An alternative experimental model for training in microsurgery

OBJECTIVE to describe a new model of training in microsurgery with pig spleen after splenectomy performed by undergraduate students of the Discipline of Operative Technique of the UFPR Medical School. METHODS after the completion of splenectomy we performed dissection of the vascular pedicle, distal and proximal to the ligation performed for removal of the spleen. After complete dissection of the splenic artery and vein with microscope, clamps were placed and the vessels were cut. We then made the anastomosis of the vessels with 9.0 nylon. RESULT the microsurgical training with a well-defined routine, qualified supervision and using low cost experimental materials proved to be effective in the practice of initial microvascular surgery. CONCLUSION the use of pig spleen, which would be discarded after splenectomy, is an excellent model for microsurgical training, since besides having the consistency and sensitivity of a real model, it saves the sacrifice of a new animal model in the initial learning phase of this technique.

Qual a durabilidade da prótese glútea

BACKGROUND: Over the last 3 years, there has been an increase in the number of patients requesting gluteoplasty. Despite this increase, there are no published data on the durability of gluteal prostheses. This study evaluated the mean useful life of silicone implants for gluteoplasty. METHODS: This was a retrospective observational study. A total of 380 medical records were reviewed to obtain data on age, the date of prosthesis insertion, and the date of reoperation. The mean time between the first surgery for insertion of the implant and the second surgery for exchanging it for a new one or to remove it was calculated. RESULTS: Of the 380 cases evaluated, 70 patients were reoperated on by the senior author; only 2.8% of these cases showed an intact prosthesis during the reoperation, and all others presented with a ruptured prosthesis. The right and left buttock prostheses were ruptured in 100% and 80% of the cases, respectively. CONCLUSIONS: The useful life of gluteal implants is shorter than that of breast implants despite being manufactured from the same material. This is because gluteal prostheses are subjected to more tension and compression, which are associated with the lifestyle and job of the patient.

Acute Abdomen After Abdominoplasty: Differential Diagnosis

Abdominoplasty is a very common elective procedure in plastic surgery. Surgical accidents can lead to serious complications that result in an acute surgical abdomen, but in rare cases, an acute surgical abdomen not related to the procedure develops. This report discusses the diagnostic approach for patients with an acute abdomen after abdominoplasty and presents a case of appendicitis in the immediate postoperative period. The importance of postoperative follow-up evaluation by the same surgeon who performed the procedure is discussed as well as the need for adequate general surgery training to obtain the correct diagnosis and perform the urgent operation required to avoid a detrimental outcome.Level of Evidence IVThis journal requires that authors assign a level of evidence to each article. Is professional recognition in plastic surgery related to activity in research

OBJECTIVE To evaluate the relation of medical research, with the participation of prominent plastic surgeon in Congress. METHODS We reviewed the scientific programs of the last 3 Brazilian Congress of Surgery, were selected 21 Brazilian plástic surgeons invited to serve as panelists or speakers in roundtable sessions in the last 3 congresses (Group 1). We randomly selected and paired by other members (associates) of the Brazilian Society of Plastic Surgery, with no participation in congress as speaker (Group 2). We conducted a search for articles published in journals indexed in Medline, Lilacs and SciELO for all doctors selected during the entire academic career and the last 5 years from March 2007 until March 2012. We assessed the research activity through the simple counting of the number of publications in indexed journals for each professional. The number of publications groups was compared. RESULTS articles produced throughout career: Group 1- 639 articles (average of 30.42 items each). Group 2- 79 articles (mean 3.95 articles each). Difference between medias: p <0.001. CONCLUSION The results demonstrate that the Brazilian Society of Plastic Surgery seeking professionals with a greater number of publications and journals of higher impact. This approach encourages new members to pursue a higher qualification, and give security to congressmen, they can rely on the existence of a technical criterion in the choice of speakers.

Síndrome de Romberg: uma série de casos

BACKGROUND: The Rombergs syndrome is a disease that was first described more than a century ago, and during this period had several names. It is characterized by a slow and progressive atrophy of the tissues of a hemiface, can affect all tissues and can also display neurological and ocular manifestations. The aim of this study is to report a series of patients with progressive hemifacial atrophy, addressing treatment options for each individual case. METHODS: Was conducted a retrospective, descriptive and analytic study of the patients treated in our service, Plastic Surgery at the Hospital de Clinicas - Universidade Federal do Parana, and the Centro de Atendimento Integral ao Fissurado Labio Palatal (CAIF), at Curitiba, PR, Brazil. RESULTS: Were analyzed 13 patients, 10 (76.9%) female and 3 (23.1%) male. The age of onset of symptoms ranged from 2 to 15 years. The first medical consultation with an specialist occurred at 11.7 years. The left hemiface was most commonly affected. Seven (53.8%) patients underwent surgery. CONCLUSIONS: The Rombergs syndrome is a rare disorder, devastating to facial appearance, and it has been widely studied. Despite great advances in medicine, its etiology definition and the treatment directed to its cause is still only a wish.

Escolha do vaso receptor em reconstrução de mama microcirúrgica

BACKGROUND: Microvascular transfer of autogenous tissue have become the gold standard for breast reconstruction. As in any free tissue reconstruction, recipient vessel choice is fundamental for adequate planning in breast reconstruction. The purpose of the present study is to determine which of the available recipient vessels (the internal mammary artery and its perforators vessels or circumflex scapular vessels) are adequate for microvascular breast reconstruction. METHODS: A retrospective analysis of 117 consecutive patients who underwent microvascular breast reconstruction between January 2005 and December 2007 was performed. An algorithm that could be applied to the selection of the recipient vessel based in the axillary node dissection, immediate or late reconstruction, preoperative radiotherapy was established. Flap related complications, conversion rate and clinical outcomes were analised. RESULTS: The internal mammary perforator, the internal mammary and the circumflex scapular are adequate recipient vessels for breast reconstruction, with similar rates of complications and viability. We also observed a lower flap viability rate when using superficial inferior epigastric artery flap comparing to deep inferior epigastric artery perfurator and transverse rectus abdominis musculocutaneous with muscle preservation flaps. CONCLUSIONS: Microsurgical breast reconstruction is a safe and reliable method, with high flap viability and low complications.

Prophylaxis of fat embolism syndrome: a current analysis

Introduction: To analyze data from the internet on deaths from fat embolism, time of onset, and other information that could determine current reality in Brazil regarding fat embolism syndrome incidence and any ,media repercussions, and also to review methods of prevention and what are the best methods available to treat this disease. Methods: A Google search was conducted from January 2000 to January 2014 using the keywords “plastic surgery” and “death.” We included and reviewed articles containing the words “embolism”, “fat embolism” and “complications in (or of) plastic surgery”. Results: We included 235 relevant news stories over the 14 included years. There were 45 cases of death related with plastic surgery that offered few data for individualization. Of these patients, 44 were women. Possible causes mentioned were pulmonary embolism (five cases), perforation of viscera (four cases), malignant hyperthermia (three cases), anesthesia (two cases), anaphylactic shock (two cases), fat embolism (one confirmed case), and “other” (five cases). Conclusion: Guidelines to prevent fat embolism in plastic surgery are needed, however, there is also the need of more evidence based studies to understand more clearly what methods are best.

Galactorrhea: how to address this unusual complication after augmentation mammoplasty

Introduction: Galactorrhea and galactocele formation after breast augmentation are complications reported in the literature, but the cause remains unknown. Methods: We present a case of a 28-year-old patient who underwent breast augmentation surgery via the inframammary fold with an implant placed in the subfascial plane, which developed galactorrhea from the incision on the seventh postoperative day, and we propose an algorithm for the diagnosis and treatment of galactorrhea after mammoplasties. Results: The complication was treated with the use of a lactation suppressor, cabergoline, presenting good outcomes. Conclusion: Galactorrhea is an uncommon complication after augmentation mammoplasties, which should always be considered in cases of secretions from an incision because it is a differential diagnosis for infections. ■ ABSTRACT

Therapeutic options in the management of giant congenital nevus: Experience of the Plastic and Reconstructing Service at Hospital de Clínicas, Federal University of Paraná

Introdução: O nevus congênito gigante (NCG) possui diversas modalidades de tratamento. A abordagem cirúrgica inclui a ressecção parcial seriada ou excisão total. Objetivo: Demonstrar as principais modalidades terapêuticas utilizadas para o tratamento do NCG e avaliar a incidência da localização, idade e tamanho destas lesões no Serviço de Cirurgia Plástica e Reparadora do Hospital de Clínicas da Universidade Federal do Paraná. Métodos: Estudo retrospectivo dos pacientes submetidos a tratamento cirúrgico do NCG no período de janeiro de 2004 a janeiro de 2010. Foram coletados dados como: idade, gênero, tratamento realizado, número de cirurgias realizadas, evolução e complicações. Resultados: Foram avaliados 11 pacientes, sendo 8 mulheres e 3 homens. A média de idade foi de 12,4 anos (3 a 25 anos). O subtipo mais encontrado foi o nevus melanocítico intradérmico correspondendo a 90,9% dos casos e 9,1% com nevus melanocítico composto. Não foram identificados casos de melanoma. A localização mais comum foi a região da cabeça e pescoço. O diâmetro médio das lesões foi de 9,1 cm.As técnicas utilizadas para reconstrução foram: sutura primária, retalhos locais, enxerto de pele e uso de expansores. No seguimento, 63,6% dos pacientes apresentavam ainda nevus residual, 27,3% apresentaram ressecção completa e 9,1% perderam seguimento. Conclusão: Foi observada maior incidência de NCG na faixa etária de 3 a 25 anos, sendo a localização mais comum na face e com tamanho médio de 9,1cm. O principal tratamento instituído para os pacientes com NCG foi a ressecção parcelada, com bons resultados. ■ RESUMO

Gestational gigantomastia: how to address this clinical situation

Gestational gigantomastia is a rare disorder characterized by an excessive and rapid enlargement of the breasts, resulting in edema and venous congestion of breast tissue. It is a painful condition that causes skin ulceration and local infection. In some cases, these complications lead to an emergency mastectomy or induced abortion. The most probable etiology is an abnormal stimulation of breast tissue that is probably triggered by an abnormally elevated level of hormones or by the hypersensitivity of breast tissue to normal hormone levels. Although the breast volume decreases after pregnancy, it rarely returns to its original state; therefore, breast reduction through mastectomy or mammaplasty is usually necessary. Moreover, there is a high likelihood of recurrence in subsequent pregnancies. The authors report a case of gigantomastia in a primigravida that required pregnancy termination, because of the risk of maternal death, and a subsequent breast reduction surgery.
