Disease prevalence in geriatric horses in the United Kingdom: Veterinary clinical assessment of 200 cases

REASONS FOR PERFORMING STUDY Geriatric horses (aged ≥ 15 years) represent a substantial proportion of the equine population, yet few studies have investigated the prevalence of diseases within this population in the UK. OBJECTIVES To describe the findings of veterinary clinical examination of 200 geriatric horses and ponies in north west England and north Wales, and to assess the effect of animal age and type (horse or pony) on the presence or absence of clinical signs and veterinary diagnosed health problems. METHODS From responses to a cross-sectional postal questionnaire survey of owners of geriatric horses (n = 918/1144), 200 horses were randomly selected to receive a veterinary clinical examination covering the major body systems. RESULTS Twenty-six percent of animals were overweight (body condition score, BCS, >3/5) and 4.5% were underweight (BCS <2/5). Seventy-one percent had a dermatological abnormality and 22% displayed hirsutism or abnormal moulting. Ophthalmic lesions frequently identified included vitreous degeneration (66.0%), cataracts (58.5%) and senile retinopathy (33.7%). The prevalence of cardiac murmurs was 20%. Whilst only 7.5% of animals had a spontaneous cough during the examination, 18.5% had some form of nasal discharge and 22% had abnormalities on thoracic auscultation at rest. Following rebreathing, 13.6% developed marked abnormalities consistent with lower airway disease. When assessed at walk, 18.6% were lame on at least one limb, while 50.5% were lame in trot. The majority of animals (83.5%) had a reduction in range of motion in at least one joint. Eighty percent of animals had hoof abnormalities. Dental abnormalities were identified in 95.4% of animals, with cheek teeth diastemata, excessive wear/cupped out teeth and focal overgrowths the most frequently identified conditions. CONCLUSIONS AND POTENTIAL RELEVANCE This study has identified many prevalent clinical signs of disease in geriatric horses. Description of the most prevalent health problems and further identification of risk factors for these conditions will aid in targeted improvements in veterinary care, owner education and welfare.

A cross-sectional study of geriatric horses in the United Kingdom. Part 1: Demographics and management practices

REASONS FOR PERFORMING STUDY With growing numbers of aged horses, geriatric medicine is becoming increasingly important in equine veterinary practice; however, there is a paucity of information on the UK equine geriatric population. OBJECTIVES To describe the demographic characteristics of the equine geriatric population and to assess management practices undertaken by owners of geriatric horses (aged≥15 years). METHODS A cross-sectional study was conducted, surveying a randomly selected sample of veterinary registered owners with horses aged≥15 years, using a self-administered postal questionnaire. RESULTS Horses aged≥15 years represented 29% of the equine population in the study area. The response rate to the questionnaire survey was 80.2%. Management practices were similar to those previously described for the general equine population. However, the level of exercise and feeding practices changed significantly with increasing age. A summary of the demographic characteristics, feeding and management of horses in a sample of the geriatric equine population is presented. CONCLUSIONS AND POTENTIAL RELEVANCE The management of geriatric horses has a direct effect on their health and welfare and this study has provided valuable information on the demographics and management of geriatric horses and ponies in the UK. The information can be used to form targeted owner education programmes for geriatric health and should aid in the investigation and prevention of disease.

A cross‐sectional study of geriatric horses in the United Kingdom. Part 2: Health care and disease

REASONS FOR PERFORMING STUDY Geriatric horses (aged≥15 years) represent a substantial proportion of the equine population, yet very few studies have investigated the prevalence of diseases within the UK equine geriatric population. OBJECTIVES To describe the provision of routine preventive health care measures, prevalence of clinical signs of disease and the prevalence of owner reported diseases. Additionally, the effect of increasing age on the provision of preventive health care and the presence or absence of clinical signs and disease was assessed. METHODS A cross-sectional study was conducted, surveying a randomly selected sample of veterinary registered owners with horses aged≥15 years, using a self-administered postal questionnaire. RESULTS As geriatric horses increased in age, there was a reduction in the provision of preventive health care measures, such as vaccination, farrier care and routine veterinary checks. Only 68.7% of horses had received a routine veterinary visit within the previous 12 months. Owners frequently observed clinical signs in their animals, with 77% reporting at least one clinical sign of disease. Increasing age was associated with increased reporting of many clinical signs of disease. Over half (58%) of horses had at least one episode of disease within the previous 12 months, yet only 31% of owners reported that their animal currently suffered from a known disease or disorder. CONCLUSIONS AND POTENTIAL RELEVANCE Although owners frequently observed clinical signs in their aged horse, there may be incorrect or under recognition of many diseases and health problems. Reduced frequency of routine preventive health care measures, along with suboptimal owner recognition of health and welfare problems may lead to compromised welfare in the geriatric population.

Comparison of owner-reported health problems with veterinary assessment of geriatric horses in the United Kingdom

REASONS FOR PERFORMING STUDY Previous studies suggest that owners underestimate or incorrectly recognise or report health problems in geriatric horses. However, few studies have directly compared owner-reported and veterinary assessed disease. OBJECTIVES To compare the findings of veterinary clinical examination of geriatric horses with owner-reported clinical signs and disease. METHODS A total of 200 horses aged ≥ 15 years were randomly selected to receive a veterinary examination, from responses to a cross-sectional postal questionnaire survey. Veterinary examinations were performed within 2 months of questionnaire return, and agreement between owner-reported data and veterinary clinical findings was assessed. RESULTS Owners under-reported many clinical signs and disease conditions detected on veterinary clinical examination. For example, dental abnormalities (detected in 95.4% of horses, reported by 24.5% of owners); cardiac murmurs (detected in 20% of horses, reported by 0.5% of owners); lameness (present in 50% of horses, reported by 23% of owners) and hoof abnormalities (detected in 80% of horses, reported by 27% of owners). Agreement between owner-reported and veterinary assessed respiratory disease (Kappa 0.02-0.2), body condition score (Kappa 0.24) and coat abnormalities (Kappa 0.42) was poor, fair and moderate, respectively. Range of motion (ROM) of the tarsal and metacarpophalangeal joints was lower in horses with owner-reported osteoarthritis (P = 0.005 and <0.001, respectively). CONCLUSIONS AND POTENTIAL RELEVANCE The low prevalence and relatively poor agreement of owner-reported disease compared to that detected on veterinary examination suggests inaccurate or under-recognition, or inaccurate reporting of health problems by owners of geriatric horses, which could lead to a delay in presentation for veterinary treatment. Increased veterinary involvement and improved owner education in the care of geriatric horses should facilitate earlier identification of disease, particularly that which is not readily detectable by owners, and aid management of health and welfare problems.

Preventive health care and owner-reported disease prevalence of horses and ponies in Great Britain

This study aimed to describe the provision of preventive health care and owner-reported disease prevalence in horses and ponies within Great Britain (GB), and to assess geographical variations in health care provision. A cross-sectional survey was conducted, using a postal questionnaire administered to a random sample of veterinary-registered owners of horses and ponies in GB (n=797). The majority of animals received regular preventive health care: 95.6% had regular hoof care; 71.3% were vaccinated for both influenza and tetanus and median time since last anthelmintic administration was 8.7 weeks. Thirty-one percent of owners indicated their animal was overweight/obese. A new health problem within the previous 7 days was reported for 7.4% of animals, 59.3% of which were veterinary-diagnosed. Thirty-two percent of animals were reported to have a long-term/recurrent condition, of which osteoarthritis (13.9%) was the most prevalent. Obesity, musculoskeletal disorders, and dermatological conditions were the most prevalent conditions affecting veterinary-registered horses/ponies.

A survey of health care and disease in geriatric horses aged 30 years or older

This study reports on aspects of preventive health care and on the prevalence of disease in geriatric (≥30 years old) horses and ponies in North-West England and North Wales. Of 128 questionnaires mailed to horse owners, 87 useable responses were received and 69 horses were subject to veterinary examination. Of the animals examined, 16% were underweight with body condition scores (BCS)<2/5, and 10% were overweight (BCS>3/5). This contrasted with the owner responses that reported 23% of the animals surveyed to be underweight and only 3% to be overweight. When assessed at trot, 77% of the horses were lame on at least one limb, with 97% having a reduced range of motion in at least one joint. Only 16% of these animals were reported by owners to have been lame within the previous 12 months. Owner-reported changes to hair coat were significantly associated with hirsutism or abnormal moulting detected at clinical examination (39%). At least one ophthalmic lesion was identified in all of the horses and ponies examined and 17% of owners reported that their animal had experienced visual problems within the previous year. Although dental abnormalities were detected in all animals where an oral examination was performed, only 42% of owners reported that their horse suffered from a known dental disorder. In general, despite a high prevalence of disease detected in these geriatric animals and with the majority having multiple abnormalities at clinical examination, there was under-reporting/under-recognition of these health problems by owners.

Demographics and management practices of horses and ponies in Great Britain: A cross-sectional study

The objectives of this study were to describe the demographic characteristics and management practices undertaken by owners of horses/ponies within GB and assess seasonal and geographical variations in management practices. A cross-sectional study was conducted, surveying a random sample of veterinary-registered owners in GB, using a self-administered postal questionnaire. A total of 797 useable responses were received. Only 4.0% of animals were stabled 24h/day, this proportion being greater in winter compared to other seasons (p<0.001). Shavings (45.7%) and straw (35.3%) were the most frequently used bedding. Ninety-two percent of animals had access to pasture (median 91 h/week). The majority of animals received forage (82.6%) and concentrate/supplementary feeding (86.1%). Retired/companion animals and ponies received supplementary feed less frequently and most animals were used for pleasure riding (60.6%). Seasonal and regional differences in management practices were identified, of relevance to practitioners and researchers in the investigation and management of disease conditions.

Does oral prednisolone treatment increase the incidence of acute laminitis

Reasons for performing study: It is accepted among equine practitioners that glucocorticoid treatment is a risk factor for the development of laminitis. However, there is little published evidence of a link between glucocorticoids and laminitis. Objectives: To determine whether horses receiving oral prednisolone are at increased risk of laminitis. Study design: Retrospective case–control study. Methods: Clinical records of horses registered with the ambulatory service at Liphook Equine Hospital between January 2001 and November 2014 were reviewed retrospectively to identify horses that had received treatment with oral prednisolone. For each treated horse, 2 time‐matched controls that received veterinary attention but were not treated with prednisolone were selected. Incidence of laminitis was compared between the 2 groups and factors associated with laminitis were assessed using Cox regression analysis. Results: Of the 416 horses treated with prednisolone, 16 (3.8%) were diagnosed with laminitis subsequent to the initiation of prednisolone treatment with an overall incidence of 2.60 (95% CI 1.49–4.22) cases per 100 horse‐years at risk. A total of 7 horses (1.7%) developed laminitis during the course of their treatment and 3 (0.7%) of the horses treated with prednisolone were ultimately subjected to euthanasia as a result of laminitis. A total of 46 (5.7%), of the 814 time‐matched control horses were diagnosed with laminitis during the study period with an overall incidence of 3.46 (95% CI 2.54–4.62) cases per 100 horse‐years at risk. Of these, 12 (1.5%) were subjected to euthanasia as a result of laminitis. There were no significant differences in the overall laminitis incidence rate (P = 0.8), incidence rate during prednisolone treatment (P = 0.09), or probability of laminitis (P = 0.3) between the 2 groups. Mean survival time was greater in the prednisolone than the control group. Equine metabolic syndrome and increasing age were associated with increased risk of laminitis. Conclusions: Administration of oral prednisolone did not increase the risk of laminitis.

Freedom from equine infectious anaemia virus infection in Spanish Purebred horses.

Introduction No cases of equine infectious anaemia (EIA) have been reported in Spain since 1983. Factors that could increase the risk of reintroducing equine infectious anaemia virus (EIAV) into Spain include the recent occurrence of the disease in Europe and the absence of compulsory serological testing before importation into Spain. Aims and objectives Given the importance of the Spanish Purebred (SP) horse breeding industry in Spain, the aim of this cross-sectional study was to provide evidence of freedom from EIAV in SP stud farms in Central Spain. Materials and methods Serum samples from 555 SP horses, collected between September 2011 and November 2013, were tested using a commercially available EIAV ELISA with a published sensitivity of 100 per cent. Results All 555 samples were negative for antibody to EIAV, providing evidence of a true EIAV seroprevalence between 0 per cent and 0.53 per cent (95% CIs of the sensitivity and specificity of the ELISA technique used Q10 were 100 per cent and 99.3 per cent, respectively) among the SP breeding population in Central Spain. Conclusions These findings should serve to increase confidence when exporting SP horses to other countries.

Seroprevalence and factors associated with seropositivity to equine arteritis virus in Spanish Purebred horses in Spain

REASONS FOR PERFORMING STUDY Equine viral arteritis (EVA), a disease caused by infection with the equine arteritis virus (EAV), is present in many European countries. In Spain, the last confirmed outbreak was reported in 1992 and there is a paucity of seroprevalence studies. The disease has a major impact on the equine breeding industry, which is mainly represented by Spanish Purebred (SP) horses in Spain. OBJECTIVES To estimate the seroprevalence of EAV in the breeding SP horse population in central Spain and identify potential horse and studfarm level factors associated with seropositivity to EAV. STUDY DESIGN Cross-sectional study. METHODS Individual serum samples from 555 SP horses, collected between September 2011 and November 2013 at 35 studfarms, were tested using a commercially available EAV antibody ELISA and seroneutralisation as the World Organisation for Animal Health reference confirmation test for samples with positive and equivocal results. Data on factors putatively associated with seropositivity to EAV were collected via a questionnaire and examined using random effects logistic regression for analysis of clustered data. RESULTS Equine arteritis virus seroprevalence in the SP breeding population in central Spain standardised for the sex distribution of the reference horse population, was estimated to be 16.8% (95% confidence interval 5.2-28.5%). Increasing numbers of breeding mares on the studfarm and increasing percentage of mares with reproductive problems during the last 12 months were identified as being positively associated with EAV seropositivity. Mares vaccinated against Equine herpesvirus-1 (EHV-1) and/or -4 (EHV-4) were also positively associated with EAV seropositivity. CONCLUSIONS These findings are of importance to ensure appropriate biosecurity measures for studfarms are carried out and may help facilitate the development of an EVA surveillance programme in the SP breeding horse population.
