Efficacy and safety of antiplatelet-combination therapy after drug-eluting stent implantation

Background/Aims Combination single-pill therapy can improve cost-effectiveness in a typical medical therapy. However, there is a little evidence about the efficacy and tolerability of combination single-pill antiplatelet therapy after percutaneous coronary intervention (PCI) with drug-eluting stents (DES). Methods From June to November 2012, in total, 142 patients who met the following criteria were enrolled: at least 18 years old; successful PCI with DES at least 3 months earlier; and regular medication of aspirin and clopidogrel with no side effects. After VerifyNow P2Y12 and aspirin assays, the combination single pill of aspirin and clopidogrel was given and laboratory tests were repeated 6 weeks later. Results At baseline, the incidence of aspirin resistance, defined as aspirin reaction unit (ARU) ≥ 550, was 9.2%, that of clopidogrel resistance, defined as P2Y12 reaction unit (PRU) ≥ 230, was 46.5%, and that of percent inhibition of PRU < 20% was 32.4%. At follow-up, the incidence of resistance by ARU value was 7.0%, 50.0% by PRU value, and 35.9% by percentage inhibition of PRU, respectively. The mean values of ARU (431.5 ± 63.6 vs. 439.8 ± 55.2; p = 0.216) and PRU (227.5 ± 71.4 vs. 223.3 ± 76.0; p = 0.350) were not significantly different before versus after antiplatelet-combination single-pill therapy. Five adverse events (3.5%) were observed during the study period. Conclusions Combination single-pill antiplatelet therapy, which may reduce daily pill burden for patients after PCI with DES, demonstrated similar efficacy to separate dual-pill antiplatelet therapy.

SU‐FF‐T‐476: Dose‐Sparing Effects of a Rectal Balloon in Proton Therapy of the Prostate

Purpose; The normal organ (bladder, rectum) sparing is very important for radiation therapy of the pelivis. Many people are studying with the effort for rectal and bladder spearing, however the actual circumstances to are satisfied which can not find the method. In this study, the decrease used the balloon to order the rectal dose. The aim of the study was to investigate the dose volume effects of a water‐filled rectal balloon in the rectum. Materials and Methods; We inserted the balloon(100cc water filled, 10cm length, 3cm diameter) in rectum for 10 proton therapy patients with prostate cancers. The systems were loaded with Eclipse proton planning system(Ver. 7.5) and Two sets of CTimages before and after ballooning were acquired. Rectum and bladder volumes were delineated on 3‐mm‐thick CTimages. A comparison between dose volume histogram(DVH) with and without balloon in rectum for proton therapy of the prostate cancers.Results; More organ and target volume changes with shape because inserted the balloon in rectum. Mean dose of rectal difference between with and without balloon was about 20%, because volume of rectum escape from a radiation area due to the balloon. Mean dose of bladder difference between with and without balloon was about 6%. Conclusion; The rectal ballon is very useful for bladder and rectal sparing with proton therapy of the prostate cancers. This research is believed to be sufficiently accurate and clinically useful.

MPS 02-02 THE SALT-TASTE THRESHOLD MIGHT NOT BE ASSOCIATED WITH THE BLOOD PRESSURE

Objective: Salt-taste threshold can influence salt appetite, and is thought to be another marker of sodium intake. Many studies mentioned the relationship between sodium intake and blood pressure. The aim of this study was to evaluate the relationship the salt-taste threshold and urinary sodium excretion between normotension and hypertension. Design and Method: We enrolled 172 patients (mean age 52 yrs, male 85 (49.4 %)), who was visited our clinic for further evaluation of high blood pressure. Hypertension was diagnosed as day time systolic/diastolic BP > 135/85mmHg by ambulatory BP monitoring. We compared the salt taste threshold assessed by graded solutions of saline and 24-hour urinary sodium excretion. Results: The salt-taste threshold was not significant between normotensive (51 patients (29.7%), mean age 53 years) and hypertensive patients (121 patients (70.3%), mean age 51 years)(6.6 ± 1.1 vs. 6.7 ± 1.2, respectively). The 24-hour urinary sodium excretion of hypertensive patients was significant higher than normontensive patients (143.8 ± 63.8 mg vs 120.0 ± 61.4 mg, respectively, P < 0.05). Also, the ration of urinary sodium-potassium was significantly higher in the hypertensive patients. However, there is no relationship between the salt-taste threshold and 24-hour urinary sodium excretion in the both groups. Conclusions: The salt-taste threshold might be not related to blood pressure status as well as urinary sodium excretion.

PS 05-76 RELATIONSHIP BETWEEN CENTRAL BLOOD PRESSURE AND TARGET ORGAN DAMAGE IN FIRST DIAGNOSED HYPERTENSION PATIENTS

Purpose: Central pressures are pathophysiologically more relevant than peripheral pressures for the pathogenesis of cardiovascular disease. In CAFE study, central pressure independently predicts outcome in treated patients with hypertension. The aim of this study was to evaluate relationship central blood pressure (BP) and target organ damage in first diagnosed hypertension patients. Methods: We enrolled 163 patients (mean age 48.4 ± 13.1 yrs, male 88 (54.0 %)), who was recently diagnosed as hypertension. Hypertension was diagnosed as day time systolic/diastolic BP > 135/85mmHg by ambulatory BP monitoring. We compared central BP, estimated glomerular filtration rate (GFR), carotid-femoral PWV(cfPWV), left ventricular mass index and left atrial volume index (LAVI). Results: The mean 24hr average systolic BP for day time and central systolic BP were 145.1 ± 14.0 and 139.6 ± 17.7 mmHg, respectively. Central systolic BP was significantly correlated to age (r = 0.295, p = 0.018), estimated GFR (r = -0.304, p = 0.025), cfPWV (r = 0.434, p < 0.0001) and LAVI (r = 0.354, p = 0.017). In multivariate regression analysis, the central BP was independently correlated to estimated GFR and LAVI after adjustment of age and PWV. Conclusions: Central BP might be associated to target organ damage in hypertension patients. However, further larger study might be needed.
