The effects of altitude/hypoxic training on oxygen delivery capacity of the blood and aerobic exercise capacity in elite athletes – a meta-analysis

[Purpose] This study was designed as a meta-analysis of randomized controlled trials comparing effectiveness of altitude/hypoxic training (experimental) versus sea-level training (control) on oxygen delivery capacity of the blood and aerobic exercise capacity of elite athletes in Korea. [Methods] Databases (Research Information Service System, Korean studies Information Service System, National Assembly Library) were for randomized controlled trials comparing altitude/hypoxic training versus sea-level training in elite athletes. Studies published in Korea up to December 2015 were eligible for inclusion. Oxygen delivery capacity of the blood was quantified by red blood cell (RBC), hemoglobin (Hb), hematocrit (Hct), erythropoietin (EPO); and aerobic exercise capacity was quantified by maximal oxygen consumption (VO2max). RBC, Hb, Hct, VO2max represented heterogeneity and compared post-intervention between altitude/hypoxic training and sea-level training in elite athletes by a random effect model meta-analysis. EPO represented homogeneity and meta-analysis performed by a fixed effect model. Eight independent studies with 156 elite athletes (experimental: n = 82, control: n = 74) were included in the metaanalysis. [Results] RBC (4.499×105 cell/ul, 95 % CI: 2.469 to 6.529), Hb (5.447 g/dl, 95 % CI: 3.028 to 7.866), Hct (3.639 %, 95 % CI: 1.687 to 5.591), EPO (0.711 mU/mL, 95% CI: 0.282 to 1.140), VO2max (1.637 ml/kg/min, 95% CI: 0.599 to 1.400) showed significantly greater increase following altitude/hypoxic training, as compared with sea-level training. [Conclusion] For elite athletes in Korea, altitude/ hypoxic training appears more effective than sea-level training for improvement of oxygen delivery capacity of the blood and aerobic exercise capacity.

Hemodynamic, Hematological, and Hormonal Responses to Submaximal Exercise in Normobaric Hypoxia in Pubescent Girls.

PURPOSE The aim of this study was to investigate hemodynamic, hematological, and immunological responses to prolonged submaximal cycle ergometer exercise at a simulated altitude of 3000 m in pubescent girls. METHODS Ten girls, 12.8 ± 1.0 years old, exercised on a cycle ergometer for 60 min at a work rate corresponding to 50% maximal oxygen consumption measured at sea level, under two environmental conditions; sea level (normoxia) and a simulated 3000 m altitude (normobaric hypoxia). RESULTS There were no significant differences in tidal volume, ventilation, oxygen consumption, cardiac output, stroke volume, and heart rate between the two exercise conditions. However, reticulocyte, adrenocorticotropic hormone, and cortisol concentrations increased significantly from pre- to postexercise in the hypoxic environment. Leukocyte and T-cell count increased and B-cell count decreased after exercise under both conditions. There were no significant changes in natural killer cell count. CONCLUSION Our simulated hypoxic environment provided a mild environmental stressor that did not impose a heavy burden on the cardiovascular, hematological, or immunological functions during submaximal exercise in pubescent girls.

The Effect of 4 Weeks Fixed and Mixed Intermittent Hypoxic Training (IHT) on Respiratory Metabolic and Acid-base Response of Capillary Blood During Submaximal Bicycle Exercise in Male Elite Taekwondo Players

[Purpose] The purpose of our study was to determine the effectiveness of 4 weeks fixed and mixed intermittent hypoxic training (IHT) and its difference from exercise training at sea-level on exercise load, respiratory metabolic and acid-base response of capillary blood during 80% maximal heart rate (HRmax) bicycle exercise in male elite Taekwondo players. [Methods] Male elite Taekwondo players (n = 25 out of 33) were randomly assigned to training at sea-level (n = 8, control group), training at 16.5%O2 (2000 m) simulated hypoxic condition (n = 9, fixed IHT group), and training at 14.5%O2 (3000 m) up to 2 weeks and 16.5%O2 (2000 m) simulated hypoxic condition (n = 8, mixed IHT group) for 3 weeks. We compared their average exercise load, respiratory metabolic, and acid-base response of the capillary blood during 80% HRmax submaximal bicycle exercise before and after 4 weeks training. [Results] Fixed and mixed IHT groups showed positive improvement in respiratory metabolic and acid-base response of the capillary blood during 80% HRmax submaximal bicycle exercise after 4 weeks training. However, all dependent variables showed no significant difference between fixed IHT and mix IHT. [Conclusion] Results suggested that mixed and fixed IHT is effective in improving respiratory metabolic and acid-base response of capillary blood in male elite Taekwondo players. Thus, IHT could be a novel and effective method for improving exercise performance through respiratory metabolic and acid-base response.

Non-exercise activity thermogenesis (NEAT): a component of total daily energy expenditure

[Purpose] The purpose of this review is to promote awareness of non-exercise activity thermogenesis (NEAT) as a new strategy to increase energy expenditure, and to manage obesity. [Methods] The content of this review is based on a literature search of PubMed and the Google Scholar search engine, using the search terms obesity, energy expenditure, non-exercise activity thermogenesis (NEAT), and sitting disease. [Results] Daily energy expenditure is of great interest because most obese individuals have no exercise activity-related thermogenesis (EAT); thus their physical activity-related energy expenditure (PEE) is comprised almost entirely of NEAT. Consequently, NEAT represents the main variable component of daily total energy expenditure (TEE); this varies considerably, both within among individuals. These somewhat unplanned and unstructured low level physical activities are associated with energy expenditure in excess of the resting metabolic rate (RMR). They may therefore have the potential to stimulate greater energy expenditure over time with a higher rate of adherence. [Conclusion] In conclusion, NEAT is a highly variable component of daily TEE and a low level of NEAT is associated with obesity. NEAT enhances lifestyle, and variations in individual and environmental factors can significantly affect daily energy expenditure. Therefore, well designed longitudinal studies that focus on personal behavioral approaches and re-engineered environments to increase NEAT should be conducted in the future.

The effect of additional carbohydrate supplements for 7 days after prolonged interval exercise on exercise performance and energy metabolism during submaximal exercise in team-sports athletes

[Purpose] The purpose of our study was to determine the effectiveness of carbohydrate loading by additional carbohydrate supplements for 7 days after prolonged interval exercise on exercise performance and energy metabolism during submaximal exercise in team-sports athletes. [Methods] Twenty male team-sports athletes (14 soccer and 6 rugby players) volunteered to participate in the study and were equally divided into the experimental group (EXP, n=10) performing additional carbohydrate supplementation for 7 days after prolonged interval exercise until blood glucose level reaches 50 mg/dL or less and the control group (CON, n=10). Then, maximal oxygen consumption (VO2max) and minute ventilation (VE), oxygen consumption (VO2), carbon dioxide excretion (VCO2), respiratory exchange ratio (RER), blood glucose level, and blood lactate level were measured in all team-sports players during submaximal exercise corresponding to 70% VO2max before and after intervention. [Results] There was no significant interaction in all parameters, but team-sports players in the EXP presented more improved VO2max (CON vs EXP = vs 5.3% vs 6.3%), VE (CON vs EXP = vs 3.8% vs 6.6%), VO2 (CON vs EXP = vs 8.5% vs 9.9%), VCO2 (CON vs EXP = vs 2.8% vs 4.0%), blood glucose level (CON vs EXP = vs -12.9% vs -7.6%), and blood lactate level (CON vs EXP = -18.2% vs -25%) compared to those in the CON. [Conclusion] These findings showed that additional carbohydrate supplementation conducted in our study is not effective in exercise performance and energy metabolism during submaximal exercise.

Exposure and Exercise Training in Hypoxic Conditions as a New Obesity Therapeutic Modality: A Mini Review

Obesity is an important health problem caused by positive energy balance. Generally, low calorie dietary intake combined with regular exercise is the most common modality to lose bodily fat in obese people. Although this is the first modality of choice for obesity treatment, it needs to be applied to obese patients for at least 12 weeks or more and it does not provide consistent results because it is difficult to suppress increased appetite due to exercise. Recently, many researchers have been applying hypoxic conditions for the treatment of obesity, as many studies show that people residing in high altitudes have a lower percentage of body fat and fewer obesity-related illnesses than people living at sea level. Hypoxic therapy treatment, including hypoxic exposure or hypoxic exercise training, is recommended as a way to treat and prevent obesity by suppression of appetite, increasing basal metabolic rate and fat oxidation, and minimizing side effects. Hypoxic therapy inhibits energy intake and appetite-related hormones, and enhances various cardiovascular and metabolic function parameters. These observations indicate that hypoxic therapy is a new treatment modality for inducing fat reduction and promoting metabolic and cardiovascular health, which may be an important and necessary strategy for the treatment of obesity. As such, hypoxic therapy is now used as a general medical practice for obesity treatment in many developed countries. Therefore, hypoxic therapy could be a new, practical, and useful therapeutic modality for obesity and obesity-related comorbidities.

Association of daily physical activity level with health-related factors by gender and age-specific differences among Korean adults based on the sixth (2014-2015) Korea National Health and Nutrition Examination Survey

[Purpose] This study examined the effects of daily physical activity level on health-related factors according to gender and identified age-specific differences among Korean adults. [Methods] Using data from the Korea National Health and Nutrition Examination Survey VI (2014-2015), we selected adults aged 19-64 years who participated in both a health examination and health interview survey. The study included 6,457 participants 19-64 years of age (2,611 men, 3,846 women). [Results] Assessment of the differences in health-related factors according to age and physical activity in men and women by repeated two-way analysis of variance (ANOVA) revealed significant interaction effects on total cholesterol (TC) and triglyceride (TG) levels and diastolic blood pressure (DBP) in male participants, but there were no significant interaction effects for any health-related factors in female participants. The group of female participants aged 40-64 years with daily physical activity levels over 200 kcal showed a significantly increased prevalence of 46% for dyslipidemia compared to that in female participants with daily physical activity levels below 200 kcal. Physical activity was positively correlated with weight and high-density lipoprotein cholesterol (HDL-C) levels in men 19-39 years of age, compared to weight, waist circumference (WC), body mass index (BMI), and DBP in men 40-64 years of age, and weight, WC, BMI, glycated hemoglobin (HbA1c) and triglyceride (TG) levels in women 19-39 years of age. In women 40- 64 years of age, physical activity was especially significantly positively correlated with weight, BMI, HDL-C and negatively correlated with fasting glucose and TG levels. [Conclusion] In male and female participants, the 40-64-year age group showed negative results for health-related factors compared to those in the 19-39-year age group. The higher the weight, WC, BMI, the higher is the physical activity level. Physical activity levels were significantly positively correlated with health-related variables.

Four-week “living high training low” program enhances 3000-m and 5000-m time trials by improving energy metabolism during submaximal exercise in athletes

[Purpose] This study aimed to determine the effect of a 4-week living high training low (LHTL) versus a living low training low (LLTL) program on energy metabolism during submaximal exercise and 3000-m and 5000-m time trial (TT) in athletes. [Methods] Male athletes (n = 20) were randomly assigned to the LLTL (n = 10, living at 1000 m and training at 700–1330 m) and LHTL (n = 10, living at simulated 3000 m and training at 700–1330 m) groups. We compared energy metabolisms during submaximal exercise on a treadmill and aerobic exercise performance (3000 m and 5000 m TT) before and after 4 weeks of training. [Results] As expected, the LHTL group demonstrated enhanced energy metabolism during submaximal exercise via significant interaction (time × group) in heart rate, oxygen consumption, and carbon dioxide excretion; these variables were significantly decreased in the LHTL group compared with the LLTL group. Additionally, both training groups revealed significantly decreased blood lactate levels during submaximal exercise, 3000 m TT, and 5000 m TT but significant interactions (time × group) in the 3000 m and 5000 m TT. Thus, the LHTL group demonstrated greater improvements in 3000 m and 5000 m TT than the LLTL group via significant interactions. [Conclusion] Our results suggest that 4-week LHTL intervention enhances 3000 m and 5000 m TT by improving energy metabolism during submaximal exercise. The proposed LHTL intervention in this study is a novel and effective method for improving aerobic exercise performance in male athletes.

Development of an anaerobic threshold (HRLT, HRVT) estimation equation using the heart rate threshold (HRT) during the treadmill incremental exercise test

[Purpose] The purpose of this study was to develop a regression model to estimate the heart rate at the lactate threshold (HRLT) and the heart rate at the ventilatory threshold (HRVT) using the heart rate threshold (HRT), and to test the validity of the regression model. [Methods] We performed a graded exercise test with a treadmill in 220 normal individuals (men: 112, women: 108) aged 20–59 years. HRT, HRLT, and HRVT were measured in all subjects. A regression model was developed to estimate HRLT and HRVT using HRT with 70% of the data (men: 79, women: 76) through randomization (7:3), with the Bernoulli trial. The validity of the regression model developed with the remaining 30% of the data (men: 33, women: 32) was also examined. [Results] Based on the regression coefficient, we found that the independent variable HRT was a significant variable in all regression models. The adjusted R2 of the developed regression models averaged about 70%, and the standard error of estimation of the validity test results was 11 bpm, which is similar to that of the developed model. [Conclusion] These results suggest that HRT is a useful parameter for predicting HRLT and HRVT.

Application of “living high-training low” enhances cardiac function and skeletal muscle oxygenation during submaximal exercises in athletes

[Purpose] The aim of this study was to determine the efficiency of the application of living high-training low (LHTL) on cardiac function and skeletal muscle oxygenation during submaximal exercises compared with that of living low-training low (LLTL) in athletes. [Methods] Male middle- and long-distance runners (n = 20) were randomly assigned into the LLTL group (n = 10, living at 1000-m altitude and training at 700-1330-m altitude) and the LHTL group (n = 10, living at simulated 3000-m altitude and training at 700-1330-m altitude). Their cardiac function and skeletal muscle oxygenation during submaximal exercises at sea level before and after training at each environmental condition were evaluated. [Results] There was a significant interaction only in the stroke volume (SV); however, the heart rate (HR), end-diastolic volume (EDV), and end-systolic volume (ESV) showed significant main effects within time; HR and SV significantly increased during training in the LHTL group compared with those in the LLTL group. EDV also significantly increased during training in both groups; however, the LHTL group had a higher increase than the LLTL group. ESV significantly increased during training in the LLTL group. There was no significant difference in the ejection fraction and cardiac output. The skeletal muscle oxygen profiles had no significant differences but improved in the LHTL group compared with those in the LLTL group. [Conclusion] LHTL can yield favorable effects on cardiac function by improving the HR, SV, EDV, and ESV during submaximal exercises compared with LLTL in athletes.
