Out of 2484 patients harboring S. mansoni seen in Rio de Janeiro, 1197 had been living permanently out of endemic area frorn one to 30 years, without any possibility of reinfection; 90.1% of these 1197 patients were first seen with, hepato-intestinal schistosomiasis and only 9.9% with hepatosplenic form. 55% of thern still had S. mansoni active infection 6 years or more after they had left the endemic area and 26.5% remained infected for more than 10 years. The patients with intestinal or hepato-intestinal schistosomiasis did not develop the most severe form whether they had been treated or not, and the hepatosplenic patients had a long time to deteriorate.

Strong correlation was observed between severe cases in 11 to 15 years oid and higher infection rates as shown by the average number of eggs of S. mansoni per gram of faeces.

Of the 203 domestic and wild animais examined in Praia Vermelha, during a study of an outbreak of mucocutaneous leishmaniasis 2 cases of Proechimus dimidiatus were found with hipocromic lesions in the ear extremities and one case of Rattus norvergicus with a back ulcer. Histopathology has shown the presence of Leishmania sp. in the three specimens.

During an outbreak of mucocutaneous leishmaniasis in Praia Vermelha, Ilha Grande (Great Island) in the county of Angra dos Reis, State of Rio de Janeiro, 4.192 species of Phlebotomus were collected in domestic and wild environment with the use of animal baits and other natural shelters from January 1976 to April 1977. In a total of 11 species collected, 2.493 samples of Lutzomyia intermedia, 1.594 of Lutzomyia migonei and 105 samples of other species were found. The Lutzomyia intermedia presented a higher density in human dwellings, while the Lutzomyia migonei presented better results with animal baits and chicken-coops. The sinantropism and high density found in the houses incriminate the species L. intermedia and L. migonei as the probable vectors of Mucocutaneous Leishmaniasis in Ilha Grande.

The authors described for the first time the appearance of an epidemic of Cutaneous Leishmaniasis in the small village of Praia Vermelha, Ilha Grande (Creat Island), county of Angra dos Reis, State of Rio de Janeiro, situated at 17 Km from the continent. 453 persons were examined and 33 cases of Cutaneous Leishmaniasis were found of which 13 were cases of active iesions and 20 of scars. Of the 33 cases, 30 were issued from the area under examination. The disease has existed in Praia Vermelha for more than 10 years and in the Ilha Grande for more than 30 as sporadic cases. In Praia Vermelha between 1974 and 1976, an epidemic occured when 27 cases of the disease were registered. The Cutaneous Leishmaniasis in Praia Vermelha has characteristics of home transmission affecting people without discrimination of sex, age or occupation. The ulcerated form appeared more frequently in the extremities of the body, with spontaneous cure of the lesion and no harm to the mucosa; one hamster in 10 inoculated with the material of the human lesion developped after four months a lesion in its muzzle with positive results for Cutaneous Leishmaniasis.

The authors present the results of a histopathologic study in muscle biosy of 16 cases of leptospirosis. The most important findings were focal necrosis of the muscle fibers, hemorrhagic infiltration in the necrotic focus and interstitial hemorrhages; the inflamatory reaction was very rare or almost inexistent. Besides those findings attention was called to sacoplasmatic vacuolization of muscle fibers preceeding the muscle necrosis. The multinuclear sacoplasmatic strueture seems to be an attempt at muscle regeneration, well succeeded. Finally, the authors consider that muscle alterations, of not generally not specific of leptospirosis, are very characteristic and help to diagnose the disease.

Em inquerito pela intradermorreacao de Montenegro, realizado em 402 pessoas da populacao da Praia Vermelha, Ilha Grande, municipio de Angra dos Reis, RJ, os autores observaram uma prevalencia de 11,94% de positividade; nao ocorreram diferencas estatisticamente significativas entre sexo e faixas etarias. Esse Inquerito ainda revelou que o risco em adquirir a infeccao e de 3 vezes maior na populacao masculina que trabalha na area durante todo o ano, quando comparada a populacao de pescadores que se ausenta da area por longos periodos; tambem revelou um risco 8 vezes maior nos moradores dos domicilios onde ocorreu um caso humano da doenca.
