The end of the beginning: context of death at birth in the Czech Republic

ABSTRACT The Czech Republic represents a very secular, Post-Socialist country, with top-ranking medical care in biomedical standards. Nevertheless, issues of death, bereavement and associated rituals have been displaced to the very edge of both social policy and public debate. The article analyses the context of perinatal loss as an experience of end of life at its very beginning. It frames the institutional setting and symbolic context of and for such ‘inappropriate’ event, employing conceptual approaches of authoritative knowledge associated with perinatal loss. The article elaborates the current medicalised practice and cultural rituals. One perspective is the ambiguity of the definition of a stillbirth with competing sets of knowledge claiming their expertise over this issue. Another perspective is represented by the rules and practice for burying bodies treated until very lately as biological waste and disposed accordingly. The appeal to ‘healthy population’ forms the atmosphere for dealing with death itself. Third, interplay among interest groups advocating for or against the status quo is elaborated on. These are complemented by two additional perspectives; the ceasing practice of public last rites and by evidence from parental discussion forum, helping us diversify the subject matter under study in a specific Post-Socialist context.

Postoje českých lékařů k medicíně a ke změnám v praxi reprodukční medicíny

Soucasne trendy v poskytovani zdravotni pece jsou spolecenskými vědci popisovany prostřednictvim konceptů jako je deprofesionalizace, rutinizace, proletarizace nebo komercionalizace. Tyto koncepty mohou poskytnout vhled take do situace v ceskem zdravotnictvi. Clanek je přispěvkem k debatam o situaci v ceske medicině. Vychazi z dotaznikoveho setřeni lekařů, ktere proběhlo v roce 2012, a v němž se lekaři vyjadřovali k situaci v ceske medicině. Byli dotazovani take na nazory k potencialnim změnam v reprodukcni medicině, jako je porod mimo nemocnici, dostupnost asistovane reprodukce pro ženy bez partnera, provaděni lekařských řezů na žadost pacientek, manipulace s DNA a embryi atd. Výsledky výzkumu narusuji obrazek homogenity lekařských nazorů. Ukazuji výrazne rozdily v postojich ceských lekařů, ktere jsou do znacne miry zavisle na jejich pracovnim prostředi, a na jiných charakteristikach, jako je pohlavi, věk nebo naboženstvi. Postoje některých lekařů, týkajici se asistovane reprodukce nebo změn v reprodukcni medicině, podporuji požadavky na změny, ktere zaznivaji ze strany pacientů. Tento trend je výraznějsi u lekařů gynekologů a porodniků. Odpovědim na otevřenou otazku, ktera se týkala největsich problemů v soucasne medicině, dominovaly výpovědi souvisejici se zkusenosti s pacienty a take s obecnějsimi problemy v organizaci zdravotni pece v CR.

Configurations of Europe

This book has sought to gender men in Europe by drawing on various sources of information and in relation to key themes and policy arenas. In this final chapter, we consider some broad issues concerning the position and impact of men in the context of changing configurations of Europe, including, but not only, in relation to the EU. There are many ways in which the more explicit gendering of men raises new sets of questions about the EU and the changing definitions of ‘Europe’.
