Effects of verb meaning on lexical integration in agrammatic aphasia: Evidence from eyetracking

Relatively little is known about the time course of access to the lexical representations of verbs in agrammatic aphasia and its effects on the prediction and integration of the verbs arguments. The present study used visual-world eyetracking to test whether verb meaning can be used by agrammatic aphasic individuals to predict and facilitate the integration of a subsequent noun argument. Nine adults with agrammatic aphasia and ten age-matched controls participated in the study. In Experiment 1, participants viewed arrays of four objects (e.g., jar, plate, stick, pencil) while listening to sentences containing either a restrictive verb that was semantically compatible only with the target object or an unrestrictive verb compatible with all four objects (e.g., Susan will open/break the jar). For both participant groups, the restrictive condition elicited more fixations to the target object immediately after the verb. Experiment 2 differed from Experiment 1 in that the auditory sentences presented were incomplete (e.g., Susan will open/break the…). For controls, restrictive verbs elicited more target fixations immediately after the verb; however, the effects of verb type were noted downstream from the verb for the aphasic listeners. The results suggest that individuals with agrammatic aphasia have preserved ability to use verb information to facilitate integration of overt arguments, but prediction of upcoming arguments is impaired. Impaired lexical-semantic prediction processes may be caused by damage to the left inferior frontal gyrus, which has been argued to support higher-level lexical processes.

Grammatical impairments in PPA

Background: Grammatical impairments are commonly observed in the agrammatic subtype of primary progressive aphasia (PPA-G), whereas grammatical processing is relatively preserved in logopenic (PPA-L) and semantic (PPA-S) subtypes. Aims: We review research on grammatical deficits in PPA and associated neural mechanisms, with discussion focused on production and comprehension of four aspects of morphosyntactic structure: grammatical morphology, functional categories, verbs and verb argument structure, and complex syntactic structures. We also address assessment of grammatical deficits in PPA, with emphasis on behavioural tests of grammatical processing. Finally, we address research examining the effects of treatment for progressive grammatical impairments. Main Contribution: PPA-G is associated with grammatical deficits that are evident across linguistic domains in both production and comprehension. PPA-G is associated with damage to regions including the left inferior frontal gyrus and dorsal white matter tracts, which have been linked to impaired comprehension and production of complex sentences. Detailing grammatical deficits in PPA is important for estimating the trajectory of language decline and associated neuropathology. We, therefore, highlight several new assessment tools for examining different aspects of morphosyntactic processing in PPA. Conclusions: Individuals with PPA-G present with agrammatic deficit patterns distinct from those associated with PPA-L and PPA-S, but similar to those seen in agrammatism resulting from stroke, and patterns of cortical atrophy and white matter changes associated with PPA-G have been identified. Methods for clinical evaluation of agrammatism, focusing on comprehension and production of grammatical morphology, functional categories, verbs and verb argument structure, and complex syntactic structures are recommended and tools for this are emerging in the literature. Further research is needed to investigate the real-time processes underlying grammatical impairments in PPA, as well as the structural and functional neural correlates of grammatical impairments across linguistic domains. Few studies have examined the effects of treatment for grammatical impairments in PPA; research in this area is needed to better understand how (or if) grammatical processing ability can be improved, the potential for spared neural tissue to be recruited to support this and whether the neural connections within areas of dysfunctional tissue required for grammatical processing can be enhanced using cortical stimulation.

Neural Correlates of Processing Passive Sentences

Previous research has shown that comprehension of complex sentences involving wh-movement (e.g., object-relative clauses) elicits activation in the left inferior frontal gyrus (IFG) and left posterior temporal cortex. However, relatively little is known about the neural correlates of processing passive sentences, which differ from other complex sentences in terms of representation (i.e., noun phrase (NP)-movement) and processing (i.e., the time course of syntactic reanalysis). In the present study, 27 adults (14 younger and 13 older) listened to passive and active sentences and performed a sentence-picture verification task using functional Magnetic Resonance Imaging (fMRI). Passive sentences, relative to active sentences, elicited greater activation in bilateral IFG and left temporo-occipital regions. Participant age did not significantly affect patterns of activation. Consistent with previous research, activation in left temporo-occipital cortex likely reflects thematic reanalysis processes, whereas, activation in the left IFG supports processing of complex syntax (i.e., NP-movement). Right IFG activation may reflect syntactic reanalysis processing demands associated with the sentence-picture verification task.

What do pauses in narrative production reveal about the nature of word retrieval deficits in PPA

Naming and word-retrieval deficits, which are common characteristics of primary progressive aphasia (PPA), differentially affect production across word classes (e.g., nouns, verbs) in some patients. Individuals with the agrammatic variant (PPA-G) often show greater difficulty producing verbs whereas those with the semantic variant (PPA-S) show greater noun deficits and those with logopenic PPA (PPA-L) evince no clear-cut differences in production of the two word classes. To determine the source of these production patterns, the present study examined word-finding pauses as conditioned by lexical variables (i.e., word class, frequency, length) in narrative speech samples of individuals with PPA-S (n=12), PPA-G (n=12), PPA-L (n=11), and cognitively healthy controls (n=12). We also examined the relation between pause distribution and cortical atrophy (i.e., cortical thickness) in nine left hemisphere regions of interest (ROIs) linked to word production. Results showed higher overall pause rates for PPA compared to unimpaired controls; however, greater naming severity was not associated with increased pause rate. Across all groups, more pauses were produced before lower vs. higher frequency words, with no independent effects of word length after controlling for frequency. With regard to word class, the PPA-L group showed a higher rate of pauses prior to production of nouns compared to verbs, consistent with noun-retrieval deficits arising at the lemma level of word production. Those with PPA-G and PPA-S, like controls, produced similar pause rates across word classes; however, lexical simplification (i.e., production of higher-frequency and/or shorter words) was evident in the more-impaired word class: nouns for PPA-S and verbs for PPA-G. These patterns are consistent with conceptual and/or lemma-level impairments for PPA-S, predominantly affecting objects/nouns, and a lemma-level verb-retrieval deficit for PPA-G, with a concomitant impairment in phonological encoding and articulation affecting overall pause rates. The greater tendency to pause before nouns was correlated with atrophy in the left precentral gyrus, inferior frontal gyrus and inferior parietal lobule, whereas the greater tendency to pause before less frequent and longer words was associated with atrophy in left precentral and inferior parietal regions.

How the brain processes different dimensions of argument structure complexity: Evidence from fMRI

Verbs are central to sentence processing, as they encode argument structure (AS) information, i.e., information about the syntax and interpretation of the phrases accompanying them. The behavioral and neural correlates of AS processing have primarily been investigated in sentence-level tasks, requiring both verb processing and verb-argument integration. In the current functional magnetic resonance imaging (fMRI) study, we investigated AS processing using a lexical decision task requiring only verb processing. We examined three aspects of AS complexity: number of thematic roles, number of thematic options, and mapping (non)canonicity (unaccusative vs. unergative and transitive verbs). Increased number of thematic roles elicited greater activation in the left posterior perisylvian regions claimed to support access to stored AS representations. However, the number of thematic options had no neural effects. Further, unaccusative verbs elicited longer response times and increased activation in the left inferior frontal gyrus, reflecting the processing cost of unaccusative verbs and, more generally, supporting the role of the IFG in noncanonical argument mapping.

Phonological facilitation of object naming in agrammatic and logopenic primary progressive aphasia (PPA)

Phonological processing deficits are characteristic of both the agrammatic and logopenic subtypes of primary progressive aphasia (PPA-G and PPA-L). However, it is an open question which substages of phonological processing (i.e., phonological word form retrieval, phonological encoding) are impaired in these subtypes of PPA, as well as how phonological processing deficits contribute to anomia. In the present study, participants with PPA-G (n = 7), participants with PPA-L (n = 7), and unimpaired controls (n = 17) named objects as interfering written words (phonologically related/unrelated) were presented at different stimulus onset asynchronies (SOAs) of 0, +100, +300, and +500 ms. Phonological facilitation (PF) effects (faster naming times with phonologically related interfering words) were found for the controls and PPA-L group only at SOA = 0 and +100 ms. However, the PPA-G group exhibited protracted PF effects (PF at SOA = 0, +100, and +300 ms). These results may reflect deficits in phonological encoding in PPA-G, but not in PPA-L, supporting the neuropsychological reality of this substage of phonological processing and the distinction between these two PPA subtypes.

Using statistical parsing to detect agrammatic aphasia

Agrammatic aphasia is a serious language impairment which can occur after a stroke or traumatic brain injury. We present an automatic method for analyzing aphasic speech using surface level parse features and context-free grammar production rules. Examining these features individually, we show that we can uncover many of the same characteristics of agrammatic language that have been reported in studies using manual analysis. When taken together, these parse features can be used to train a classifier to accurately predict whether or not an individual has aphasia. Furthermore, we find that the parse features can lead to higher classification accuracies than traditional measures of syntactic complexity. Finally, we find that a minimal amount of pre-processing can lead to better results than using either the raw data or highly processed data.

Tracking sentence comprehension: Test-retest reliability in people with aphasia and unimpaired adults

PURPOSE Visual-world eyetracking is increasingly used to investigate online language processing in normal and language impaired listeners. Tracking changes in eye movements over time also may be useful for indexing language recovery in those with language impairments. Therefore, it is critical to determine the test-retest reliability of results obtained using this method. METHODS Unimpaired young adults and people with aphasia took part in two eyetracking sessions spaced about one week apart. In each session, participants completed a sentence-picture matching task in which they listened to active and passive sentences (e.g., The [N1+Auxwoman was] [Vvisiting/visited] [NP/PP2(by) the man]) and selected between two pictures with reversed thematic roles. We used intraclass correlations (ICCs) to examine the test-retest reliability of response measures (accuracy, reaction time (RT)) and online eye movements (i.e., the likelihood of fixating the target picture in each region of the sentence) in each participant group. RESULTS In the unimpaired adults, accuracy was at ceiling (thus ICCs were not computed), with moderate ICCs for RT (i.e., 0.4 - 0.58) for passive sentences and low (<0.4) for actives. In individuals with aphasia, test-retest reliability was strong (0.590.75) for RT for both sentence types. Similarly, for the unimpaired listeners, reliability of eye movements was moderate for passive sentences (NP/PP2 region) and low in all regions for active sentences. But, for the aphasic participant group, eye movement reliability was excellent for passive sentences (in the first second after sentence end) and strong for active sentences (V and NP/PP2 regions). CONCLUSION Results indicated moderate-to-low reliability for unimpaired listeners; however, reliable eye movement patterns were detected for processes specific to passive sentences (e.g., thematic reanalysis). In contrast, individuals with aphasia exhibited strong and stable performance across sentence types in response measures and online eye movements. These findings indicate that visual-world eyetracking provides a reliable measure of online sentence comprehension, and thus may be useful for investigating sentence processing changes over time.

Recovery of Online Sentence Processing in Aphasia: Eye Movement Changes Resulting From Treatment of Underlying Forms

Purpose The present study tested whether (and how) language treatment changed online sentence processing in individuals with aphasia. Method Participants with aphasia (n = 10) received a 12-week program of Treatment of Underlying Forms (Thompson & Shapiro, 2005) focused on production and comprehension of passive sentences. Before and after treatment, participants performed a sentence-picture matching task with active and passive sentences as eye movements were tracked. Twelve age-matched controls also performed the task once each. Results In the age-matched group, eye movements indicated agent-first predictive processing after hearing the subject noun, followed by rapid thematic reanalysis after hearing the verb form. Pretreatment eye movements in the participants with aphasia showed no predictive agent-first processing, and more accurate thematic analysis in active compared to passive sentences. After treatment, which resulted in improved offline passive sentence production and comprehension, participants were more likely to respond correctly when they made agent-first eye movements early in the sentence, showed equally reliable thematic analysis in active and passive sentences, and were less likely to use a spatially based alternative response strategy. Conclusions These findings suggest that treatment focused on improving sentence production and comprehension supports the emergence of more normal-like sentence comprehension processes.

Recovery of Sentence Production Processes Following Language Treatment in Aphasia: Evidence from Eyetracking

Introduction: Sentence production impairments in aphasia often improve with treatment. However, little is known about how cognitive processes supporting sentence production, such as sentence planning, are impacted by treatment. Methods: The present study used eyetracking to examine changes in sentence production resulting from a 12-week language treatment program focused on passive sentences (Treatment of Underlying Forms (TUF); Thompson and Shapiro, 2005). In two pre-treatment and two post-treatment sessions, nine participants with mild-to-moderate agrammatic aphasia performed a structural priming task, which involved repeating primed sentences (actives or passives) and then, using the same verb, producing sentences describing pictured events. Two individuals with aphasia performed the eyetracking task on the same schedule without intervening language treatment. Ten unimpaired older adults also performed the task to identify normal performance patterns. Sentence production accuracy and speech onset latencies were examined, and eye movements to the pictured Agent and Theme characters were analyzed in the first 400 ms after picture onset, reflecting early sentence planning, and in the regions preceding the production of the sentence subject and post-verbal noun, reflecting lexical encoding. Results: Unimpaired controls performed with high accuracy. Their early eye movements (first 400 ms) indicated equal fixations to the Agent and Theme, consistent with structural sentence planning (i.e., initial construction of an abstract structural frame). Subsequent eye movements occurring prior to speech onset were consistent with encoding of the correct sentence subject (i.e., the Agent in actives, Theme in passives), with encoding of the post-verbal noun beginning at speech onset. In participants with aphasia, accuracy improved significantly with treatment, and post-treatment (but not pre-treatment) eye movements were qualitatively similar to those of unimpaired controls, indicating correct encoding of the Agent and Theme nouns for both active and passive sentences. Analysis of early eye movements also showed a treatment-induced increase in structural planning. No changes in sentence production accuracy or eye movements were found in the aphasic participants who did not receive treatment. Conclusion: These findings indicate that treatment improves sentence production and results in the emergence of normal-like cognitive processes associated with successful sentence production, including structural planning.
