This paper presents a mobile and unobtrusive platform that enables the accurate monitoring of physical activities in daily life, and is integrated into a healthcare system supporting out-of-hospital services. The main focus of the paper is to describe and evaluate a complete data processing chain for recognizing different activities, and estimating their intensity level. To develop a mobile platform that is more applicable in real-life scenarios than systems in previous work, an additional other activity class is introduced to deal with various everyday, household and fitness activities. Evaluation of the proposed methods is done on a dataset recorded by 9 subjects performing 16 different activities, while wearing the mobile platform consisting of three IMUs, a HR-monitor and a mobile companion unit. Another important aspect of this work is the integration of the mobile platform with an EHR, providing access for both the clinician (e.g. to enter a patients medical record or to set up a care plan) and the patient (e.g. to watch assigned educational material). Feedback about the patients daily progress is also given both to the patient and the clinician, preserving or even increasing the patients motivation to follow the defined care plan, and providing valuable information on program adherence for the clinician.

Stroke is a disease with very high socio-economic impact. In average, the healthcare expenditure cost for Strokes across different countries in Europe and USA exceeds 3% of their entire healthcare expenditure, including inpatient treatments, outpatient hospital visits and long-term rehabilitation and care1 Therefore, there is an urgent need for devising an effective long-term care and rehabilitation strategy for stroke patients, which would actively involve patients in the rehabilitation process while minimizing costly human support. This paper reports on the results of the FP7-StrokeBack project where game-based training system has been proposed allowing physicians to supervise the rehabilitation of patents at home. The proposed approach empowers the patients and their caretakers for effective application of rehabilitation protocols in their home settings, while leading physicians are enabled to supervise the progress of the rehabilitation (and intervene if needed) through the use of Personal Health Record (PHR) system. The increased rehabilitation speed and ability to perform training at home directly improves quality of life of patients.

This chapter describes the general look and feel of the StrokeBack system. It concentrates on real-world requirements and previous experiences in telemedicine work and provides the blue print for the StrokeBack system development. The implementation and integration of the overall project solution into a “Smart Table” and “Mobile Station” of MEYTEC is described. In the follow-up aspects related to exploitation of the project results, commercialization and business potential are discussed. It provides the details of the associated market, the profiles of the tentative clients and the opportunities for further post-project exploitation. The same is done for by-products of the project. In addition the reader is provided with details related to regulatory framework regarding medical devices and software and the assessment of the consortium regarding a characterization of exploitable outcomes from this framework.

The aim of this Chapter is to present the basic characteristics of the Personal Health Record Systems (PHR-S) and indicate how the relevant platforms can serve as a basis for developing value added services such as a Care Management service. The basic features of a generic architectural approach for PHR-S are presented as well as the business challenges addressed by the proposed approach. In the context of StrokeBack an ICT-based post-stroke rehabilitation service was implemented. In order to support this service we designed and implemented a set of applications on top of the proposed PHR-S framework. The main features of these applications are also presented in this chapter.
