Risk factors for falls in hospitalized adult patients: an integrative review

Objetivo: Identificar os fatores de risco para a ocorrencia de quedas em pacientes adultos hospitalizados. Metodo: Revisao integrativa realizada nas bases de dados LILACS, SciELO, MEDLINE e Web of Science, abrangendo artigos publicados entre 1989 e 2012. Resultados: Setenta e um artigos compuseram a amostra final do estudo. Os fatores de risco para quedas apresentados nesta revisao foram relacionados ao paciente (intrinsecos), ao ambiente hospitalar e ao processo de trabalho dos profissionais da saude, em especial a enfermagem (extrinsecos). Conclusao: A triagem sistematica de fatores de risco para queda foi identificada como fator contribuinte para a reducao desse agravo, auxiliando a nao ocorrencia deste evento que, apesar de ser prevenivel, pode determinar consequencias graves incluindo o obito.OBJECTIVE Identifying risk factors for the occurrence of falls in hospitalized adult patients. Laser therapy in pressure ulcers: evaluation by the Pressure Ulcer Scale for Healing and Nursing Outcomes Classification

OBJECTIVE To describe the pressure ulcer healing process in critically ill patients treated with conventional dressing therapy plus low-intensity laser therapy evaluated by the Pressure Ulcer Scale for Healing (PUSH) and the result of Wound Healing: Secondary Intention, according to the Nursing Outcomes Classification (NOC). METHOD Case report study according to nursing process conducted with an Intensive Care Unit patient. Data were collected with an instrument containing the PUSH and the result of the NOC. In the analysis we used descriptive statistics, considering the scores obtained on the instrument. RESULTS A reduction in the size of lesions of 7cm to 1.5cm of length and 6cm to 1.1cm width, in addition to the increase of epithelial tissue and granulation, decreased secretion and odor. CONCLUSION There was improvement in the healing process of the lesion treated with adjuvant therapy and the use of NOC allowed a more detailed and accurate assessment than the PUSH.

Tempo despendido na execução do processo de enfermagem em um centro de tratamento intensivo

Este estudio tuvo como objetivo medir el tiempo dedicado por el personal de enfermeria en una Unidad de Cuidados Intensivos (UCI) en la aplicacion de los pasos del Proceso de Enfermeria (PE). Se trata de una investigacion descriptiva, observacional y exploratoria, utilizando el metodo cuantitativo. Fueron cronometradas las etapas del PE llevadas a cabo por seis enfermeros en 29 pacientes internados en la UCI. El estudio fue aprobado por la Comision de Etica en Investigacion de la Institucion. Se midio el tiempo en cada fase: evaluacion diaria del paciente: 8,34 (3,78); diagnostico y la prescripcion de enfermeria: 3.65 (2.27-5.45); programacion de la prescripcion en enfermeria: 2.30 (1, 14); y la evolucion de asistencia: 11,29 (2,55). El enfermero paso 25.58 minutos por paciente en la realizacion del PE. Los datos fueron importantes para dimensionar los recursos humanos necesarios para el cuidado de pacientes criticos, con el fin de calificar la asistencia.Resumen This study had as a goal the measure of time spent on an Intensive Care Unit (ICU) by nurses when carrying out the steps of the Nursing Process (NP). It is a descriptive research, observational and exploratory, using quantitative methodology. Steps of the NP as carried out were timed by six nurses on 29 ICU patients. Research was approved by the Institutional Ethics Review Board. Time was measured in the following steps: daily patient assessment: 8.34 (3.78); diagnosis and nursing prescription: 3.65 (2.27-5.45); scheduling nursing prescription: 2.30 (1.14) and evaluation of care: 11.29 (2.55). Nurses spent 25.58 minutes per patient when carrying out the NP. The data provide a way to measure human resources required for caring for critical patients, so as to qualify care giving.

Risk factors for fall occurrence in hospitalized adult patients: a case-control study

ABSTRACT Objective: to identify risk factors for falls in hospitalized adult patients. Methods: a matched case-control study (one control for each case). A quantitative study conducted in clinical and surgical units of a teaching hospital in Southern Brazil. The sample comprised 358 patients. Data were collected over 18 months between 2013-2014. Data analysis was performed with descriptive statistics and conditional logistic regression using Microsoft Excel and SPSS version 18.0. Results: risk factors identified were: disorientation/confusion [OR 4.25 (1.99 to 9.08), p<0.001]; frequent urination [OR 4.50 (1.86 to 10.87), p=0.001]; walking limitation [OR 4.34 (2.05 to 9.14), p<0.001]; absence of caregiver [OR 0.37 (0.22 to 0.63), p<0.001]; postoperative period [OR 0.50 (0.26 to 0.94), p=0.03]; and number of medications administered within 72 hours prior the fall [OR 1.20 (1.04 to 1.39) p=0.01]. Conclusion: risk for falls is multifactorial. However, understanding these factors provides support to clinical decision-making and positively influences patient safety.
