Approaching countertransference in psychoanalytical supervision: a qualitative investigation.

A recent examination of the literature concerning countertransference and its developments reveals its clinical usefulness in different psychoanalytical cultures. Nevertheless, a shortage of publications is apparent with respect to its approach in supervision. The authors aim in this study was to examine the concepts of transference and countertransference and how countertransference is approached in supervision, in the training of candidates, at an institute of psychoanalysis belonging to a society affiliated to the International Psychoanalytical Association (IPA). Qualitative research was carried out, interviewing supervisors and supervisees. Through analysis of the material acquired, the authors classifi ed the data into initial, intermediate and fi nal categories. The principal fi ndings were subdivided into three categories: the concepts of transference and countertransference, psychoanalytical listening and the complementarity of the phenomena, and the approach to countertransference. The concepts of transference and countertransference predominantly used by those interviewed are based on the totalistic outlook. Countertransference in supervision has been approached in a more direct and objective way when compared with the previous period, although great care is taken to delimit the boundaries between supervision and personal analysis. The main aim of supervision is to broaden comprehension and to deepen the interpretations directed towards the patient. These fi ndings suggest that the evolution of the concept of countertransference in different psychoanalytical cultures and developments in the analytical fi eld are contributing to this change.

A supervisão psicanalítica: revisão e uma proposta de sistematização

A formacao psicanalitica esta alicercada sobre tres pilares basicos: a analise didatica, os seminarios teoricos e a supervisao clinica de analises. O modelo de supervisao psicanalitica, como um todo ou parte dele, e utilizado em diversas areas do ensino da medicina, da psiquiatria e da psicologia, principalmente na psicanalise e na psicoterapia de orientacao analitica, o que motiva um permanente estudo e aperfeicoamento da sua aplicacao pratica. A presente revisao tem como finalidade tentar sistematizar aspectos da supervisao psicanalitica, visando a contribuir para uma area ainda em desenvolvimento, e que conta com crescentes contribuicoes realizadas em nosso meio.

Contratransferência em psicoterapia e psiquiatria hoje

The goal of this article is to demonstrate that even though there are differences between different theoretical schools in the field of psychoanalysis, a narrow convergence area has emerged: the utility of countertransference as a technical element to understand the patient. Countertransference is included in the techniques of psychoanalysis, either with its original denomination (countertransference per se), or with some correlate concept that comprises it, such as projective identification, analytic field, role-responsiveness, enactment, intersubjectivity and analytic third, character and possible histories, etc. It is a concept upon which other concepts are built but its meaning does not concern only to technique. The psychoanalytical theory has changed after such a concept, becoming a double theory, or an attachment theory, about phenomena that occur between patient and therapist, and not only with the patient. In this sense, there is a paradigm change, as the facts are not related to a single individual anymore but to an interaction between two individuals, which is understood only while a product built from both. The psychology of one became the psychology of the attachment between two. The article emphasizes how a technical element can be useful in psychiatry, psychotherapy and psychoanalysis. The use of medication, its acceptance or not, and compliance with treatment or not can be better understood if the psychiatrist uses his or her feelings and makes an effort to understand the attachment between patient and therapist, either he or she is psychotic, borderline or neurotic.Os autores procuram demonstrar que, embora ainda existam diferencas entre as diversas escolas teoricas da psicanalise, uma estreita area de convergencia emergiu relativa a utilidade da contratransferencia como elemento tecnico para compreender o paciente. A contratransferencia esta incluida na tecnica psicanalitica, seja com a denominacao original (contratransferencia propriamente dita), seja em algum conceito correlato que a inclui como identificacao projetiva, campo analitico, role-responsiveness, enactment (encenacao), intersubjetividade e terceiro analitico, personagem e historias possiveis, etc. Tornou-se um conceito a partir do qual outros conceitos se constroem. Seu significado nao se prende apenas a tecnica, no entanto. A teoria psicanalitica muda a partir dele, tornando-se uma teoria da dupla, ou do vinculo, isto e, dos fenomenos que ocorrem no par analitico, e nao mais apenas no paciente. Ha, nesse sentido, uma mudanca do proprio paradigma, na medida em que os fatos agora se referem nao mais a um individuo, mas a uma interacao entre dois individuos, so sendo compreendida enquanto produto daqueles dois. O que era uma psicologia de um torna-se uma psicologia do vinculo que une dois. E enfatizado como este elemento da tecnica pode ser util em psiquiatria, psicoterapia e psicanalise. A utilizacao da medicacao, sua aceitacao ou nao e a aderencia ao tratamento podem ser compreendidas de forma mais completa se o psiquiatra usar de seus sentimentos e buscar entender o vinculo que se estabeleceu entre ele e seu paciente, seja ele psicotico, borderline ou neurotico. Os autores concluem que a contratransferencia nao se refere apenas aos sentimentos do terapeuta na sessao, mas significa a utilizacao, de forma ampla, da subjetividade do proprio analista/terapeuta/clinico para a compreensao mais ampla e profunda do seu paciente, de um modo mais completo, por abarcar nao somente fenomenos visiveis a superficie, mas, principalmente, por incluir sentimentos e significados que jazem no âmago inconsciente de cada individuo, ocultos, obscuros, mas determinantes e definidores de seu comportamento.

The impact of intrusive identification in the analytic process: some implications of real trauma and phantasy.

In this paper, the author aims to show a possible understanding of very primitive identifications, especially intrusive identifications, when associated with traumatic situations and expressed through current phantasies related to these experiences. At first glance, this understanding could be considered quite straightforward. However, the original contribution offered by the author is the participation of this special kind of combination in the formation of the primitive intrusive identifications and its association with the imprisonment inside the primitive object of identification, especially the mother. The author proposes the amplification of the clinical use of the concept of ‘life in the claustrum’, originally described by Meltzer, moving beyond persecutory claustrophobic situations. He illustrates the phenomenon with the analytic work carried out with a patient whose narcissistic and intrusive character was structured on the basis of primitive intrusive identifications and phantasies related to the claustrum inside the mother. The patients imprisonment inside the maternal compartments has, as its background, the phantasies related to the infantile traumatic experience of the death of the patients brother, which are reproduced in the analytic relationship.
