This report details the experience with nine patients over a 3-year period who had partial or total nasal reconstruction using an expanded forehead flap. The history of nasal reconstruction is reviewed, emphasizing the evolution of the forehead flap as the ideal donor site. The authors experience with skin expansion of the forehead to produce a thin ideal flap is presented in detail. Complications of the procedure are reviewed. Technical considerations to achieve a good result are emphasized. The forehead donor site is minimal and well accepted. This procedure provides a solution to a major problem with partial and total nasal reconstruction.

Six cases of congental deformity of the extensor tendons of the fingers and thumb associated with soft-tissue flexion contractures are presented. The severity of the deformity varied from involvement of a single thumb to involvement of all ten digits. Various procedures to correct this deformity have been described. The procedure used for correcting the deformities in three of the cases in this series is described in detail. It consists in transfer of combinations of the superficial flexor tendons of the long, ring, and little fingers to the central slips of the extensor hood at the base of the proximal phalanges of the involved digits after release of the soft-tissue contractures.

An adjunctive technique for lower lid blepharoplasty is presented. This operative procedure uses the principle of anchoring the upper margin of the lower lid by suturing a triangular muscle flap from it to the lateral-superior part of the orbital rim. This more effective support for the lower lid margin permits one to excise more redundant tissue without getting an ectropion.

A case report of a metastasizing basal cell carcinoma is presented, primary in the leg with regional inguinal lymph node metastasis, followed by extensive metastasis to skin and bone marrow with myelophthisic anemia and terminal splenohepatic and unilateral adrenal metastasis.
