We developed a high-speed, swept source OCT system for widefield OCT angiography (OCTA) imaging. The system has an extended axial imaging range of 6.6 mm. An electrical lens is used for fast, automatic focusing. The recently developed split-spectrum amplitude and phase-gradient angiography allow high-resolution OCTA imaging with only two B-scan repetitions. An improved post-processing algorithm effectively removed trigger jitter artifacts and reduced noise in the flow signal. We demonstrated high contrast 3 mm×3 mm OCTA image with 400×400 pixels acquired in 3 seconds and high-definition 8 mm×6 mm and 12 mm×6 mm OCTA images with 850×400 pixels obtained in 4 seconds. A widefield 8 mm×11 mm OCTA image is produced by montaging two 8 mm×6 mm scans. An ultra-widefield (with a maximum of 22 mm along both vertical and horizontal directions) capillary-resolution OCTA image is obtained by montaging six 12 mm×6 mm scans.

We developed a handheld optical coherence tomography angiography (OCTA) system using a 100-kHz swept-source laser. The handheld probe weighs 0.4 kg and measures 20.6 × 12.8 × 4.6 cm3. The system has dedicated features for handheld operation. The probe is equipped with a mini iris camera for easy alignment. Real-time display of the en face OCT and cross-sectional OCT images in the system allows accurately locating the imaging target. Fast automatic focusing was achieved by an electrically tunable lens controlled by a golden-section search algorithm. An extended axial imaging range of 6 mm allows easy alignment. A registration algorithm using cross-correlation to register adjacent OCT B-frames with propagation from the central frame was used to effectively minimize motion artifacts in volumetric OCTA images captured in relatively short durations of 1 and 2.1 seconds. 2.5 × 2.5 mm (200 × 200 pixels) and 3.5 × 3.5 mm (300 × 300 pixels) retinal angiograms were demonstrated on two awake adult human subjects without the use of any mydriatic eye drops.

The hematocrit dependence of flow signal (split-spectrum amplitude decorrelation angiography-SSADA decorrelation value) was investigated in this paper. Based on the normalized field temporal correlation function and concentration dependent particle scattering properties, the relationship between hematocrit and flow signal was analytically derived. Experimental verification of the relationship was performed with custom-designed microfluidic chips and human blood with 45%, 40% and 32% hematocrit. It was found that, in large flow channels and blood vessels, the normal hematocrit is near the decorrelation saturation point and therefore a change in hematocrit has little effect on the SSADA decorrelation value (flow signal). However, in narrow channels in the capillary size range, the effective hematocrit (adjusted for the overlap between OCT beam and channel) is in the range of 6.7-9.5% and therefore variation in hematocrit does significantly affect the flow signal.

Conjunctivochalasis is a common cause of tear dysfunction due to the conjunctiva becoming loose and wrinkly with age. The current solutions to this disease include either surgical excision in the operating room, or thermoreduction of the loose tissue with hot wire in the clinic. We developed a near-infrared laser thermal conjunctivoplasty system. The system utilizes a 1460-nm programmable laser diode system as the light source. At this wavelength, a water absorption peak exists and the blood absorption is minimal, so the heating of redundant conjunctiva is even and there is no bleeding. A miniaturized handheld probe delivers the laser light and reshapes the laser into a 10 × 1 mm2 line on the working plane. A foot pedal is used to deliver a preset number of calibrated laser pulses. A fold of loose conjunctiva is grasped by a pair of forceps. The NIR laser light is delivered through an optical fiber and a laser line is aimed exactly on the conjunctival fold by a cylindrical lens. Ex vivo experiments using porcine eye was performed to investigate the induced shrinkage of conjunctiva and decide the optimal laser parameters. It was found that up to 45% of conjunctiva shrinkage could be achieved.

Incontinentia pigmenti (IP) is a rare X-linked dominant disorder that can cause retinal nonperfusion, neovascularization, and retinal detachment. Evaluation of the peripheral retinal vasculature and appropriate treatment can reduce the risk of blindness. The authors report the use of a handheld prototype optical coherence tomography angiography (OCTA) and ultra-widefield OCT (UWF-OCT) during exam under anesthesia of a 2-year-old with a history of severe early onset IP. UWF-OCT and OCTA may be used as noninvasive imaging modalities for IP and similar retinal vascular disorders in supine young children. [Ophthalmic Surg Lasers Imaging Retina. 2018;49:273-275.].

Using a commercial available 200K swept source laser, we demonstrated high resolution wide field angiographic imaging of human retinal. 8mm by 8mm and 10mm by 6mm retina areas were imaged in a single scan within 4 seconds. By montaging four 10 x 6mm scan, 10 x 20mm wide field OCT angiography images were demonstrated.
