SummaryTwelve cases of peptic ulcer with diabetes mellitus were found in 165 hospitalized diabetics. All of them had gastric ulcer and no duodenal ulcers were found. The incidence of peptic ulcer in diabetics was comparatively higher than the previously reported series. But there was no significant correlation between the duration of diabetes and the onset of gastric ulcer. The gastric ulcer with poorly controlled diabetes showed more intractability than those without triopathy and well-controlled diabetes.

Doubling of the pyloric channel is one of the rare anomalies of the intestinal tract. There have been 23 cases of double pylorus that we have been able to find in the literature.1–6,9–12,14–17 As the etiologic aspects of double pylorus the majorities of the cases were thought to be acquired lesions from ulcer disease except three cases. We present a case of double pylorus which seems to be acquired lesion, with a review of the literature.

SummaryComparative clinical trials were conducted by the double-blind technique in a series of 302 cases of gastric ulcer at fifteen medical institutions in order to investigate the clinical course of peptic ulceration and the effect of pharmacotherapy on it, using N-acetyl-L-glutamine aluminum complex (KW) as test drug and basic aluminum sucrose sulfate (UL) as reference drug as well as an inactive placebo of lactose (PL) with the results as follows:In 12 weeks of treatment with inactive placebo, a higher cure rates were noted in inpatients than in outpatients, and in patients with ulceration in the corpus ventriculi than in those with ulceration at the angulus. The overall evaluation of therapeutic responses indicated that, both KW and UL were significantly superior in efficacy to placebo in 12 weeks of therapy. These two medicaments displayed different modes of efficacy, possibly reflecting their different mechanisms of pharmacologic action. The efficacy of the medicaments was particularly superior in outpatients and patients with peptic ulceration at the gastric angle both of which showed lower rates of response to placebo.

The difference of inhibit ion mechanisms between secretin and gastrone on gastrin-stimulation was investigated from the view point of the change of carbonic anhydrase activity (CA), histamine content (Hm), histidine decarboxylase activity (HD) in gastric mucous membrane ; gastric mucin secretion, enzyme activity in parietal cell (histochemically on succinic dehydrogenase (SD) and CA and mitochondria activity (Mt) (phase contrast microscopically). The results obtained are as follows; 1) Both secretin (S) and gastrone (Go) inhibited the increase of CA due to gastrin-stimulation. 2) Although secretin did not show any effects on the decrease of H m and on the increase of HD due to gastrin stimulation, Go resulted in the increase of Hm and decrease of HD. 3) Both secretin and gastrone showed the same effect son gastric mucin secretion, SD, CA, and Mt on gastrin stimulation. From the above results, we noted that S and G are the same in the effects, but not in the mechanisms of inhibiton.
