occurred. Decompensation of RPE affected about 10% of the patients.

Manifestações oculares observadas em indivíduos infectados por HTLV-I no Rio de Janeiro

Purpose: The present study evaluates the frequency of ocular manifestations observed in HTLV-I seropositive patients in Rio de Janeiro. Methods: The study included 17 patients with TSP/HAM (tropical spastic paraparesis / HTLV-I associated myelopathy) and 55 seropositive patients to HTLV-I without TSP/HAM or ATLL (adult T-cell leukemia/lymphoma). Results: In the TSP/HAM patients sample we observed the frequency of 11.8% of anterior uveitis, 11.8% of retinal vasculitis and 5.9% of vitreous opacity. In the seropositive patients to HTLV-I without TSP/HAM or ATLL the frequency of 1.8% of retinal vasculitis and 1.8% of cotton-wool spot were observed. Conclusion: In conclusion, the HTLV-I must be considered as one of the etiological agents to be thought in these ocular manifestations in endemic areas such as Rio de Janeiro.
