H-reflex of the upper extremities in cervical myelopathy.

SummaryA study was made of the H-reflex in the upper extremities to express objectively the degree of spastic paralysis in cases of cervical myelopathy. This reflex cannot normally be elicited in healthy persons. The H-reflex was elicited in 63 out of 80 patients with advanced symptoms of cervical myelopathy and was most frequently recorded from the median nerve. The H/M amplitude ratio is used to express the degree of spastic paralysis, the higher the ratio the more severe the dysfunction. The longer the clinical history the greater the tendency for the H-type of reflex to change to the HF-type and the higher the H/M amplitude ratio. The decrease rate of the pre- and post-operative H-reflex was high amongst soft disc cases whilst that in spondylosis and ossification of the posterior longitudinal ligament was low compared to the improvement in symptoms. It is concluded that the H-reflex in the upper extremities is a good method of expressing the degree of spastic paralysis in cervical myelopathy.RésuméAfin dobtenir une représentation plus objective du degré de spasticité dans les paralysies dues à une myélopathie cervicale, les auteurs ont effectué une étude du réflexe H au niveau des membres supérieurs. Ce réflexe, qui ne peut être retrouvé chez les sujets normaux, a été mis en évidence chez 63 des 80 patients présentant des signes sévères de myélopathie cervicale, plus particuliérement en cas datteinte du nerf médian. Le rapport damplitude H/M permet dexprimer le degré de spasticité: plus ce rapport est élevé, plus la perturbation fonctionnelle est importante. Plus la maladie est ancienne, plus le type H tend à évoluer vers le type HF et plus le rapport damplitude H/M a tendance à augmenter. Dans les cas de ramollissement discal le rapport H/M diminue de façon importante entre les phases pré- et post-opératoires. A linverse, en cas darthrose cervicale ou dossification du ligament commun vertébral postérieur, la diminution est faible comparativement à lamélioration clinique. Le réflexe H au niveau des membres supérieurs constitue une bonne méthode dappréciation du degré de spasticité dans les paralysies secondaires à une myélopathie cervicale.
