Objective: The purpose of this study was to prospectively compare the short‐term benefit of endoscopic sinus surgery for smokers and nonsmokers using a disease specific, clinically validated, quality of life outcomes measure, the Sinonasal Outcomes Test‐20 (SNOT‐20).

The purpose of this study is to report on long‐term outcomes in a prospective study comparing the quality‐of‐life outcomes in smokers and nonsmokers undergoing functional endoscopic sinus surgery.

As experience with endoscopic sinus surgery has grown, so have its extended applications. Endoscopic management of transnasal intracranial foreign bodies can be successfully performed in selected patients and yields an improved cosmetic outcome and a decreased risk in complications. The patient was a 21 year old male who was driving intoxicated and sustained a motor vehicle accident. Patient was ejected from his vehicle and launched toward a tree. Patient presented to our institution with a tree branch which had entered the left aspect of his oral cavity piercing through his gingival labial sulcus, traveling into his left nasal cavity, penetrating through the septum into the right nasal cavity, traveling intracranially and piercing white matter. There was no evidence of a CSF leak at time of presentation.

100% of patients avoided a neck scar, corresponding to zero conversions; length of operative procedures average 95minutes, ranging from 45-190 minutes;1 transient tongue paresthesias ( resolved in 3wks), 79% of patients had limitation in tongue movements (up to the 3rd postoperative day), 0 hypoglossal nerve dysfunction, 0 facial nerve dysfunction; hospital stay averaged 36 hours,2 patients stayed 48 and 62 hours due to elevated blood sugar and uncontrolled hypertension respectively, 5 patients discharged on the same day.

Rectus Abdominis free flap was used for reconstruction. Patient M.G. was a 47 year old previously healthy, athletic male who presented to our Multidisciplinary Tumor Board and was diagnosed with a T1N2AM0 squamous cell carcinoma of the right tonsil. It was the consensus of the Tumor Board that he would best be treated with Radiation and Chemotherapy, followed by the possibility of a post-treatment neck dissection should his cervical lymphadenopathy not resolve. His biopsy at the time of diagnosis revealed high risk HPV status.
