Limited value of acyclovir in the treatment of uncomplicated herpes zoster: A placebo-controlled study

SummaryThe effect of intravenous acyclovir (at a dosage of 30 mg/kg per day for five days) on uncomplicated herpes zoster was investigated in 51 patients in a double-blind study. Although existing herpes zoster lesions tended to heal more rapidly and new lesions ceased to appear somewhat earlier in the acyclovir group, these differences were not statistically significant. During treatment, patients on acyclovir had significantly lower pain scores than placebo-treated patients. At follow-up, however, there was no difference between the two groups. Complications of herpes zoster occurred only in the placebo groups (generalization in two and keratitis in two cases). With the possible exception of trigeminal zoster or severe pain, acyclovir seems to offer little benefit for immunocompetent patients with herpes zoster.ZusammenfassungDie Wirkung von Acyclovir intravenös (in einer Dosis von 30 mg/kg pro Tag für fünf Tage) auf den unkomplizierten Herpes zoster wurde bei 51 Patienten in einer Doppelblindstudie geprüft. Obwohl bei den vorhandenen Herpes zoster-Läsionen eine Tendenz zu rascherer Heilung bestand und das Neuauftreten von Läsionen in der Acyclovir-Gruppe etwas früher aufhörte, waren diese Unterschiede statistisch nicht signifikant. Unter der Behandlung war die Schmerz-Score bei Patienten, die Acyclovir erhielten, signifikant niedriger als bei Patienten, die Placebo erhielten. Bei der Verlaufsbeurteilung fanden sich zwischen den Gruppen jedoch keine Unterschiede. Zu Komplikationen des Herpes zoster kam es nur in der Plazebo-Gruppe (in je zwei Fällen Generalisation und Keratitis). Mit der möglichen Ausnahme des Zoster im Trigeminus-Bereich oder starker Schmerzen scheint Acyclovir bei immunkompetenten Patienten mit Herpes zoster nur von geringem Wert zu sein.

Acyclovir in severe herpes virus infections

Forty-five patients with severe herpes virus infections were treated with acyclovir intravenously for five days. Nine patients had varicella (eight of whom were immuno- or myelocompromised), 23 had herpes zoster (14 compromised patients) and 13 had herpes simplex (nine compromised patients). No patient died from the viral infection and in eight of the patients the beneficial effect of acyclovir was beyond doubt. Six of these patients had herpes simplex infections. In 21 patients acyclovir was probably beneficial, whereas in 16 patients the effect was doubtful or absent. As expected, in patients with established neurologic damage there was no effect. Except for transient elevation in transaminases in one patient and occasional infusion thrombophlebitis, no toxicity of acyclovir was encountered in this series.

Is Aciclovir Prophylaxis Necessary After Bone Marrow Transplantation

SummaryTo assess the cost-effect relationship of aciclovir prophylaxis versus early treatment, we performed a retrospective study in 44 allogeneic bone marrow transplant recipients, who had only received aciclovir for therapeutic purposes. After bone marrow transplantation 18 herpes simplex infections occurred in 15 of the 33 patients who were seropositive for herpes simplex virus. In ten patients without clinical signs, routine viral cultures yielded herpes simplex virus. Aciclovir was given intravenously to the patients with mucocutaneous herpes infection. All infections responded rapidly. It can be calculated that restricting the drug to therapeutic use reduced the amount of aciclovir used, which in turn diminished the cost of treatment and the risk of aciclovir resistance.ZusammenfassungBei 44 Empfängern von allogenen Knochenmarkstransplantaten, die Aciclovir ausschließlich therapeutisch erhalten hatten, wurde eine retrospektive Studie durchgeführt, um die Kosten-Nutzen-Beziehung für die Prophylaxe im Vergleich zur Frühtherapie zu bestimmen. Bei 15 der 33 Herpes-simplex-Virus-seropositiven Patienten traten nach Knochenmarkstransplantation 18 Herpes-simplex-Virus-Infektionen auf. Bei zehn klinisch symptomfreien Patienten wurde in Routinekulturen Herpes-simplex-Virus nachgewiesen. Patienten mit mukokutaner Herpesinfektion erhielten Aciclovir intravenös appliziert. Alle Infektionen sprachen rasch auf die Therapie an. Es läßt sich errechnen, daß eine Beschränkung auf den therapeutischen Einsatz des Medikamentes den Verbrauch von Aciclovir vermindert und somit die Behandlungskosten und das Risiko der Resistenzentwicklung gegen Aciclovir erniedrigt.
