Foreign body in the stomach: Dentures

İsmail Zihni; Oktay Karaköse; Kazım Çağlar Özçelik; Hüseyin Pülat; Ali Duran

Although it is mostly observed in childhood, swallowing foreign bodies causes morbidity and mortality of a high percentage in the adult age group. Swallowed foreign bodies can be both harmless as well as life-threatening. The number, shape, size, and time of the swallowed foreign body to get deposited in the specific location determines the type of treatment. Late diagnosis and treatment is an important problem that causes perforation and obstruction. In this article, we presented a 39-year-old male patient who swallowed his denture while sleeping.

Introduction:Ancient schwannoma is a rare tumor of the peripheral nerve sheath. As degenerative properties are defined histologically, it can be wrongly interpreted as malignant. Case presentation:The case presented here is of a giant ancient schwannoma with a pelvic retroperitoneal location, which was mimicking an adnexal mass. Conclusion:In the rarely seen cases in the retroperitoneum, it may reach very large dimensions.

Background Planned re-laparotomies are a series of surgical interventions that are rarely used and have a high mortality rate. The aim of this study was to investigate the factors affecting mortality and the effectiveness of the use of the Bogota bag for temporary closure of the abdomen in patients for whom re-laparotomy was planned. Material/Methods A retrospective examination was made of data of patients in whom a Bogota bag was used in planned re-laparotomies for various reasons in the General Surgery Department of Suleyman Demirel University Medical Faculty between June 2008 and April 2014. Results Bogota bags were used in a total of 38 patients, comprising 23 (60.5%) males and 15 (39.5%) females, with a mean age of 58.94±17.89 years. The mean period of hospitalization was 14.5 days (range, 1–143 days) and the mean number of operations during that time was 3 (range, 1–11). The mean duration of intensive care unit stay was 6 days (range, 1–143 days). Malignancy was determined in 8 patients (21.1%). Indications were intra-abdominal sepsis in 23 patients (60.5%), mesenteric vascular disease in 10 patients (26.3%), and packing was required in 5 patients (13.2%). Mortality developed in 25 patients (65.8%). A significant relationship was determined between mortality and a diagnosis of mesenteric artery ischemia (p: 0.035). The mortality rate was 56% (n: 13) in patients diagnosed with intra-abdominal sepsis. A relationship was determined between mortality and age (p: 0.015), duration of hospital stay (p: 0.007), need for cardiac inotrope (p: 0.01), and need for mechanical ventilation (p: 0.01). The mean Apache II score was 26.4±5 for patients who died and 15.8±5.2 for surviving patients (p<0.001). In 5 (38.4%) of the 13 surviving patients, primary repair was applied to the abdomen, and in the remaining 8 patients abdominal wall repair was performed using dual mesh. Conclusions In patients in whom a Bogota bag was used, which is a cheap and easy method for temporary closure of the abdomen, the high mortality rates seen are related to diagnosis, Apache II score, age, and organ failure.

Kolonda nonobstruktif kitle nedenleri arasinda cok nadir gorulen, siklikla akla pek gelmeyen nedenlerden biri de ekstrameduller plazmasitomlardir. Plazmasitomlar plazma hucrelerinin anormal proliferasyonu sonucunda olusur- lar. Anahtar Kelimeler: Ekstrameduller plazmasitom, multipl miyelom, kolon kanserleri A rare and elusive cause of nonobstructive mass in colon is extramedullary plasmacytoma. Plasmacytoma is a neo- plastic proliferation of plasma cells; when systemic involvement occurs, the disease is named multiple myeloma. Extramedullary plasmacytomas comprise 4% of all plasma cell tumours and occur mainly in the upper respiratory tract; gastrointestinal system involvement is rare. In this case we describe a rare cause of gastrointestinal system mass: extramedullary plasmacytoma involving the left colon.Extramedullary plasmacytoma is a rare cause of nonobstructive colonic masses, which is often disregarded. Nadir bir inguinal herni nedeni: skrotal sistosel

İsmail Zihni; Ali Duran; Volkan Soysal

Herniation of the urinary bladder into the inguinal canal is rare. It constitutes 1-3% of all inguinal hernias. Bladder herniation is usually asymptomatic, and it is usually diagnosed during surgery or as a result of intra-operative bladder injury. Male patients with lower urinary tract symptoms or with a history of previous hernia surgery are at increased risk of bladder herniation. Typically, patients complain of inguinal or scrotal swelling, dysuria and shrinkage of scrotal swelling after voiding. Early diagnosis with radiological imaging is important to prevent complications during surgery. Intravenous pyelography, retrograde cystography, pelvic ultrasound, computed tomography and magnetic resonance imaging can be used for preoperative diagnosis. In this report, a patient is presented who presented to our clinic with scrotal swelling and was diagnosed with scrotal bladder herniation by retrograde cystography, taken due to clinical suspicion based on his history.Herniation of the urinary bladder into the inguinal canal is rare. Urachal cyst is an unusual clinical condition, which is usually asymptomatic. In some adult cases, it may lead to complications. The cyst is between umbilicus and urinary bladder. It is diagnosed via ultrasonography and computed tomography. However, in some cases, the diagnosis is made by means of surgical exploration and histopathological evaluation. In this paper, we report a case of a 17-year-old female presenting with painful abdominal mass. At the first evaluation, the case was diagnosed as a mesenteric cyst because the mass located in the mesentery, and final histopathological report revealed the urachal cyst.

çağlar kazım özçelik; İsmail Zihni; Oktay Karaköse; Hüseyin Pülat; Hasan Erol Eroğlu

Aim/Objective: Retroperitoneal soft tissue sarcoma; epidemiological, histopathological features of cases in our clinic, treatments and their clinical courses were compared with the literature. Materials and Methods: Records of 7 patients who were operated in Surgical Oncology Clinics of Suleyman Demirel University between 2007-2013 after being detected with primary retroperitoneal- intraperitoneal masses in imagings and being reported as soft tissue tumor in histopathological examination were analyzed retrospectively. Results: The patients were 6 males and 1 female. Abdominal tomography was applied to all patients and in imagings there were both intraperitoneal and retroperitoneal masses in 4 patients (57,1%), left retroperitoneal masses in 2 patients (28,6%) and intraperitoneal mass in 1 patient (14,3%). Distant metastases weren’t detected in any of the patients at the time of diagnosis. While mass excision was done in 2 patients, enbloc organs resection (colon and kidney) was done along with excision in 3 patients, two patients were admitted as unresectable. In pathological examination, 42,9% (in 3 patient) good differential liposarcoma, 28,6% (in 2 patient) leiomyosarcoma and 14.3% (1 patient) dedifferentiated liposarcoma and rhabdomyosarcoma were detected. Recurrence developed in 2 patients who were applied mass excision in 11th month (10th month and 12th month) on the average. Lung metastases developed in one patient in the 6th month who were applied enbloc organ and mass resection. Conclusion: Retroperitoneal soft tissue sarcomas are rarely seen. However, recurrence often develops generally due to incomplete surgery. Therefore, aggressive surgery is recommended in these tumors in order to provide negative surgery border
