The Concordance and Validity Assessment of Diagnosis for the Expert in Sasang Constitution

Abstract The Concordance and Validity Assessment of Diagnosis for the Expert in Sasang Constitution Young-Hwa Baek, Ho-Seok Kim, Si-Woo Lee, Eun-Su JangKorea Institute of Oriental Medicine ObjectivesIt is very important to diagnose Sasang Constitution (SC) correctly. The aim of this study was to reveal the concordance and validity of the specialists in Sasang Constitutional Medicine (SCM).Methods Three experts who had more than 5 years of experience and used more than 90% of Sasang Constitutional prescription in clinical practice of SCM participated in this study. The data composed of body shape, face, voice, temperament and physio-pathological symptoms were collected in the Korea Constitutional Multicenter Bank and 81 subjects’data were extracted randomly. The experts reviewed all 81 subjects data independently. Kappa analysis was conducted.Results 1) The concordance is from 52.5% to 68.4% among three experts. 2) The validity between individual expert and gold standard is from 54.3% to 63% and Kappa’s coefficients are from 0.283 to 0.421. 3) The validity is from 68.5% to 70.7% when two or three experts agree the Sasang constitution independentlyand 75.7% when all of experts agree it. ConclusionsThe concordance and validity of experts in SCM seems to be moderate. We think further study is needed. Key Words: Sasang Constitution, Concordance, Validity, Expert

An Overview on the Construction of Korea Constitutional Multicenter Bank for Sasang Constitutional Medicine

1. Objectives : The aim of the present study was to describe the Korea Constitutional Multicenter Bank (KCMB), which was various clinical data bank for scientific advancement of constitutional diagnosis and treatment. 2. Methods and Results : The KCMB is based on the 28 multi-institute cooperative clinical research networks including domestic and international institute. Since the KCMB was founded in 2006, data have been accumulated from 10,412 subjects. The KCMB consists of three parts: The clinical constitutional data, the measurement data, and the biological data. The method of data entry is via web based electronic Case Report From (eCRF). The eCRF has been developed and programed to ensure the efficient entry of data. We also developed coding manual, ongoing quality control assessment to obtain high-quality data. 3. Conclusions : The KCMB with various data contribute to providing the scientification of Sasang Constitutional Medicine.

Correlation between Positive·Negative Affect and Quality of Life in different Sasang Constitutions

Abstract Correlation between Positive·Negative Affect and Quality of Life in different Sasang Constitutions Ho-Seok Kim, Sang-Hyuk Kim, Si-Woo Lee, Young-Hwa Back, Jong-Hyang YooKorea Institute of Oriental MedicineObjectivesThe purpose of this study was to examine the effects of positive and negative emotion on the quality of life(QoL)in each Sasang contitution, using Positive Affect and Negative Affect Schedule(PANAS) and Short Form-12 HealthSurvey Questionnaire(SF-12) questionnaire.MethodsA total of 1,123 participants completed the Korean version of the PANAS and SF-12 questionnaire. The partici pantswere classified into one Sasang constitution by QSCCU . The effects of emotion on the QoL were compared betweenthe each Sasang contitution using analysis of variance(ANOVA).Results We found significant differences between constitutions in Positive Affect(PA), PA-Joy, PA-Interest and PA-Activationof PANAS. And we found significant differences between constitutions in Mental Component Summary(MCS) of SF-12. Negative correlation of less Q 0.4 was shown in Negative Affect(NA), NA-Afraid, NA-Upset of PANASand MCS of SF-12.ConclusionsThe Soyangin has significantly higher score than Soeumin in PA, PA-Joy, PA-Interest and PA-Activation of PANASas previous studies. Three-component herbal tea alleviates prolonged fatigue and improves sleep quality: A randomized controlled pilot study

Introduction: Prolonged fatigue is common among adults, yet only a small percentage of affected individuals seek medical attention. Following findings that complementary and alternative medicine can be effective strategies for improving fatigue, we evaluated the effect of an herbal tea regimen on prolonged fatigue. Methods: In this pilot randomized controlled trial, participants with prolonged fatigue were randomly allocated to either a tea (n = 20) or control (n = 20) group. The tea group consumed herbal tea consisting of Astragali Radix, Angelicae gigantis Radix, and Zizyphi Fructus twice daily for 4 weeks, whereas the control group received no intervention. Outcomes were assessed at baseline and 4 and 8 weeks. The primary outcome was fatigue severity, measured using the Chalder Fatigue Scale. Secondary outcomes included sleep quality, depression symptoms, and quality of life. Results: Among the 40 individuals screened, 35 completed the study. Fatigue severity was significantly lower in the tea group than the control group at 4 weeks, and within the tea group, fatigue severity and sleep quality improved at the 4‐ and 8‐week follow‐ups vs. baseline in the tea group. Depression also significantly improved in the tea group at 8 weeks. There were no changes within the control group on any measure. Conclusion: The findings suggest that our herbal tea formulation reduces fatigue severity and improves sleep quality in adults with prolonged fatigue.

The Association Between Sleep Duration, Sleep Quality and Obesity According to Sasang Constitution

The Association Between Sleep Duration, Sleep Quality and Obesity According to Sasang Constitution Kyoung-Sik JeongᆞSi-Woo LeeᆞHo-Seok KimᆞYoung-Hwa Baek Korea Institute of Oriental Medicine Objectives Sleep duration and sleep quality are closely related to obesity, however there are few studies considering the Sasang constitution. This study is to investigate the relationship between sleep status and obesity according to Sasang constitution. Methods This cross-sectional research studied 2,672 participants (1,293 men and 1,379 women) using the Korean Medicine Data Center (KDC) of the KIOM. Sasang constitution was diagnosed by KS-15 questionnaire. Obesity was divided into two categories: general obesity was defined as Body Mass Index(BMI) ≥25 kg/m, and abdominal obesity was defined as waist circumference ≥90 cm in men and ≥85 cm in women. Sleep quality was assessed using the Pittsburgh Sleep Quality Index. Results The distribution of the subjects was 56.9% for Taeumin and 43.1% for non-Taeeumin., Poor sleep quality increased both BMI and waist circumference in Taeeumin. In men Taeeumin, sleep duration was correlated with BMI, and sleep quality was correlated with waist circumference. The prevalence of obesity was significant difference according to sleeping duration in men Taeeumin, it was the highest at 6 hours of sleeping time. Conclusions Sleep duration and sleep quality was associated with obesity in Taeeumin. Especially, in men Taeeumin, adequate sleep duration was an important factor for prevention and treatment of obesity.

A Comparison for Constitutional Body Type between Korean and Chinese-Korean - Using Propensity Score Matching -

Abstract A Comparison for Constitutional Body Type between Korean and Chinese-Korean- Using Propensity Score Matching - Hoseok Kim, Younghwa Baek, Siwoo Lee, Jonghyang YooKorea Institute of Oriental MedicineObjectivesThe body shape was a key information for diagnosing the Sasang constitution (SC) in Sasang constitutional medicine. The aim of this study was to compare the body shapes and mainly focusing on the Korean and Chinese-KoreanMethods We calculated the propensity score for each SC type in males and females separately, and compared body shape including 8 circumference and 5 width between Korean and Chinese-Korean according to the sex and SC. ResultsKoreans have larger trunk and hip area compared to Chinese-Koreans, while Chinese-Koreans have larger abdomen compared with Koreans. Most variables were significantly different among SC types, for both Korean and Chinese-Korean. Especially, the Taeumin (TE) type has the largest body shape compared with the other SC types,it was similar between Korean and Chinese-Korean.ConclusionsThis study showed that the TE type has the largest body shape, followed by Soyangin (SY) and Soeumin (SE) in order, for both Korean and Chinese-Korean respectively. These results suggests that the body shape of Chinese-Koreanis similar with Korean based on SC type.Key Words:

A Report on the Collecting Status of the Prospective Clinical Data Included the Constitutional Prescription Sang-Hyuk Kim, Ho-Seok Kim. Ki-Hyun Park, Siwoo Lee Mibyeong Research Center, Korea Institute of Oriental Medicine Objectives The purpose of this study is to report on the collecting status of the prospective clinical data, that is designed to find clinical factors for determining the constitutional prescription. Methods We collected the prospective clinical data of 1208 subjects in the 10 oriental hospitals and 2 oriental clinic, from September 2012 to July 2015. Subjects were diagnosed and prescribed by the Sasang Constitution specialist. We counted the usage frequency of each constitutional herbal formula. Besides we check the main chief complaints on each constitutional herbal formula. Results & Conclusions In TE type, Yeoldahanso-tang, Cheongsimnyeonja-tang were used in high frequency. In SE type, Palmulgunja-tang, Gwakhyangjunggi-san were used in high frequency. In SY type, Hyeongbangjihwang-tang, Yanggyeoksanhwa-tang were used in high frequency. The both of high frequency-prescriptions in TE types were for the Interiorheat disease, expecially liver-heat lung-dry symptomatology. The main chief complaint of high frequency-prescriptions was fatigue in every constitution. We hope that the process of constitutional prescription would be standardized through the subsequent researches.

Status Report on Usage of Electronic Case Report Form for Clinical Study

With the help of information technology, it is the most important to collect clinical data correctly in clinical trials. For this, recently, in many pharmaceutical companies and Clinical Research Organizations (CRO) have tried to adopt an Electronic Case Report Form (eCRF) system. The eCRF system is a computerized system designed for collecting and managing clinical data in clinical trials. Although the eCRF is recognized as a significant system for clinical trials, until now there are no widely useable, due to the huge effort and expense at the initiatory stage of adopting eCRF system. In this study, we inquired the use of eCRF system developed for clinical trial at Korea Institute of Oriental Medicine. Especially, we focused on the time required for entering clinical data into the eCRF, hours of use, and the number of queries according to the time flow.
