Benign subdural collections of infancy.

During the past two years we studied six infants with subdural collections of fluid. All patients had macrocrania and excessive transillumination of the head. Rapid head growth was common but five patients were otherwise asymptomatic. Subdural taps performed on five children confirmed the presence of abnormal fluid over the cerebral convexities. Subdural fluid in four patients was compatible with effusion and in another with a hematoma. Computerized tomographic evaluation of all infants showed ventricular enlargement, wide cerebral sulci, decreased density in the anterior temporal regions, large sylvian cisterns, prominent interhemispheric fissures, and decreased density over the cerebral convexities. The CT findings resembled cerebral atrophy but psychomotor development and neurologic examinations have been norma.

Clivus epidural hematoma: a case report.

An acute traumatic epidural hematoma extending from the odontoid process to the dorsum sella is described. The mechanism for the formation of an extradural hematoma in this unusual location seems to be related to age and a severe hyperflexion injury.

BILATERAL HOMONYMOUS HEMIANOPIA

Bilateral homonymous hemianopia occurred in 15 patients seen during a four-year period. Persons with this visual deficit had similarly shaped visual field defects on corresponding sides of the vertical midline for each eye, equal visual acuity which is generally normal, and normal pupil and fundus examinations. The most common causes of the bilateral posterior cerebral artery insufficency were arteriosclerosis (40%), uncalherniation (20%), and migraine (13%).

During the past 2 years six infants were seen at University of Wisconsin Hospitals (UWH) with subdural hematomas (effusions). This communication describes the abnormalities found on computerized tomographic (CT) evaluation of the six infants.

Evaluation of extracranial occlusive disease by digital subtraction angiography

Abstract The intravenous injection of contrast medium for angiography is a relatively new means of studying the arteries of the head and neck. Digital subtraction angiography allows the evaluation of patients with suspected vascular disease without the risk of arterial catheterization.

Intraarterial Digital Subtraction Angiography

Using digital electronic techniques it is possible to process the video signal from a conventional image intensified fluoroscopic system so as to allow isolation and amplification of a small iodine signal produced after the intravascular injection of contrast medium. Such a device has been in use in our department for the last two years, primarily for performance of intravenous arteriography. More recently, we have been using the apparatus to perform arteriography after injection of the iodinated contrast medium directly into the arterial system. In this report the technique, advantages and limitations of such an approach will be discussed.

Doppler Ultrasound Evaluation of Extracranial Cerebrovascular Disease

Carotid Doppler examination was compared with arteriographic findings in 125 patients. The results demonstrated exact correlation in 87% of vessels studied. Clinically significant errors occurred at a rate of 4.5%. Clinically insignificant errors (8.5%) were usually the result of wall disease being mistaken for nonhemo-dynamically significant stenosis.
