Analysis of the Potent Prognostic Factors in Luminal-Type Breast Cancer

Purpose Luminal-type breast cancer has a good prognosis compared to other types, such as human epidermal growth factor receptor 2 and triple negative types. Luminal-type breast cancer is classified into luminal A and B, according to the proliferation index. We investigated the clinicopathological factors that affect the prognosis of the luminal-type subgroups. Methods We reviewed the medical records and the pathologic reports of 159 luminal-type breast cancer patients who were treated between February 2005 and November 2007. We divided luminal-type breast cancer into luminal A and B, according to Ki-67 (cutoff value, 14%) and analyzed the clinicopathologic factors, such as age at diagnosis, intensity score of estrogen receptor and progesterone receptor, histologic grade, and Bcl-2. Moreover, we compared the disease-free survival (DFS) of each group. Results In the univariate analysis, age (p=0.004), tumor size (p=0.010), lymph node metastasis (p=0.001), and Bcl-2 (p=0.002) were statistically significant factors in luminal-type breast cancer. In the multivariate analysis, lymph node (p=0.049) and Bcl-2 (p=0.034) were significant relevant factors in luminal-type breast cancer. In the subgroup analysis, the increased Bcl-2 (cutoff value, 33%) was related with a longer DFS in the luminal B group (p=0.004). Conclusion In our study, luminal A breast cancer showed a longer DFS than luminal B breast cancer, further, Bcl-2 may be a potent prognostic factor in luminal-type breast cancer.

Comparison of early postoperative decline of serum antiMüllerian hormone levels after unilateral laparoscopic ovarian cystectomy between patients categorized according to histologic diagnosis

OBJECTIVE To evaluate the change of ovarian reserve after unilateral laparoscopic ovarian cystectomy (LOC) in the early postoperative period between patients divided according to histologic diagnosis of cyst type. MATERIALS AND METHODS Sixty-five patients who were undergoing unilateral LOC for benign ovarian disease were included in this study. All participants were divided into three groups according to histologic diagnosis: endometrioma (n=26), mature teratoma (n=24), and other benign cyst (n=15). Serum samples were collected preoperatively and 3-days postoperatively and assayed for antiMüllerian hormone (AMH) levels, and the changes between the two samples were analyzed. RESULTS Preoperative AMH levels were not significantly different between the three groups. On postoperative Day 3 AMH levels of all three groups were significantly reduced compared with preoperative AMH levels. The rates of decline of AMH levels after LOC in patients with endometrioma (26.37±20.70%) and in those with teratoma (28.25±22.16%) were both significantly higher than those with other benign cyst (12.03±18.56%). No statistically significant differences were found in the rate of decline of AMH levels between patients with endometrioma and teratoma. Decline of AMH after LOC was not significantly correlated with age, body mass index, the size of cyst, or preoperative AMH levels. CONCLUSION Our results suggest that ovarian reserve is reduced after unilateral LOC in the early postoperative period, and that the postoperative decline of AMH levels after LOC is similar between patients with endometrioma and those with mature teratoma.

Correlation Between Cervical Lengths Measured by Transabdominal and Transvaginal Sonography for Predicting Preterm Birth

Cervical length assessment is important for predicting preterm birth. Cervical length measurement using transabdominal sonography is more comfortable for the patient but has more limitations than transvaginal sonography. Our aim was to determine whether transabdominal sonography could identify those women who should undergo transvaginal sonography for prediction of preterm birth.

Study on the communication system for Korean emergency medical dispatch

The emergency cases are increasing in proportion to the growing complexity of our society, and the emergency diseases and hazards present other problems in health and welfare. We are required to develop our emergency care system which remains behind the foreign countries both in quality and quantity. Since the most crucial part of the emergency system is the communication system, our own communication system of Korean Emergency Medical Dispatch Priority Reference System (K-EMDPRS) has been developed. It consists of four protocols focusing on the easy use for the emergency communication. And it is expected that this K-EMDPRS contribute to the development of our emergency care system.

Neonatal Hemoperitoneum Secondary to Hemorrhage from Omental Venous Malformation

Hemoperitoneum in the neonate is uncommon. We report our experience with a neonate who suddenly developed hemoperitoneum in late gestation. Ascites with right pleural effusion initially suggested fetal hydrops. However, a decrease in hemoglobin with bloody ascites from the paracentesis gave the impression of hemoperitoneum. Ultrasonography and computed tomography showed no abnormality in the solid organs. Using 3-mm laparoscopic exploration, we found the omental mass and excised it. It was histologically confirmed as a venous malformation. Venous malformation of the omentum with significant bleeding requiring exploration is a very rare condition in the neonate, but it should be considered as a cause of neonatal hemoperitoneum.

Pregnancy in woman with Klippel-Trenaunay syndrome: A case report

The Klippel-Trenaunay syndrome is rare congenital disease characterized by a triad of cutaneous vascular malformations, varicose veins, and hypertrophy of bone and soft tissue. Although several cases of pregnancy in women with Klippel-Trenaunay syndrome were reported as there may be increased risk of pregnancyand/or delivery-associated complications such as varicosities, thromboembolism and hemorrhage, there is little general information regarding how to deal with pregnancy in patients with Klippel-Trenaunay syndrome and their obstetric outcome. We describe the case of a woman suffering from Klippel-Trenaunay syndrome delivering a healthy child with a brief review of concerned literature.

IMPACT OF HYSTERECTOMY ON OVARIAN RESERVE IN EARLY POSTOPERATIVE PERIOD

Methods We retrospectively reviewed the medical records and laboratory data of patients aged 30-50 years with regular menstruation before hysterectomy, and 31 patients were included in this study. All patients enrolled underwent scheduled hysterectomy with conservation of both ovaries (laparoscopy-assisted vaginal hysterectomy, n = 21; and total abdominal hysterectomy, n = 10), and received serum anti-Mullerian hormone (AMH) tests preoperatively and 3 days after operation. Changes between preoperative and postoperative AMH levels of each patient were calculated.

A case of congenital high airway obstruction syndrome caused by complete tracheal obstruction with associated anomalies

An obstruction of the fetal upper airway in utero due to various reasons may cause a fatal condition congenital high airway obstruction syndrome (CHAOS). The prenatal ultrasound fi ndings of CHAOS include large echogenic lungs, fl attened or inverted diaphragms, dilated airways distal to the obstruction, and fetal ascites and/or hydrops [1]. If it is associated with severe hydrops and/ or associated anomalies, its prognosis is fatal. Here we present a case of antenatally diagnosed CHAOS with associated anomalies and a literature review.
